Cerebrospinal fluid cytology of an endolymphatic sac tumor.

Endolymphatic sac tumor (ELST) is a rare neoplasm which is seldom evaluated by cytopathology. We report the clinicopathologic course and cytologic cerebrospinal fluid (CSF) findings in a 58-year-old patient with brainstem lesions who originally presented with vertigo but progressed to having left 7th, 8th, 9th, and 10th cranial nerve palsies, right-sided weakness, and occipital headaches. Cytospin of the CSF revealed large epithelioid cells similar to cells seen in a surgical resection of a brain mass three months previously. Review of the surgical specimen revealed a well-differentiated gland

Angiographically occult brainstem vascular malformation: a longitudinal comparison of magnetic resonance imaging (MRI) and multimodal evoked potential (EP) recordings.

Over a period of several years, a patient with angiographically occult vascular malformation (AOVM) involving the brainstem was longitudinally studied by means of serial Magnetic Resonance Imaging (MRI) and multimodal Evoked Potential (EP) recordings (visual-VEP, brainstem auditory-BAEP, somatosensory--SEP--by stimulating median and peroneal nerves). MRI did contribute to an accurate definition of AOVM features. In particular, it was able to follow over time the AOVM size, and to discriminate between recent and old bleedings. Multimodal EP recordings displayed different pathological BAEP and p

Testicular Plasmacytoma Masking as Epididymo-orchitis in a Known Multiple Myeloma Patient.

BACKGROUND/AIM: Extramedullary plasmacytoma (EMP) is defined as a localized plasma cell neoplasm that arises in tissues other than the bone. The most common sites of involvement of EMP are the upper airways followed by lymph nodes, gastrointestinal tract, thyroid gland, skin, brain, liver, and lungs. Testicular plasmacytoma has a very rare occurrence with about 70 cases reported in literature to date. CASE REPORT: We describe a 52-year-old-male with a diagnosis of multiple myeloma presenting with lytic lesions of the axial skeleton. He had lambda light chain restricted, R-ISS stage II with hi

Coaxial insulated bipolar electrode for monopolar and bipolar mapping of neural tissue: technical note with emphasis on the principles of intra-operative stimulation.

To facilitate use of various stimulation modes for intra-operative monitoring during neurosurgical procedures, we designed and produced a variable stimulation system that consists of a coaxial bipolar flush-tip insulated stimulation electrode, and a switch box to administer monopolar or bipolar stimulation through the same stimulation electrode. The electrode is composed of components that are readily available, so that its construction can be duplicated by others at minimal expense. For mapping of neural tissue during surgical procedures, we use the monopolar stimulation mode to identify rela

Magnetic resonance imaging of peri-aqueductal lesions.

Fourteen lesions in the peri-aqueductal region were evaluated by 0.5T magnetic resonance imaging (MRI) and computed tomography (CT). Fifty normal magnetic resonance studies of the same region were reviewed. The aqueduct of Sylvius is seen in the midline sagittal MRI sections in all normal patients. It is not usually visible in cases of benign stenosis but may be seen in membranous obstruction. Benign stenosis is distinguished from neoplastic obstruction by the lack of increased signal on T2 weighted techniques. Blood within pineal tumours is easily detected and may be differentiated from lipom

CNS Tumors - clinical and radiological aspects.

Tumors of the central nervous system (CNS) include primary tumors - itraaxial, growing from brain and spinal cord cells (neuroepithelial tumors) or extraaxial, growing from surrounding structures (brain and spinal cord, nerve sheaths, vascular structures, lymphatic tissue, germ cells, malformations, pituitary glands). Much more often they are located in the intracranial space a solitary or multiple metastatic spread of malignancy originating from another organ (eg lung, breast, malignant melanoma, Grawitzs tumor). The occurrence of metastases of solid tumors is then in the intraaxial or extraa

Transpetrosal approach: surgical anatomy and technique.

Transpetrosal operations have been shown to offer distinct advantages over traditional operations in approaching lesions of the petroclival area. Confusion about these approaches exists due to the variety of names given to these procedures and the lack of detailed descriptions needed to perform them. After extensive review of the literature, we have determined that all transpetrosal techniques fall into one of two categories: anterior petrosectomy or posterior petrosectomy. Combining one of these procedures with existing conventional procedures accurately describes all existing transpetrosal o

Frequency and outcomes of midline gliomas in a tertiary care hospital in Pakistan: a retrospective study.

INTRODUCTION: Midline structures in the central nervous system include the thalamus, brainstem, and spinal cord. Within the midline tumors, Diffuse midline gliomas (DMGs) and diffuse intrinsic pontine gliomas (DIPG) have a poor prognosis. DMGs are inclusive of all diffuse intrinsic pontine gliomas (DIPG), previously usually used for only pontine gliomas, to emphasize that these lesions are not solely centered in the pons/brainstem. In this retrospective review, we aim to report the frequency and outcomes of midline gliomas amongst all midline tumors in a tertiary care setup. METHODS: Data wer

Brainstem glioma: clinical manifestations of meningeal gliomatosis.

