Bilateral traumatic patellar fracture: a case report and review of literature.

Simultaneous isolated bilateral patellar fractures are very rare injuries and most often associated with systemic disorders such as hyperparathyroidism, osteoporosis, stress fracture and kidney failure. Isolated bilateral traumatic fracture of patella following an unusual mode of injury is seldomly reported in the literature. We reported such a case following a road traffic accident without any associated injuries or co-morbid condition. The patella on the right side had transverse open fracture which was fixed with two Kirschner wires following tension band principle, and that on the left sid

Complete acromioclavicular separations. A comparison of operative methods.

A retrospective study of the surgical treatment methods for complete acromioclavicular (AC) dislocations was initiated to investigate the efficacy of each. During the years 1972-1985, a total of 95 surgical procedures were performed for complete AC dislocations. The medical records of 90 cases were available for review. The operative methods compared coracoclavicular (CC) with AC fixation methods. Excision of the distal clavicle was performed for chronic dislocations. AC fixation methods included Kirschner wires alone, Kirschner-wire fixation with coracoacromial ligament transfer, and Kirschne

Palmar dislocation of scaphoid and lunate as a unit: case report with special reference to carpal instability and treatment.

A case of palmar dislocation of scaphoid and lunate as a unit is reported with a 3 1/2-year follow-up. Closed reduction was followed by redislocation after a week. Secondary open reduction could not prevent a dorsiflexed intercalated segmental instability deformity, which ended with arthritic changes in the wrist. A plea is made for early open reduction and internal fixation.

Spinal Needle-Guided Arthroscopic Transosseous Repair for Bony Bankart Lesions.

Bony Bankart lesions present considerable challenges in shoulder surgery, particularly when large. While smaller lesions can often be treated effectively using the single-row technique, larger lesions require more complex approaches. Several arthroscopic techniques have been developed to manage these lesions. Although the double-row technique is effective, it has the disadvantage of placing suture material on the articular surface. In contrast, the transosseous technique permits adequate compression and surface contact without involving the articular surface; however, it is technically demandi

Accessibility of osteochondral lesion at the capitellum during elbow arthroscopy: an anatomical study.

INTRODUCTION: Osteochondrosis dissecans (OCD) at the capitellum is a common pathology in young patients. Although arthroscopic interventions are commonly used, there is a lack of information about the accessibility of the defects during elbow arthroscopy by using standard portals. MATERIALS AND METHODS: An elbow arthroscopy using the standard portals was performed in seven fresh frozen specimens. At the capitellum, the most posterior and anterior cartilage surface reachable was marked with K-wires. Using a newly described measuring method, we constructed a circular sector around the rotationa

In vitro activity of exebacase against methicillin-resistant Staphylococcus aureus biofilms on orthopedic Kirschner wires.

Orthopedic foreign body-associated infection can be difficult to treat due to the formation of biofilms protecting microorganisms from both antimicrobials and the immune system. Exebacase is an antistaphylococcal lysin (cell wall hydrolase) under consideration for local treatment for biofilm-based infections caused by methicillin-resistant Staphylococcus aureus (MRSA). To determine the activity of exebacase, we formed MRSA biofilms on orthopedic Kirschner wires and exposed them to varying concentrations (0.098, 0.98, 9.8 mg/ml) of exebacase and/or daptomycin over 24 h. The biofilm consisted of

Dual Parallel Intramedullary K-Wires for Arthrodesis of the Finger Distal or Thumb Interphalangeal Joints.

OBJECTIVE: K-wire arthrodesis methods are commonly used during arthrodesis of the finger distal interphalangeal (DIP) or thumb interphalangeal (IP) joints. Here we propose an advantageous approach involving dual parallel intramedullary K-wires with the K-wire tips cut to bury underneath the skin. METHODS: From January 2017 to December 2021, 35 patients (43 joints) underwent finger DIP or thumb IP joint arthrodesis using this method. Radiographic outcomes were evaluated, while functional outcomes were assessed using the visual analogue scale (VAS) for pain and the Quick Disabilities of the Arm

Risk of tendon failure with repeated passage of Kirschner wires or hypodermic needles: an experimental study.

During hand surgery, tendons may be at risk of damage. This biomechanical study aims to assess the risk of tendon rupture due to passage of Kirschner wires or hypodermic needles. Porcine extensor tendons were divided into four groups. Group 1: a control group was tested to ensure that repeated stress alone did not cause failure. Group 2a: 1.1-mm Kirschner wires were hand pushed through tendons 50 times and then stressed to 40 N, repeated until tendon failure. In Group 2b, K-wires were passed while rotating using a drill. Group 3: the experiment was repeated using a 20 G hypodermic needle. Grou

[Comparison of Three Methods of Sternal Closure after Open Heart Surgery:Analysis of 722 Cases for 11 Years].

