The effect of omitting botulinum toxin from the lower eyelid in blepharospasm treatment.

We randomly selected 26 patients with essential blepharospasm to receive either botulinum toxin or saline injection in their lower eyelids to evaluate the necessity of lower eyelid injection to relieve blepharospasm. As diplopia may occur from botulinum toxin injections for blepharospasm, most commonly from injection of the lower eyelid, and surgical relief of blepharospasm is often achieved by excision of only the upper eyelid protractors, omission of toxin from the lower eyelid seemed both desirable and possible. All patients received botulinum toxin in the upper eyelids, above the eyebrows,…

Long-term results of treatment of idiopathic blepharospasm with botulinum toxin injections.

One hundred and one patients with idiopathic blepharospasm have been treated with injections of botulinum toxin A into the orbicularis oculi. Ten had previously had facial nerve avulsions and responded well, normal visual function being restored in the majority (7/10) for an average of 14 weeks. Without prior surgical treatment the response was more variable, but 71/91 regained normal or near normal vision. Older patients, those with a family history of the condition, and those without oromandibular dystonia responded slightly better. The severity of the blepharospasm, the length of the histor…

Blepharospasm and orofacial-cervical dystonia: clinical and pharmacological findings in 100 patients.

We evaluated prospectively 100 patients, the largest reported series, with blepharospasm and orofacial-cervical dystonia, or Meige syndrome. The mean age at onset was 51.7 years, and 81% presented between the ages of 40 and 70. Women outnumbered men three to two. Blepharospasm was the initial symptom in 58 patients, but only 23 had involuntary movements localized to the orbicularis oculi. Sixty-one patients had the complete syndrome, blepharospasm and oromandibular dystonia, and 60 had neck or generalized dystonia in addition to the orofacial movements. Twenty-one patients with spasmodic dysph…

Hemifacial Spasm and Blepharospasm: Divergent Social Cognition Performance Despite Similar Neurocognitive Profiles.

BACKGROUND: Hemifacial spasm (HFS) and blepharospasm (BSP) are two neurological disorders characterized by involuntary contractions in the eye and face area. These two diagnostic groups' neurocognitive and social cognition performances are not adequately understood regarding behavioral neurology. OBJECTIVES: This study compared HFS and BSP groups with healthy controls (HCs) regarding neurocognition and social cognition performances. METHODS: Twenty people from each group with equal distribution in terms of age, education level, and gender were included in the study. All participants were adm…

Ocular Surface Metrics in Blepharospasm Patients After Treatment With Botulinum Toxin Injections.

PURPOSE: The purpose of this study was to assess blepharospasm patients regarding the effect of botulinum toxin in ocular surface parameters. METHODS: A prospective study was performed in blepharospasm patients treated with onabotulinumtoxinA. A smartphone (iPhone 6S, Apple) and custom-made software were used to record the eyelid movements for 3 minutes in a standardized manner. Optical coherence tomography of the tear meniscus was used to assess the tear meniscus area. Tear break-up time and the Ocular Surface Disease Index questionnaire were also evaluated. Patients were assessed before and…

Pantothenate Kinase-Associated Neurodegeneration (PKAN) With Concomitant Blepharospasm: Unveiling a Clinical Enigma.

Pantothenate kinase-associated neurodegeneration (PKAN) is a rare and complex neurodegenerative disorder. It occurs due to mutations in the sequencing of the PANK2 gene. Here, we describe the case of a 22-year-old male patient who presented with severe blepharospasm; he had abnormal facial distortions, shaky limbs, rigid muscles, and a slow pace of movement, making a diagnosis tricky. Accumulation of iron in excessive amounts in the basal ganglia, a part of the brain that governs movement, is linked to PKAN. In this case, the "eye of the tiger" indication, a distinctive pattern only seen by MR…

Machine Learning for Predicting Individual Severity of Blepharospasm Using Diffusion Tensor Imaging.

Accumulating diffusion tensor imaging (DTI) evidence suggests that white matter abnormalities evaluated by local diffusion homogeneity (LDH) or fractional anisotropy (FA) occur in patients with blepharospasm (BSP), both of which are significantly correlated with disease severity. However, whether the individual severity of BSP can be identified using these DTI metrics remains unknown. We aimed to investigate whether a combination of machine learning techniques and LDH or FA can accurately identify the individual severity of BSP. Forty-one patients with BSP were assessed using the Jankovic Rati…

Comparable Botulinum Toxin Outcomes between Primary and Secondary Blepharospasm: A Retrospective Analysis.

BACKGROUND: Blepharospasm is a focal cranial dystonia, which could be idiopathic in origin or secondary to an underlying disorder that commonly impairs quality of life. Botulinum toxin (BoNT) injections have become the treatment of choice; however, a less favorable response to BoNT is expected in secondary blepharospasm. No studies have been conducted comparing outcomes between blepharospasm cohorts. We therefore aim to compare BoNT outcomes in primary and secondary blepharospasm subjects. METHODS: A retrospective review of 64 blepharospasm subjects receiving BoNT therapy was conducted. Demog…

The blink reflex recovery cycle differs between essential and presumed psychogenic blepharospasm.

BACKGROUND: Psychogenic blepharospasm is difficult to distinguish clinically from benign essential blepharospasm (BEB). The blink reflex recovery cycle measures the excitability of human brainstem interneurons and is abnormal in BEB. We wished to study the blink reflex recovery cycle in patients with atypical (presumed psychogenic) blepharospasm (AB). METHODS: This was a prospective data collection study investigating the R2 blink reflex recovery cycle at interstimulus intervals (ISI) of 200, 300, 500, 1,000, and 3,000 msec in 10 patients with BEB, 9 patients with AB, and 9 healthy controls. …

Cortical gray matter changes in primary blepharospasm: a voxel-based morphometry study.

