Quality of life of obese patients submitted to bariatric surgery.

INTRODUCTION: The quality of life (QOL) of patients with morbid obesity (MO) is reduced given the restrictions it imposes. Bariatric surgery is considered an efficient treatment for MO as it leads to marked and progressive weight reduction. Weight loss, appropriate nutritional advice and follow up may induce significant improvement in QOL. AIM: To evaluate the degree of QOL in patients with MO before and after bariatric, surgery (Fobi-Capella reducing gastroplasty). CASES. MATERIAL AND METHODS: 95 morbidly obese(BMI > 40 kg/m2) or moderately obese (BNI 35 - 39 kg/m2) patients with co-morbidi

Severe protein-calorie malnutrition after bariatric procedures.

BACKGROUND: Serious nutritional complications after Roux-en-Y gastric bypass (RYGBP) are infrequent. In a retrospective study of patients operated during a 68-month period, malnutrition was investigated to analyze circumstances associated with nutritional failure. METHODS: In 236 consecutive RYGBPs, 11 patients with severe malnutrition were identified (4.7%) with age 45.1 +/- 10.6 years (10 females/1 male) and initial BMI 54.6 +/- 8.4 kg/m(2). RESULTS: In these 11 patients, the derangement was diagnosed 17.9 +/- 15.8 months after RYGBP, following defined events in 63.6% (gastric stenosis, as

A controlled study of peripheral neuropathy after bariatric surgery.

BACKGROUND: Although peripheral neuropathy (PN) occurs after bariatric surgery (BS), a causal association has not been established. OBJECTIVES: To ascertain whether PN occurs more frequently following BS vs another abdominal surgery, to characterize the clinical patterns of PN, to identify risk factors for PN, and to assess if nerve biopsy provides pathophysiologic insight. METHODS: Retrospective review identified patients with PN after BS. The frequency of PN was compared with that of an age- and gender-matched, retrospectively evaluated cohort of obese patients undergoing cholecystectomy.

A cluster of polyneuropathy and Wernicke-Korsakoff syndrome in a bariatric unit.

BACKGROUND: Wernicke-Korsakoff syndrome and peripheral neuropathy are very uncommon in bariatric surgical practice. The literature indicates that these complications tend to strike patients receiving unbalanced diets or undergoing rapid weight-loss. METHODS: In a retrospective analysis of the initial experience of a bariatric team in the city of Belem, Pará, in northern Brazil, 5 cases were diagnosed in the first year, 4 of them following gastric bypass and the last one after therapy with an intragastric balloon. RESULTS: All episodes followed periods of severe vomiting, which certainly inte

Current practices in the prophylaxis of venous thromboembolism in bariatric surgery.

BACKGROUND: Morbidly obese patients undergoing bariatric surgery have commonly been concluded to be at high risk for the development of perioperative venous thromboembolism. Due to its clinically silent nature, primary prevention is the key to reduce morbidity and mortality. There is no clear consensus in the literature regarding the optimum approach to minimize this preventable phenomenon. METHODS: Members of the American Society for Bariatric Surgery were surveyed regarding their current practices in the prophylaxis of venous thromboembolism in their bariatric patients. RESULTS: 31% of the

Splenic injury and repair during bariatric surgical procedures.

Incidental splenectomy during exploratory laparotomy significantly increases morbidity and mortality. These rates, elevated in the morbidly obese, prompted a splenic preservation policy for any splenic injury during bariatric surgery. This review was needed to assess how this policy affected this population's intrinsically high perioperative risks. Six of 200 patients having primary or revisional vertical banded gastroplasty for morbid obesity or failure of previous bariatric surgery had splenic injury. In one case, splenectomy was necessary; in the other five, splenic repair was relatively ea

2022 American Society of Metabolic and Bariatric Surgery (ASMBS) and International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) Indications for Metabolic and Bariatric Surgery.

MAJOR UPDATES TO 1991 NATIONAL INSTITUTES OF HEALTH GUIDELINES FOR BARIATRIC SURGERY: Metabolic and bariatric surgery (MBS) is recommended for individuals with a body mass index (BMI) >35 kg/m2, regardless of presence, absence, or severity of co-morbidities.MBS should be considered for individuals with metabolic disease and BMI of 30-34.9 kg/m2.BMI thresholds should be adjusted in the Asian population such that a BMI >25 kg/m2 suggests clinical obesity, and individuals with BMI >27.5 kg/m2 should be offered MBS.Long-term results of MBS consistently demonstrate safety and efficacy.Appropriately

The Impact of Hospital Volume on Bariatric Surgery Safety: A Population-Based Study using Data from the National Bariatric Surgery Registry of the United Kingdom.

BACKGROUND: Hospital procedural volume is recognised as an important determinant of outcomes in complex surgery. However, the volume-outcome relationship in bariatric surgery remains incompletely defined. This study examined the association between hospital procedural volume and major complications in primary metabolic bariatric surgery using the United Kingdom (UK) National Bariatric Surgery Registry (NBSR). METHODS: Prospectively collected NBSR data from 2014 to 2023 were analysed. Adults undergoing RYGB, SG, or OAGB were included. Hospitals were stratified into quartiles using a four-year

Predictors and outcomes of acute kidney injury after bariatric surgery: analysis of the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program data registry.

