Radiological evaluation of primary breast sarcoidosis presenting as bilateral breast lesions.

Sarcoidosis is an idiopathic multisystemic inflammatory disease that may exceptionally involve the breast and can have imaging features suspicious for benign or malignant lesions. Biopsy should be required to distinguish between breast sarcoidosis and malignancy, because clinical, mammographic and sonographic findings can be ambiguous or inconclusive. This case discusses the radiological manifestations and the value of different diagnostic features, and names the most relevant differential diagnosis.

Genetic Alterations in Benign Breast Biopsies of Subsequent Breast Cancer Patients.

Background: Fibrocystic changes are associated with an increased risk of breast cancer. Genetic alterations have been found in fibrocystic changes with or without epithelial changes, suggesting that critical oncogenic events are occurring at an early stage. Methods: We investigated a unique collective of 17 breast cancer patients who, prior to the diagnosis of invasive breast cancer, underwent open surgical biopsy showing fibrocystic changes of the breast. The time span between biopsy for fibrocystic changes and invasive carcinoma ranged from 1 to 11 years (average 5.3 years). Ten (58.8%) of t…

Delayed Breast Cellulitis following Surgery for Breast Cancer: A Literature Review.

BACKGROUND: Delayed breast cellulitis (DBC) is a relatively rare complication following breast-conserving surgery. It is often challenging to distinguish DBC from other clinical conditions such as postoperative infection, inflammatory reaction following radiation, and recurrent inflammatory carcinoma. The definition of DBC, diagnostic approach, and treatment are not well established in the literature. METHODS: We performed a literature search with the keywords 'Delayed breast cellulitis' and 'Breast conservation therapy cellulitis', without limitations to the dates or the article types, in th…

Localized morphea after breast implant for breast cancer: A case report.

PURPOSE: Early breast cancer follow-up guidelines for patients who underwent surgery suggest a regular and accurate clinical examination of the breast area, for an early identification of cutaneous or subcutaneous breast cancer relapse. Nonetheless, breast skin lesions arising in patients treated with mastectomy for breast cancer can be caused by several diseases. A series of diagnostic hypotheses should be considered, not only focusing on cutaneous metastasis, but also on dermatologic and systemic diseases. CASE REPORT: In February 2015, a 37-year-old patient underwent a right subcutaneous m…

[Modern surgical treatment of breast cancer. 3rd Breast Cancer Consensus Conference].

Therapy for breast cancer today is characterised by ever more precise diagnostic methods and ever more effective oncological treatments, a trend which will certainly continue into the future. Breast preservation and the application of oncoplastic principles are increasingly popular. A sentinel lymph node biopsy in the surgical treatment of the axilla is primary, with the indication for axillary block dissection (ABD) narrowing and radiation therapy becoming an alternative to ABD in certain cases. This publication summarises our recommendations on the surgical treatment of breast cancer based o…

Multifocal Bilateral Breast Cancer and Breast Follicular Lymphoma: A Simple Coincidence?

Breast cancer coexisting with lymphoma is a rare condition with various diagnostic and therapeutic implications. In this report, we describe the case of a 55-year-old Caucasian woman who presented with simultaneous ductal carcinoma in situ of the right breast, and follicular lymphoma involving an inguinal lymph node and the left breast. The patient underwent local excision and radiotherapy for the ductal carcinoma in situ, while a watch and wait strategy was adopted for the lymphoma. Two years later, the patient presented with multifocal ductal carcinoma of the left breast and reappearance of …

Detection of breast lesions using an automated breast volume scanner system.

OBJECTIVE: This study investigated the clinical utility of an automated breast volume scanner (ABVS) system for the detection of breast lesions. METHODS: The breasts of 81 patients referred for ultrasonographic examination were scanned using the ABVS system and hand-held ultrasonography independently by two experienced examiners. The ABVS was used to perform scans of the breast in three directions (anteroposterior, lateral and medial), with the addition of further inferior and superior scans if necessary. The scanning data were then stored and automatically reconstructed. For hand-held ultras…

Genetic variants associated with breast size also influence breast cancer risk.

BACKGROUND: While some factors of breast morphology, such as density, are directly implicated in breast cancer, the relationship between breast size and cancer is less clear. Breast size is moderately heritable, yet the genetic variants leading to differences in breast size have not been identified. METHODS: To investigate the genetic factors underlying breast size, we conducted a genome-wide association study (GWAS) of self-reported bra cup size, controlling for age, genetic ancestry, breast surgeries, pregnancy history and bra band size, in a cohort of 16,175 women of European ancestry. RE…

Automated whole-breast ultrasound: advancing the performance of breast cancer screening.

Screening has been successful in the early detection of some cancers, including cervical, colon, and breast. However, the success in mortality reduction by screening mammography has been limited in women with mammographically dense tissue. Magnetic resonance imaging has been used with success in the screening of high-risk women, but it is expensive. Also, its use in a screening program requires a contrast medium that has not been tested in humans or animals for long-term safety in humans or animals for repeated biannual or annual injections. Ultrasound also has the potential to be an ideal scr…

Frequency of breast cancer among Sudanese patients with breast palpable lumps.

BACKGROUND: Breast cancer mortality is high in Sudan and most patients are detected at later stages of the disease due to the lack of awareness and absence of screening programs. This study aimed to determine the pattern and frequency of breast cancer among patients presenting with palpable breast lumps within one year duration. SETTINGS AND DESIGN: As a part of the continuous development in cancer management, this descriptive longitudinal study was conducted in Khartoum, Sudan. METHODS AND MATERIALS: We obtained information (patient's personal data) and Fine-Needle Aspiration (FNA) material…

Preliminary result of accelerated partial breast irradiation after breast-conserving surgery.

