Biomarkers and clinical factors associated with sepsis-induced coagulopathy: A systematic review and meta-analysis.

ObjectiveSepsis-induced coagulopathy (SIC) is a life-threatening complication that significantly impacts patient prognosis. This systematic review and meta-analysis aimed to evaluate clinical and biological indicators associated with the development of SIC to facilitate early risk identification.MethodsWe searched nine databases for studies reporting predictive factors for SIC. Data were pooled using random-effects or fixed-effects models as appropriate. Methodological quality was assessed using the Newcastle-Ottawa Scale.ResultsA total of 34 studies involving 37,459 patients were included. Me

Viscoelastic testing in patients with very rare bleeding disorders of secondary hemostasis, a scoping review.

INTRODUCTION: Very rare bleeding disorders (VRBDs) are hereditary disorders which cause increased risk of bleeding. In general, VRBDs consists of rare platelet function disorders, very rare coagulation factor deficiencies (other than Factor (F) VIII or FIX) and disorders of the fibrinolytic pathway. The rarity of these disorders along with the scarcity of specialized hemostasis laboratories capable to perform the necessary diagnostic tests, results in significant challenges in diagnosing and monitoring patients with VRBD. Viscoelastic testing could potentially address these challenges. Aim Thi

The evaluation and management of coagulopathies in the intensive therapy units.

Coagulopathies are common in intensive therapy units (ITUs) and may represent both the cause and the consequence of a critically ill status. Because coagulopathies are associated with increased complications in terms of both increased ischemic and bleeding events, the prevention, assessment, and treatment of these clinical conditions are important issues to be considered in the ITU. In this review, we explore the pathophysiological mechanisms, discuss the most common causes, and provide practical recommendations on the assessment and treatment of coagulopathies in the ITU.

Coagulopathy in Isolated Traumatic Brain Injury: Myth or Reality.

INTRODUCTION: Traumatic Brain Injury (TBI) has been shown to be associated with altered hemostasis and coagulopathy, that correlates with worsening secondary injury and clinical outcomes. Isolated Traumatic Brain Injury (iTBI), that is TBI without significant extracranial injuries, has also been shown to be associated with systemic coagulopathy and derangements in hemostasis. METHODS: Literature Review. RESULTS: Present your results in logical sequence in the text, tables, and figures, giving the main or most important findings first. Do not repeat all the data in the tables or figures in th

Diagnosis and Treatment of Trauma-Induced Coagulopathy by Viscoelastography.

This article explores the application of viscoelastic tests (VETs) in trauma-induced coagulopathy and trauma resuscitation. We describe the advantages of VETs over conventional coagulation tests in the trauma setting and refer to previous disciplines in which VET use has reduced blood product utilization, guided prohemostatic agents, and improved clinical outcomes such as the mortality of critically bleeding patients. We describe different VETs and provide guidance for blood component therapy and prohemostatic therapy based on specific VET parameters. Because the two most commonly used VET sys

Evaluation of Abnormal Bleeding in Children.

Abnormal bleeding or bruising is a relatively common clinical concern, frequently causing parents to seek care from their children's primary care provider. Although ecchymosis from playful trauma, occasional epistaxis, and heavy menstrual bleeding around the time of menarche and beyond are common occurrences, it can be difficult for pediatricians and other primary care providers to determine if their patient has age-appropriate symptoms versus abnormal or excessive bleeding. This review discusses considerations for abnormal bleeding in pediatrics and discusses an initial approach for evaluatio

Management of surgical patients with bleeding disorders.

Invasive procedures for patients with bleeding disorders require planning on the part of the health care team. The patient population affected involves those with hereditary bleeding disorders, such as von Willebrand disease or hemophilia; in addition, patients who use antithrombotic drugs must be considered and their care managed. The choice of treatment depends on a number of factors, including the procedure planned, the type and severity of the disorder, and the age and morbidity of the patient. The indications, dosing, and timing of presurgical and surgical interventions will be reviewed,

Defective fibrinolytic states as triggers of myocardial infarction: the cardiologist's view.

