Characteristics of Cervical Spine Injury in Pediatric Patients With Facial Fractures.

OVERVIEW: Cervical spine injury may present with pediatric patients having sustained fractures of the craniofacial skeleton. Management considerations of the cervical spine often take priority to the fractures of the facial skeleton. The goal of this study was to examine this subset of patients with a focus on initial presentation and need for intervention. METHODS: A retrospective review from 2000 to 2012 of all facial fractures in patients ≤ 18 years at a level 1 trauma center was performed. Patient demographics, location of fractures, and the presence of a cervical spine injury were collec

Significance of Cervical Spine Computed Tomography Hounsfield Units to Predict Bone Mineral Density and the Subsidence After Anterior Cervical Discectomy and Fusion.

STUDY DESIGN: This study was a retrospective review. OBJECTIVE: The purpose of this study was to investigate the correlation analysis between Hounsfield units (HU) and dual x-ray absorptiometry (DXA) based on the clinical results of patients who underwent anterior cervical discectomy and fusion (ACDF) surgery. SUMMARY OF BACKGROUND DATA: There is no technique to directly measure bone mineral density (BMD) in the cervical spine. As computed tomography is a very popular preoperative planning modality, using the HU value from that analysis to predict osteoporosis is important for patient outcom

Recurrent atlantoaxial rotatory fixation in children: a rare complication of a rare condition. Report of four cases.

Atlantoaxial rotatory fixation (AARF) is an uncommon condition of childhood. Occasionally AARF may recur. The authors describe the cases of four patients with recurrent AARF (RAARF). The probable cause of the RAARF and operative procedure selected are discussed. In three cases, attempts were made to stabilize the atlantoaxial complex rather than to perform fusion to preserve the function of the joint complex. Joint stabilization is performed by incorporating a "check ligament" into the joint construct between the axial spinous process and the atlantal lateral mass. The authors believe this tec

Congenital fusion C0-C2 associated with spondylolysis of C2.

This is the report of a very rare malformation at the cranio-cervical junction, including congenital occipito-cervical fusion C0-C2, spondylolysis of C2, and hypoplasia of the right vertebral artery. The malformation itself is part of a Klippel-Feil syndrome with sensory-neural hearing loss on the left side, congenital high thoracic scoliosis, rib agenesis on the right side, and kidney malformations on both sides.

Effect of age-adjusted alignment goals and distal inclination angle on the fate of distal junctional kyphosis in cervical deformity surgery.

BACKGROUND: Age-adjusted alignment targets in the context of distal junctional kyphosis (DJK) development have yet to be investigated. Our aim was to assess age-adjusted alignment targets, reciprocal changes, and role of lowest instrumented level orientation in DJK development in cervical deformity (CD) patients. METHODS: CD patients were evaluated based on lowest fused level: cervical (C7 or above), upper thoracic (UT: T1-T6), and lower thoracic (LT: T7-T12). Age-adjusted alignment targets were calculated using published formulas for sagittal vertical axis (SVA), pelvic incidence-lumbar lord

Craniocervical hyperpneumatization with concurrent pneumorrhachis, pneumomediastinum, and subcutaneous emphysema in a weightlifter.

BACKGROUND CONTEXT: Acquired hyperpneumatization of the skull base and upper cervical vertebrae is extremely rare and is thought to occur in patients who habitually perform the Valsalva maneuver or engage in repetitive positive pressure activities such as scuba diving or free diving. Craniocervical hyperpneumatization has been reported to cause intracranial and extracranial pneumatoceles but is not generally considered as a cause of pneumorrhachis (air in the spinal canal). Pneumorrhachis is relatively rare, and usually occurs in a localized form, either in the cervical spine secondary to skul

Stabilization of the atlantoaxial joint with C1-C3 lateral mass screw constructs: biomechanical comparison with standard technique.

BACKGROUND: Anatomically and biomechanically, the atlantoaxial joint is unique compared with the remainder of the cervical spine. OBJECTIVE: To assess the in vitro stability provided by 2 C2 screw sparing techniques in a destabilized model of the atlantoaxial joint and compare with the gold standard system. METHODS: The 3-dimensional intervertebral motion of 7 human cadaveric cervical spine specimens was recorded stereophotogrammetrically while applying nonconstraining, nondestructive pure moments during flexion-extension, left and right axial rotation, and left and right lateral bending. Atlantoaxial instability in a white-tailed deer fawn (Odocoileus virginianus).

On 14 March 2001, an 8 mo old, male white-tailed deer (Odocoileus virginianus) was found in lateral recumbency exhibiting neurologic signs including inability to rise, opisthotonus, paddling, and respiratory distress. There was evidence of minor cranial trauma. Postmortem examination revealed atlantoaxial instability with ventral deviation of the axis due to malformation of the caudal atlas and cranial axis. Given the age of the fawn, the instability was assumed to be congenital with minor trauma inducing severe, acute neurologic signs.

Transoral exposure of the atlantoaxial region.

A broad spectrum of disease entities affects the atlantoaxial-clival region. This area is readily accessible through a transoral approach, which offers capabilities for canal decompression and fusion. A case is reported that required transoral odontoidectomy with concurrent excision of an osteophyte from the base of an ununited odontoid fracture. Operative preparation, technique, and postoperative management are described in detail. The advantage and applications of the procedure are discussed.

[Internal fixation with U-shaped pin in the treatment of fracture-dislocation of the cervical spine].

Twenty cases with fracture-dislocation of the cervical spine were treated by U-shaped pin fixation. The U-shaped pin made of Steiman pin was used to hold sublaminar fixation of two segments above and below the vertebra of fracture. When atlas or axis was injured, a pair of small holes were drilled on the occipitale. The closed end of the U-Shaped pin was wired through the holes. Early postoperative ambulation was allowed without external fixation. 18 cases were followed up for 8-35 months. 5 cases of six patients with neurologic deficits recovered completely. The remaining one improved, no fai
