A Rare Presentation of Choledochoduodenal Fistula Due to Ovarian Cancer Metastasis.

Choledochoduodenal fistula (CDF) is an abnormal communication between the common bile duct and the duodenum. It accounts for about 5% to 25% of the total biliary fistulas and is usually due to a perforated duodenal ulcer, choledocholithiasis, and complications secondary to tuberculosis or could be iatrogenic. Primary intrabilliary tumors usually cause obstructive jaundice and rarely biliary metastasis arising from other organs like colon, breast, and lungs can cause obstructive jaundice. There has been a case report of metastasis from ovarian cancer to the major papilla of the duodenum but no …

[Intrabiliary rupture of hepatic hydatid cysts: results of 17 years' experience].

UNLABELLED: Between 1990 and 2006 in the III-rd Surgical Clinic Cluj-Napoca, 366 pacients with hepatic hydatid cyst were admitted and underw … type of rupture (frank or occult). Although the opening of the biliary duct is sutured, the risk of biliary fistulas is not clearly corelated with this approach; in such cases internal drainage provides a good alternative with low morbidity.

Preliminary Report of Percutaneous Cholecystostomy as Diagnosis and Treatment of Biliary Tract Trauma.

BACKGROUND: Biliary leak following severe blunt liver injuries is a complex problem becoming more frequent with improvements in non-operative management. Standard treatment requires main bile duct drainage usually performed by endoscopic sphincterotomy and stent placement. We report our experience with cholecystostomy as a first minimally invasive diagnostic and therapeutic approach. METHODS: We performed a retrospective analysis of consecutive patients with post-traumatic biliary leak between 2006 and 2015. In the first period (2006-2010), biliary fistula was managed using perihepatic draina…

Misleading Mercedes-Benz sign: A case report of misdiagnosed cholecysto-hydatid cyst fistula in acute cholecystitis.

INTRODUCTION: Liver hydatid cysts represent a significant health concern globally, particularly in endemic regions like Tunisia. While they often lead to complications such as biliary fistulas, diagnostic errors can arise from radiologic signs like the "Mercedes Benz sign," which indicates gas within the gallbladder. This report highlights the challenge of diagnosing a rare cholecysto-hydatid cyst fistula, where the presence of gas in the gallstones initially suggested a fistula. CASE PRESENTATION: A 30-year-old female presented with right hypochondrium pain and fever. Ultrasound suggested ch…

Left hepatic duct injury and thoracobiliary fistula after abdominal blunt trauma.

Thoracobiliary fistula after blunt hepatic trauma is rare. We report a case of pleurobiliary fistula after a blunt hepatic trauma leading to a left hepatic lobe laceration together with a left hepatic duct injury. The management of this traumatic lesion is discussed and related to the existing literature data. The diagnosis of traumatic thoracobiliary fistula rests upon clinical suspicion in the setting of a persistent right pleural effusion. Demonstration of the presence of bile in the pleural cavity by thoracocentesis is considered a proof of pleural biliary fistula. We think that laparotomy…

[New techniques of postoperative cholelith endoscopy via the "T" tube fistula].

OBJECTIVE: to explore some new techniques of postoperative cholelith endoscopy via the fistula. METHODS: Netting, washing, clamping, pushing, and electrohydraulic techniques were used in 260 cases of postoperative cholelith endoscopy via the fistula and the clinical data were analyzed retrospectively. RESULTS: The stones in 257 cases (98.8%) were removed completely in an average of 2.7 times. Three patients were operated on again because of biliary stricture and huge stones. CONCLUSION: The application of some new techniques in postoperative cholelith endoscopy via the fistula favors the co

A large common bile duct stone migrated from the gallbladder through a cholecystohepaticodochal fistula: an unusual complication of Mirizzi syndrome type II.

Mirizzi syndrome with a biliobiliary fistula (Mirizzi syndrome type II) is a rare complication of a long-standing gallbladder stone disease. It is even rarer for a gallbladder stone to migrate through a biliobiliary fistula into the common duct. We encountered this interesting complication of Mirizzi syndrome type II in an 86 year-old female patient. A large gallbladder stone migrated into, and impacted into the distal common bile duct through a cholecystohepaticodochal fistula. The stone was resistant to mechanical lithotripsy and was treated with biliary endoprosthesis and oral bile acids.

Cholecystocolonic fistula: A rare case report of Mirizzi syndrome.

INTRODUCTION: Mirizzi syndrome is a rare complication of gallstone disease that more rarely is associated with the formation of cholecystoenteric fistula. PRESENTATION OF CASE: The patient presented with a five-day history of abdominal pain in the right upper quadrant (RUQ), nausea, and emesis. Further ultrasound (US) imaging demonstrated a large gallstone with associated thickened gallbladder with pericholecystic fluid. Computed tomography (CT) imaging, preoperative Hepatobiliary Scintigraphy and Endoscopic Retrograde Cholangiopancreatography (ERCP) displayed findings consistent with a Csend

Cholecystoduodenal fistula in a patient post-Roux-en-Y gastric bypass.