Disclosure of a specific prothrombotic/antifibrinolytic state by determination of plasminogen activator inhibitor type 1 (PAI-1), tissue-type plasminogen activator (t-PA), and t-PA/PAI-1 complex levels in plasma may help to define patients at high risk of developing acute coronary syndromes. According to the mechanisms of PAI-1 up-regulation, i.e. genetic factors, an activated renin-angiotensin-aldosterone system, and other atherosclerotic risk factors (hypertriglyceridemia, insulin resistance), PAI-1 plasma levels might be reduced by the action of antidiabetic drugs, fibrates, statins, as wel

The pathophysiology of bleeding disorders presenting as abnormal uterine bleeding.

Abnormal uterine bleeding is often the presenting complaint in women with underlying coagulopathies. A clear understanding of the pathophysiology of common bleeding disorders will help the practicing obstetrician/gynecologist in the diagnosis and treatment of these conditions. The normal hemostatic process can be divided into three phases. The first phase, primary hemostasis, consists of platelet adhesion and aggregation. After vascular injury, proteins in the subendothelium are exposed that promote platelet adhesion. Platelet adhesion is uniquely dependent on von Willebrand factor, a plasma p

[Traumatic emergencies and hemostasis].

The occurrence of bleeding in trauma patients is a life-threatening problem which can be explained by different mechanisms. The infusion of cristalloids, colloids, packed red blood cells, or even fresh frozen plasma is very rarely responsible for bleeding but it can contribute to dilute the patient's platelet pool, and especially dilutional thrombocytopenia is the first cause of bleeding after massive transfusion. Blood coagulation factor activity is decreased after a massive fluid infusion is performed but it has to reach a dramatically low plasma level in order to induce troubles. It has to

Haemorrhagic diathesis in children associated with vitamin K deficiency.

A haemorrhagic diathesis is described in infants; this is preceded and accompanied by constitutional symptoms such as fever, diarrhoea, vomiting, anorexia, and pallor. These children had a severe coagulation abnormality, due to deficiency of vitamin-K-dependent coagulation factors, and it was corrected by administration of vitamin K. No conclusion could be drawn as to the aetiology of this condition but some possible causes are discussed.

Hypercoagulable States: What the Oral Surgeon Needs to Know.

Thrombophilia or hypercoagulable conditions can be thought of as either inherited or acquired. The inherited disorders include deficiencies of antithrombin, protein C, or protein S or the common disorders of factor V Leiden and prothrombin G20210A gene mutation. All these disorders are inherited as autosomal dominant and predispose individuals primarily to venous thrombosis. Acquired thrombophilic conditions are seen in individuals with cancer, phospholipid antibodies, and a whole host of other conditions that alter endothelial function, change blood levels of coagulant or anticoagulant protei

Role of the blood coagulation cascade in hepatic fibrosis.

Liver is the primary source of numerous proteins that are critical for normal function of the blood coagulation cascade. Because of this, diseases of the liver, particularly when affiliated with severe complications like cirrhosis, are associated with abnormalities of blood clotting. Although conventional interpretation has inferred cirrhosis as a disorder of uniform bleeding risk, it is now increasingly appreciated as a disease wherein the coagulation cascade is precariously rebalanced. Moreover, prothrombotic risk factors are also associated with a more rapid progression of fibrosis in human

Upregulation of coagulation pathway and enhanced platelet activity in hypoxia-exposed rat blood.

To investigate the effects of chronic hypoxia on platelet characteristics and overall coagulation function, and to elucidate the mechanisms underlying high-altitude-induced blood system disorders. Rats were exposed to chronic hypoxic conditions. Platelet counts were measured using routine blood analysis. Plasma levels of platelet activation markers (P-selectin and sCD40L) were quantified by ELISA. A label-free quantitative proteomic approach was employed to identify alterations in the platelet coagulation cascade proteome, with key findings validated by western blotting. Coagulation function w

[Diagnosis and management of bleeding disorders caused by hyperfibrinolysis (designated intractable disease No. 347 in Japan)].

Hyperfibrinolysis-related bleeding is well recognized in conditions such as the acute phase of trauma, disseminated intravascular coagulation with hyperfibrinolysis, and acute promyelocytic leukemia. In contrast, whether primary hyperfibrinolysis can directly cause bleeding in the absence of an underlying disorder has not been sufficiently investigated, largely due to the lack of an established disease concept and the limited availability of reliable functional assays. Congenital deficiencies of individual fibrinolysis inhibitors-such as plasminogen activator inhibitor-1 (PAI-1) deficiency, α2