Previous voxel-based morphometry studies of patients with primary blepharospasm documented gray matter volumetric differences of the striatum, cerebellum, thalamus, and parietal lobe areas. However, these results were inconsistent across studies, which recruited relatively small samples and did not always provide detailed clinical information on patients with blepharospasm. The objective of this study was to analyze whole-brain gray matter volume in a larger sample of patients with blepharospasm and to expand on previous works by evaluating whether clinical features of blepharospasm correlate …

Botulinum toxin in blepharospasm and oromandibular dystonia: comparing different botulinum toxin preparations.

Amongst all regions of the body, the craniocervical region is the one most frequently affected by dystonia. Whilst blepharospasm--involuntary bilateral eye closure--is produced by spasmodic contractions of the orbicularis oculi muscles, oromandibular dystonia may cause jaw closure with trismus and bruxism, or involuntary jaw opening or deviation, interfering with speaking and chewing. Both forms of dystonia can be effectively treated with botulinum toxin injection. This article summarizes injection techniques in both forms of dystonia and compares doses, potency and efficacy of different comme…

Frontalis suspension for essential blepharospasm unresponsive to botulinum toxin therapy. First results.

We performed frontalis suspension in 12 patients presenting with essential blepharospasm or "apraxia" of eyelid opening who did not respond sufficiently to botulinum toxin injections. An improvement could be observed in 9 patients. During the follow-up period (4-18 months) the effect of surgical intervention remained stable. As opposed to other surgical procedures (excision of the orbicularis muscle, resection of facial nerve branches), frontalis suspension can be considered as a minimally invasive and even reversible treatment.

Tear film lipid layer thickness and blink dynamics in patients with blepharospasm.

BACKGROUND: This study investigates the effect of botulinum toxin A on lipid layer thickness (LLT) and blink dynamics in patients with benign essential blepharospasm (BEB) compared to dry eye disease (DED) patients. METHODS: We reviewed the medical records of patients with dry eye disease (DED) and BEB treated with botulinum toxin A (BoT A) injections. Data on demographics, lipid layer thickness (LLT), meiboscore, and blink dynamics measured using a LipiView II interferometer before and 2 months after BoT A were collected. RESULTS: Each 28 eyes from 28 patients with BEB and age- and sex-matc…

A pilot exploratory study for analysis of clinical features and plasma catecholamines in blepharospasm.

Blepharospasm (BSP) is a common focal movement disorder linked to the basal ganglia and plasma catecholamines (CAs). This study aimed to analyze clinical characteristics of BSP patients and explore the relationship with plasma CAs. Clinical characteristics, clinician-rated scales, and plasma CAs were recorded, including dopamine (DA), 3-methoxytyramine (3-MT), and the 3-MT/DA ratio. Plasma CA levels were measured using liquid chromatography-tandem mass spectrometry. Clinical features, plasma CAs, and metabolite contents were compared between groups. Spearman's correlation analysis examined the…

New trends in the treatment of blepharospasm: replacing the standardized approach with a personalized approach.

INTRODUCTION: Blepharospasm is a type of dystonia associated with overactivity of the orbicularis oculi and nearby muscles causing significant disability. Available treatments are only symptomatic, and many patients experience disability related to incomplete symptom relief. AREAS COVERED: The authors provide a review the phenomenology of blepharospasm and its usual treatments. The evidence for botulinum toxin, oral pharmaceuticals and surgical treatments are summarized. Despite considerable phenotypic heterogeneity, the literature indicates a standardized approach to treatment is often pursu…

High-Density Glasses in Blepharospasm: Influence of Migraine and Early Morning Effects.

Photophobia and somatosensory disorders in patients with benign essential blepharospasm (BEB) are caused by the accumulation of light input to the brain, resulting in persistent eye-opening difficulties. Consecutive patients with photophobia and photosensitivity due to BEB were recruited. The patients were asked to participate in a trial using high-density glasses, with only 1.5% visible light transmittance, produced by Tokai Optical Co. Ltd. (HD glass®) (Aichi, Japan). Patients were instructed to wear glasses 2-3 times per day for 20 min or more in a dimly lit room every day for at least two …

Visual dysfunction of superior colliculus and lateral geniculate nucleus in idiopathic blepharospasm.

BACKGROUND: The etiology and pathophysiology of idiopathic blepharospasm (BP) are still largely unknown. It has been hypothesized that BP is the consequence of a dysfunction of the basal ganglia loop, although cortical areas, cerebellum, and other brainstem structures may be involved. There is some evidence that the superior colliculus (SC), a sensorimotor brainstem structure, is involved in another adult-onset focal dystonia, the cervical dystonia. To date, there is no data concerning the implication of the SC in BP. OBJECTIVES: Our study aims to investigate the role of the SC in people with…

Postural stability in blepharospasm: the effects of dual-tasking and botulinum toxin therapy.

BACKGROUND: Blepharospasm is a focal dystonia that presents as involuntary, intermittent, continuous contractions of the eyelids. Abnormal eyelid contractions in blepharospasm are expected to cause balance problems, but there is no clear information. OBJECTIVE: This study was designed to evaluate the effect of blepharospasm on postural stability (PS) in patients with blepharospasm. As a secondary endpoint, the efficacy of botulinum toxin type-A (BoNT-A) treatment on static balance in patients with blepharospasm was investigated. METHODS: Twenty-four patients with blepharospasm receiving regu…