BACKGROUND: Acute kidney injury (AKI) after surgery increases long-term risk of kidney dysfunction. The major risk factor for AKI after bariatric surgery is having preoperative renal insufficiency. Little is known about the outcomes and risk factors for developing AKI in patients undergoing bariatric surgery with normal renal function. OBJECTIVE: We aimed to describe factors that may increase risk of AKI after primary bariatric surgery in patients without history of kidney disease. SETTING: Academic hospital, United States. METHODS: We performed a retrospective analysis of the Metabolic and

Effectiveness of a Bariatric-Specific Multivitamin Versus Conventional Targeted Supplementation for Preoperative Micronutrient Deficiency Correction in Bariatric Surgery Candidates: A Multicenter Retrospective Cohort Study.

BACKGROUND: Micronutrient deficiencies (MD) are highly prevalent among candidates for bariatric surgery (BS) and are associated with adverse perioperative and postoperative outcomes. Although guidelines recommend systematic preoperative screening and correction, conventional targeted supplementation (CTS) often requires multiple products, potentially limiting adherence and delaying surgical readiness. Bariatric-specific multivitamins (BSM) may simplify nutritional management, but their real-world effectiveness for preoperative correction of multiple MD remains insufficiently investigated. OBJ

The Impact of Breastfeeding on Maintaining Body Weight After Bariatric Surgery-A Retrospective Analysis of Maternal Outcomes of Bariatric Surgery and Pregnancy Study (MOMBARIS) Multicenter Data.

BACKGROUND: Metabolic bariatric surgery (MBS) is the most effective obesity treatment. The increasing number of MBS among young patients has led to more post-bariatric pregnancies. Our study investigates how breastfeeding duration impacts postpartum weight retention and health outcomes for mothers and babies after MBS. METHODS: MOMBARIS (Maternal Outcomes of Bariatric Surgery and Pregnancy Study) is a multicentre study. The population is derived from MOMBARIS 2, which focused on obstetric outcomes. It included patients who became pregnant following MBS, analysing pre-operative metrics, type o

Bariatric surgery after childbirth: does a history of pregnancy affect weight loss after surgery? The Maternal Outcomes of Bariatric Surgery and Pregnancy Study (MOMBARIS).

INTRODUCTION: Obesity is a major global health issue associated with comorbidities, such as type 2 diabetes mellitus (T2DM) and cardiovascular disease. Bariatric surgery is effective, but its outcomes vary. Obstetric history may influence results, as pregnancy induces lasting metabolic and hormonal changes, though current evidence remains unclear. AIM: This study aimed to evaluate whether preoperative pregnancy history affects weight loss outcomes after bariatric surgery. MATERIALS AND METHODS: A retrospective multicenter analysis was conducted within the Maternal Outcomes of Bariatric Surge

Implementation of the enhanced recovery after bariatric surgery (ERABS) protocol with clinical hypnosis: effects on pain and postoperative nausea and vomiting (PONV) management in bariatric surgery.

INTRODUCTION: Obesity is a major global health issue requiring tailored anesthetic and analgesic strategies due to associated physiological and pharmacokinetic changes. The Enhanced Recovery After Bariatric Surgery (ERABS) protocol has been introduced to optimize perioperative care, but its implementation remains inconsistent. This pilot study investigates the integration of a perioperative clinical hypnosis (CH) protocol with ERABS to assess its effects on postoperative pain, nausea, and analgesic/antiemetic drug consumption in patients undergoing bariatric and metabolic surgery (BMS). Previous bariatric surgery before knee arthroplasty: A cautious interpretation.

We read with interest the nationwide register-based study by Aljuboori et al. examining previous bariatric surgery (BAS) and reintervention after knee arthroplasty. The authors appropriately state that their findings do not establish a benefit of BAS or negate the influence of residual obesity. We highlight several interpretive constraints. Matching on body mass index (BMI) at arthroplasty conditions on a post-exposure variable, while restriction to patients who ultimately underwent arthroplasty excludes those who improved sufficiently to avoid surgery. The 95% confidence interval for 2-year r

Beyond Postprandial Symptoms: Fasting Hypoglycemia in Post-Bariatric Hypoglycemia.

BACKGROUND: Post-bariatric hypoglycemia (PBH) is characterized by postprandial neuroglycopenia resulting from an exaggerated hyperinsulinemic response to nutrient intake in patients who have undergone Roux-en-Y gastric bypass, sleeve gastrectomy, or other upper gastrointestinal surgical procedures for weight loss. Fasting hypoglycemia is atypical. OBJECTIVE: We present a cohort of patients who developed symptomatic fasting hypoglycemia in the setting of established PBH and discuss the possible pathophysiological mechanisms. We also examine the role of β-cell hyperfunction in PBH. METHODS: A