We presented a case of a cholecystoduodenal fistula in a patient 4 years post-Roux-en-Y gastric bypass. The patient presented with biliary colic symptoms after a stone became impacted in the fistula and outflow through the cystic duct was intermittently obstructed by a second stone. The fistulous tract was taken down with a cholecystectomy and duodenum repaired with a modified Graham patch.

Successful management of cholecystocolic fistula by endoscopic retrograde cholangiopancreatography: a report of two cases.

Cholecystocolic fistula is a rare cause of diarrhoea, and the diagnosis may be significantly delayed. Air in the biliary tree on imaging should raise suspicion, and barium enema or endoscopic retrograde cholangiopancreatography will be diagnostic. Cholestyramine should provide effective symptomatic relief until definitive treatment is arranged. We report on two patients with cholecystocolic fistula presenting with severe diarrhoea. They were treated successfully by endoscopic retrograde cholangiopancreatography.

Use of SpyGlass™ DS II Direct Visualization System With SpyBite™ Max Forceps for the Removal of Echinococcus Granulosus Causing Biliary Obstruction.

Hydatid disease, also known as cystic echinococcosis, affecting the liver can vary in clinical presentation from uncomplicated to most complicated one that can rupture into the bile duct. In the past, surgery was the mainstay of treatment for biliary complications. However, with the increasing expertise of endoscopic retrograde cholangiopancreatography, biliary complications are less common. Endoscopic retrograde cholangiopancreatography can be used preoperatively and postoperatively in patients presenting with jaundice to treat biliary obstruction and biliary fistula. In our case report, we i…

[State of the art in therapeutic biliary endoscopy].

Almost all biliary diseases are now amenable to endoscopic treatment. Progress in therapeutic biliary endoscopy is currently focusing on identifying the best treatment options to be used in each case in relation to available evidence: how should a malignant stricture of the common bile duct or hilium be drained? How should a "difficult" biliary stone be extracted? How should an external biliary fistula be treated? What should one do with the gallbladder after extraction of common bile duct stone? Should sedation or general anesthesia be offered? Progress has been made in the technique of endos…

[Spontaneous intestinal biliodigestive fistula and its treatment (author's transl)].

The current clinical and therapeutic status of spontaneous intestinal biliodigestive fistulae in 14 of the author's own cases is presented. Two groups are distinguished: those "short-circuit" connections to the gastro-intestinal tract due to biliary disease--the biliodigestive fistula in the narrower sense as well as enterobiliary fistulae caused by gastro-intestinal disease. Differences in etiology, clinical presentation, therapy and operative lethality make it seem advisable to form two categories, A and B, and to compare the one with the others.

Less is more: an outcome assessment of patients operated for gallstone ileus without fistula treatment.

BACKGROUND: The treatment of gallstone ileus (GI) consists of surgical removal of the impacted bilestone with or without cholecystectomy and repair of the biliodigestive fistula. The objective of this study was to assess whether sparing patients a definitive biliary procedure adversely influenced the outcome. MATERIALS AND METHODS: Patients with a diagnosis of GI were reviewed. Two groups were identified: patients who underwent a definitive biliary procedure with relieving the intestinal obstruction (group 1/G1) and those who did not have a definitive biliary procedure (group 2/G2). In G2, pa

Biliopleural Fistula Following Percutaneous Transhepatic Drainage: A Rare Cause of Bilious Pleural Effusion.

Biliopleural fistula is a rare but serious complication of hepatobiliary interventions. It may present with atypical respiratory symptoms and requires a high index of suspicion for diagnosis. We report the case of a 79‑year‑old woman with choledocholithiasis complicated by failed endoscopic retrograde cholangiopancreatography (ERCP), post‑ERCP pancreatitis, and cholangitis. Following percutaneous transhepatic drainage (PTD) with external biliary catheter placement, she developed a loculated right‑sided pleural effusion. Diagnostic thoracentesis revealed bilious pleural fluid, and magnetic reso

Perfoation of the gallbladder: 'bait' for the unsuspecting laparoscopic surgeon.

INTRODUCTION Cholecystectomy is one of the most common elective procedures carried out by general surgeons. Most patients present with typical biliary anatomy and simple gallstone disease. Intraoperative and postoperative courses are frequently predictable and uncomplicated. Nevertheless, a small but significant number of patients experience complicated disease with rare presentations and complex biliary anatomy. Unfortunately, consensus on appropriate care for such patients is lacking. CASE HISTORY We describe three patients who presented with complex manifestations of gallbladder perforation