722 patients who returned to the intensive care unit( ICU) after completing the cardiac surgery and closing the median sternotomy from Jan 2010 to Feb 2021 at our hospital were divided into 3 groups according to the different sternal closures. Sternum was fixed with 6 wires alone in group A (n=333), with 2 absorbable plates and 6 wires in group B( n=259) or with 3 titanium plates with 20 screws and 4 wires in group C (n=130). Background characteristics were not different between the 3 groups. Total number of 3 complications (postsurgical bleeding, mediastinitis and delayed cardiac tamponade) w

SmartToe, ToeGrip and buried k-wire versus percutaneous k-wire fixation for 2nd PIPJ arthrodesis: A comprehensive review of outcomes.

BACKGROUND: Proximal interphalangeal joint arthrodesis is a reliable and repeatable method of surgical correction for the semi-rigid and rigid hammer toe deformity. In recent years the authors have noted a significant increase in the use of novel intramedullary devices in place of the traditional percutaneous Kirschner wires (k-wire). This paper set out to critically review three methods of internal fixation; ToeGrip implant, SmartToe implant and buried k-wire technique in comparison to the traditional method of percutaneous k-wire fixation during arthrodesis of the proximal interphalangeal jo

Are percutaneous pinning the best treatment for gartland type iii supracondylar humeral fractures in children?

Background. Supracondylar humeral fractures are the most frequent fractures of the elbow in children. Gartland type III fractures require a surgical treatment. The preferred management is closed reduction and percutaneous pinning with K-wires. Methods. We have treated 15 patients (mean age 6,5 years) with Gartland type III fractures. In 14 patients the percutaneous reduction with K-wires was possible but in 1 case the open reduction was necessary due to the irreducibility of the fracture. In all cases a cycle of rehabilitation was performed. Results. All patients have been evaluated using Flyn

Plantar dislocation of the metatarsophalangeal joint: case report.

Dislocations of lesser metatarsophalangeal joints are rare and are almost always dorsal hyperextension injuries. The patient reported sustained plantar dislocations of the second and third metatarsophalangeal joints in a motor vehicle accident; the mechanism of injury was hyperflexion. Closed reduction was not possible, and at the time of open reduction, the extensor digitorum longus and brevis tendons to the third toe were trapped beneath the plantar aspect of the third metatarsal head. Once the tendons were retracted dorsomedially, the joint was easily reduced. Fixation with a Kirschner wire

[Metatarsus primus double osteotomy, a logical and non-disabling surgical technique for treatment of hallux valgus].

The procedure known as the Metatarsus Primus Double Osteotomy ("L'ostéotomie métatarsienne bipolaire" of Schnepp-Carret) for the treatment of the hallux valgus is a logical, efficient and non-mutilating procedure. The technique consists in reducing the hallux valgus deformity by a subcapital bone wedge resection of the metatarsus primus. The bone wedge is then pinched into a second proximal osteotomy of the metatarsus primus, correcting the varus deformity in a valgus direction. This procedure is little known and is not even mentioned in the classic orthopedic literature of German and English

Operative stabilization of the posttraumatic thoracic and lumbar spine: a comparative analysis of the Harrington distraction rod and the modified Weiss spring.

To assess the efficacy of operative stabilization techniques, a retrospective study of Harrington distraction rod (HDR) and modified Weiss spring instrumentation was performed in 90 patients. An operation was performed for one or both of two indications: persistent spine instability or the presence of a neurological deficit in patients with incomplete neurological injuries and myelographic evidence of spinal cord or cauda equina compression. The lateral extracavitary operative approach to the spine for decompression and anterior interbody fusion was performed with an accompanying HDR (47 patie

[Scapho-capitate fracture syndrome of both hands--first description of a bilateral occurrence of a rare carpal injury].

The scapho-capitate fracture syndrome is a rare but severe injury of the carpus. It is characterized by a rotation of the proximal fragment of the capitate bone of 90 or 180 degrees and a fracture of the navicular bone in the intermediate third. This injury is caused by a fall on the outstretched and dorsiflected hand. The scapho-capitate fracture syndrome was firstly described in 1956 by Fenton. There are 31 cases described in literature but none of these involves both hands. Here we describe the first case of scapho-capitate fracture syndrome in both hands, in a 21 year old woman who jumped

Bilateral vertebral artery occlusion secondary to atlantoaxial dislocation with os odontoideum: implication for prophylactic cervical stabilization by fusion--case report.

A 27-year-old male presented with an unusual atlantoaxial dislocation associated with os odontoideum, after suffering sudden onset of significant neurological deterioration. Angiography demonstrated simultaneous occlusion of the bilateral vertebral arteries assumed responsible for the devastating neurological deficits. He initially underwent cervical immobilization with a Crutchfield tong for 2 months and subsequent posterior fusion using an iliac bone graft and wires. The neurological symptoms gradually subsided, and 5 years later good healing of the bone graft without instability and ample s

Total subperiosteal resection treatment of solitary bone cysts of the humerus.

The solitary bone cyst is most frequently located in the upper arm and the average age of the affected patients is between 7 and 9 years, thus perceptibly lower than in cases where solitary bone cyst occurs elsewhere, where the average age is 15. The tendency towards recurrence before 10 years of age is twice as great as the tendency after that age. Investigation of the results obtained from the treatment of 26 patients suffering from solitary bone cyst of the humerus showed a recurrence rate of 55% after curettage and filling-in of the defect with cancellous bone grafts, whereas after total s