Brainstem gliomas of children are variably malignant tumors that rarely have been reported to produce subarachnoid dissemination. Nevertheless, during a two-year period, 5 of 15 such patients treated by us developed symptoms of leptomeningeal metastases. The diagnosis of an anaplastic astrocytoma with meningeal gliomatosis was confirmed postmortem in all 5. In 3 children, meningeal symptoms preceded other signs of posterior fossa recurrence. Symptoms of meningeal gliomatosis included local or radiating back pain (5 patients), segmental weakness (3), paresthesia (2), and incontinence (2). Myelo

Exploration of the prognostic value of the resection of adult brainstem high-grade glioma based on competing risk model, propensity score matching, and conditional survival rate.

BACKGROUND: Adult brainstem high-grade glioma (HGG) is a refractory disease, and the treatment strategy of resection is still controversial. OBJECTIVE: To investigate the prognostic value of brainstem HGG resection in adults. METHODS: We collected 126,386 samples from the Surveillance, Epidemiology, and End Results (SEER) database between 1998 and 2016, and screened 973 patients diagnosed with adult brainstem HGG, who were in turn, grouped into 899 cases of non-resections and 74 cases of resections. Competing risk models were used to screen independent prognostic factors. Propensity score ma

The role of diffusion tensor imaging and tractography in the surgical management of brainstem gliomas.

OBJECTIVE: Diffusion tensor imaging (DTI) and diffusion tensor tractography (DTT) have the ability to noninvasively visualize changes in white matter tracts, as well as their relationships with lesions and other structures. DTI/DTT has been increasingly used to improve the safety and results of surgical treatment for lesions in eloquent areas, such as brainstem cavernous malformations. This study aimed to investigate the application value of DTI/DTT in brainstem glioma surgery and to validate the spatial accuracy of reconstructed corticospinal tracts (CSTs). METHODS: A retrospective analysis

Infratentorial supracerebellar resection of a pineal tumor using a high definition video exoscope (VITOM®).

A telescope based high definition (HD) video system (VITOM®, Karl Storz GmBH & Co., Tuttlingen, Germany) has recently been proposed as an alternative to the operating microscope for microneurosurgery. It remains unclear which clinical situations will benefit from its advantages. In light of the uncomfortable surgeon position and fatigue often associated with pineal region surgery, we used the VITOM® HD exoscope system to perform an infratentorial supracerebellar resection of a pineal tumor. The VITOM® dramatically improved surgeon comfort and ease of operating by permitting the surgeon to stan

Giant nevocellular nevi with rickets and brainstem tumor.

A boy, 9 years, 8 months of age, with hairy pigmented nevi over the trunk, upper extremities, and face with bone deformities characteristic of rickets was admitted because of general weakness and poor appetite for 3 weeks. Repeated examinations demonstrated marked brainstem dysfunction; a brainstem tumor was visualized by computed tomography. The patient died 14 days after admission despite supportive treatment. The relationship between giant intradermal nevocellular nevi and brain tumor is discussed.

[Electric response audiometry (brain stem potentials and late potential N1) in patients with acoustic neurinoma or space-occupying lesion in the region of the brain stem].

In patients with acoustic neuronoma or space-occupying lesion in the region of the brainstem we recorded preoperatively the brainstem potentials as well as the late potential N1. With one exception all patients investigated showed changes in the brainstem potential pattern. When stimulating monaurally there exists a clear side difference, that means the changes in the potential pattern are more distinct on the side of the tumour than on the opposite side and then only if an obstructive hydrocephalus exists. Moreover we could demonstrate in patients with tumours in the region of the brainstem t

Stem Cell-Based Delivery of Immunomodulators for Overcoming Glioblastoma Immune Evasion.

Glioblastoma (GBM) is the most aggressive primary brain tumour in adults, characterised by rapid progression, extensive heterogeneity, and poor outcomes despite surgery, radiotherapy, and temozolomide (TMZ). A subpopulation of glioblastoma stem cells (GSCs) with self-renewal and multi-lineage differentiation capabilities drives tumour initiation, progression, recurrence, and therapeutic resistance. GSCs evade conventional treatments via enhanced DNA repair, multidrug efflux, activation of survival pathways, epigenetic reprogramming, and entry into quiescent states. Moreover, these cells utilis

Exploring the Dual Roles of Neural Stem Cells in Glioblastoma: Therapeutic Implications and Opportunities.

Glioblastoma (GBM) is recognized as the most aggressive and lethal form of primary brain tumor, characterized by rapid proliferation and significant resistance to conventional therapies. Recent studies have illuminated the complex role of Neural Stem Cells (NSCs) in both the progression and treatment of GBM. This review examines the specific molecular pathways influenced by NSCs, focusing on critical signaling cascades such as Notch, P13K, and SHH, which are implicated in tumor development and maintenance. Furthermore, we explore the dual role of NSCs in glioblastoma, where they can act as bot

Stem Cells and Targeted Gene Therapy in Brain and Spinal Cord Tumors.

The CNS tumors, in particular those with malignant characteristics, are prominent burdens around the world with high mortality and low cure rate. Given that, researchers were curious about novel treatments with promising effectiveness which resulted in shifting the dogmatism era of neurogenesis to the current concept of postnatal neurogenesis. Considering all existing stem cells, various strategies are available for treating CNS cancers, including hematopoietic stem cells transplantation, mesenchymal stem cells transplantation, neural stem cells (NSCs) transplantation, and using stem cells as