PURPOSE: To investigate the feasibility of accelerated partial breast irradiation (APBI) for Japanese patients, we started high-dose-rate interstitial brachytherapy (HDR-ISBT) as monotherapy after breast-conserving surgery (BCS). METHODS: We implanted 45 Tis-2 breast cancer patients at National Hospital Organization Osaka National Hospital between June 2002 and June 2006. Our eligibility criteria were broader than the ones used previously in western countries. We included margin-positive cases and younger patients (median age: 44; range: 26-68) to adapt the criteria for Japanese women. Total …

Towards evidence-based management of inherited breast and breast-ovarian cancer.

Inherited breast-ovarian cancer was described in 1866. The underlying genetic defects in BRCA1/2 were demonstrated 128 years later. We now have 10 years of experience with genetic testing in BRCA kindreds. The majority of breast cancer kindreds (familial breast cancer) do not demonstrate ovarian cancer and are not associated with BRCA mutations. The effect of early diagnosis and treatment is monitored through international collaborations.BRCA1-associated breast cancer is biologically different from other breast cancers, including a worse prognosis. BRCA2-associated breast cancer is, beside ear…

Breast conservation surgery using nipple-areolar resection for central breast cancers.

HYPOTHESIS: Resection of the nipple-areolar complex (NAC) for central breast cancers that involve the nipple or areola, with postoperative radiation therapy, adheres to the oncologic principles established for breast conservation surgery of other breast cancers. Good or excellent cosmetic results can be achieved. The rate of ipsilateral breast recurrence will be similar to that seen with peripheral breast cancers. The indications for breast conservation surgery can be safely extended to include patients with breast cancers that involve the NAC. DESIGN: Retrospective medical record review; fol…

[Treatment results of breast cancer patients who hoped for breast preservation].

From 1983 to August 1990, 143 patients (148 breasts) were treated with breast conservation therapy in our department. Age ranged from 27 to 75 years (median: 45 years). Forty-one were stage I, 91 were stage II, 6 were stage III, and 5 were stage IV. Surgery consisted of lumpectomy and axillary dissection up to level II. When axillary lymph node(s) were pathologically involved (n(+)), chemotherapy (CMF) was performed. Radiation dose was 50 Gy to the entire breast, and booster irradiation was not performed. After October 1989, patients without palpable axillary node(s) underwent lumpectomy witho…

Hypernatremia in breast-fed infants due to elevated breast milk sodium.

The author describes severe hypernatremic dehydration in a 12-day-old, breast-fed infant. The mother's breast milk sodium level at the 13th day of the infant's life was 47 mEq/L, more than three times normal value. At the 30th day, the mother's breast milk sodium level was 16 mEq/L, still more than twice the normal value. To the author's knowledge, this is the first time in a case of this type that the breast milk sodium value of the mother has been followed this late in the infant's life. Results are consistent with the hypothesis that delayed maturation of breast milk composition is of centr…

Monoclonal antibodies, benign breast disease, and breast cancer. A panel discussion.

Hybridoma technology has now advanced to the point where hundreds of monoclonal antibodies have been produced and are reported to bind to normal and malignant cell surfaces or intracellular organelles. The original hope for tumor-specific antigens has not materialized, but certain antibodies with rather restricted ranges of binding activities have been identified. This panel discusses the use of such monoclonal antibodies in diagnosis, prognosis, and treatment of human breast disease and breast cancer.

Effects of public education about breast cancer and breast self-examination.

In a field experiment severity of and susceptibility to breast cancer were varied in four different persuasive messages about breast cancer and breast self-examination (BSE). The purpose of this study was to find out whether such health messages in a real-life setting had an effect on knowledge about symptoms, attitudes and behavior relating to BSE. The second goal of this study was to investigate whether fear, aroused by these persuasive communications, had a significant role in influencing the recommended behavior (BSE). In spite of a successful manipulation of seriousness and susceptibility…

Surgical considerations in managing the remaining breast during postmastectomy breast reconstruction.

The age and general condition of the patient, size and shape of the remaining breast, various risk factors such as the histologic type of carcinoma, and family history are all critical in governing the definitive management of the remaining breast during postmastectomy breast reconstruction. The philosophy, timing, and method of dealing with the remaining breast must be discussed carefully with the patient. The psychological, social, and physical status of the patient must be addressed, with special emphasis in regard to her future and the degree of risk involved.

Breast cancer and preceding clinical benign breast disorders. A chance association.

A review of 1059 patients seen in office consultation from 1970 to 1975 for breast disorders revealed that patients with clinically benign disorders were six times as numerous as those with breast cancer. Only 11% of patients with breast cancer had had a previous benign breast-biopsy specimen and on average they were 20 years older than the peak age for benign clinical disease. Of the 163 patients with a previous benign breast-biopsy specimen only 14 (9%) had breast cancer compared with 13% in women without such a history. It is concluded that clinical benign breast disorders do not predispose…

Accuracy of Abbreviated Breast MRI in Diagnosing Breast Cancer in Women with Dense Breasts Compared with Standard Imaging Modalities.

BACKGROUND: Breast density is an independent risk factor for breast cancer and affects the sensitivity of mammography screening. Therefore, new breast imaging approaches could benefit women with increased breast density in early cancer detection and diagnosis. OBJECTIVES: To assess the diagnostic performance of abbreviated breast MRI compared with mammography and other imaging modalities in screening and diagnosing breast cancer among Saudi women with dense breast tissue. METHODS: A retrospective diagnostic study was conducted using anonymized medical images and histopathology information fr…
