Catheter ablation of right-sided para-Hisian ventricular arrhythmias using a simple pacing strategy.

BACKGROUND: Ablation of para-Hisian ventricular arrhythmias (PH-VAs) is challenging because of the close relationship of the origin site and His bundle. Using pacing techniques to differentiate the near-field from far-field His activations, thereby avoiding atrioventricular block, has been reported in patients with para-Hisian accessory pathways. OBJECTIVES: We applied the same pacing technique and 3-dimensional mapping to guide radiofrequency (RF) ablation in patients with right-sided PH-VAs and investigated the clinical outcome of such cases. METHODS: Fourteen patients with right-sided PH-

Ventricular standstill complicating cardiac catheterization.

A patient is described in this paper who developed repeated episodes of ventricular standstill during left heart catheterization. The patient had a resting pattern of right bundle branch block with left anterior fascicular block; subsequent electrophysiology study was normal. Ventricular standstill during catheterization was apparently due to catheter-related trauma of His bundle connections.

Rhabdomyoma of the heart producing right bundle branch block.

A 4-day-old female had an uneventful prenatal course and delivery. Immediate cyanosis improved with oxygen therapy. Cardiomegaly on chest radiograph, right bundle branch block on electrocardiogram, and apparent increased tricuspid valve excursion on echocardiography led to a diagnosis of Ebstein's anomaly. Death occurred unexpectedly. Autopsy demonstrated a rhabdomyoma of the interventricular septum, which had produced obstruction to right ventricular outflow and had interrupted the right bundle branch of the conducting system. It appears that the massive size of the septal tumor led to its mi

Exact location of the branching bundle in the living heart.

AIMS: The His bundle electrogram is believed to reflect the exact location of the His bundle. However, the distinction between distal His bundle potential and proximal right bundle branch potential is challenging. The aim of this study was to pinpoint the location of the branching point of the His bundle, and to compare that site with the site of recording of the largest His bundle electrogram (LH) during sinus rhythm. METHODS: We hypothesized that the site of earliest His activation (EH) during retrograde conduction via the left bundle branch is the branching point. We studied 15 nonconsecut

Fascicular ventricular tachycardia arising from the left side His and its adjacent region: a subset of upper septal idiopathic left ventricular tachycardia.

AIMS: Fascicular ventricle tachycardia (FVT) arising from the proximal aspect of left His-Purkinje system (HPS) has not been specially addressed. Current study was to investigate its clinical, electrocardiographic, and electrophysiological characteristics. METHODS AND RESULTS: Eighteen patients who were identified as this rare FVT were consecutively enrolled, and their scalar electrocardiogram and electrophysiological data were collected and analysed. The ventricular tachycardia (VT) morphology was similar to sinus rhythm (SR) in eight patients, left bundle branch block type in one patient, r

Early dose of Adenosine, postRadiofrequency abLation of accessory pathwaY in determining acute procedural success (EARLY study).

INTRODUCTION: Post ablation of the accessory pathway (AP), the patient is observed in the catheterization laboratory for a variable period for resumption of pathway conduction. Aim of the study was to determine whether the administration of intravenous adenosine at 10 min after ablation of AP would have the same diagnostic accuracy as waiting for 30 min in predicting the resumption of AP conduction. METHODS: This was a prospective interventional study conducted in two centers. Post ablation of the AP, intravenous adenosine was administered at 10 min to look for dormant pathway conduction. The

Current of injury is an indicator of lead depth and performance during left bundle branch pacing lead implantation.

BACKGROUND: Monitoring of lead depth is crucial to achieve left bundle branch pacing (LBBP) with a low capture threshold and avoid septal perforation, but lacks informative approach. OBJECTIVE: We aimed to prospectively assess the predictive value of current of injury on the occurrence of inadequate left bundle branch (LBB) capture threshold and acute septal perforation. METHODS: Consecutive patients who received LBBP were enrolled. ST-segment elevation ≥ 25% of intrinsic R-wave amplitude on the unipolar intracardiac electrogram was defined as a sign of distinct current of injury. An LBB cap

M-beat-A novel marker for selective left bundle branch capture.

INTRODUCTION: Premature-ventricular-complexes (template/fixation beat) guided left bundle branch pacing (LBBP) was recently described as a novel method of successful lead deployment by rapid rotations. METHODS: We aimed at analyzing the incidence of a unique morphology template beat, which we labelled as 'M-beat' in patients undergoing PVC-guided LBBP, its ability to predict selective LBB-capture and clinical significance. RESULTS: Overall 210 out of 217 attempted-patients (96.7%) underwent successful LBBP. Template beat was noted in 90.4% patients (n = 190) and M-beat in 32.8%(n = 69). Non-

Quantitative distance and electrocardiographic parameters for lead-implanted site selection to enhance the success likelihood of left bundle branch pacing.

BACKGROUND: Left bundle branch pacing (LBBP) is a novel near-physiological pacing method that still lacks quantitative criteria to guide the selection of lead-implanted sites to enhance the success likelihood of lead deployments. This study aimed to quantitatively analyze the relationships of LBBP success likelihood to the distribution of lead-implanted sites and the lead-localization-pacing electrocardiographic (ECG) features. METHODS: All the lead-implanted sites in patients with finally successful LBBP were enrolled for analysis, including successful and failed sites. A novel coordinate sy

A rare case of narrow QRS tachycardia with "hidden" concealed left-sided accessory pathway.

A 54-year-old-woman with narrow QRS tachycardia (heart rate, 210 beats per minute) was admitted to the authors' hospital. Tachycardia terminated after administration of adenosine. The 12-lead electrocardiogram after conversion to sinus rhythm was unremarkable. During electrophysiological study, tachycardia could be easily induced by programmed atrial stimulation without jump phenomenon. The cycle length was 260 ms. During tachycardia, the atrial activation sequence in the coronary sinus (CS) was distal-to-proximal. However, during decremental ventricular pacing, retrograde conduction showed ex

Repetitive polymorphic ventricular tachycardia initiated by phase 4 block in the His-Purkinje system.

We report a case of a patient who developed repetitive episodes of polymorphic ventricular tachycardia with a stereotypical pattern of initiation. A premature atrial complex would result in a brief pause followed by left bundle branch block aberrancy. Ventricular bigeminy would ensue followed by episodes of polymorphic ventricular tachycardia, some requiring cardioversion. We postulate that delay within the His-Purkinje system initiated by phase 4 block was proarrhythmic in this patient.

[Electrocardiographic criteria for the diagnosis of the left septal fascicular block and its frequency among primarily elderly hospitalized patients].

Characteristic electrocardiographic findings of the left septal fascicular block consist of a prominent anterior QRS force. Therefore, the following criteria were proposed for the diagnosis of left septal fascicular block, based on the normal limits of the R and S waves and R/S of V1 and V2. (1) Right ventricular hypertrophy, complete right bundle branch block, preexcitation (Type A), high posterior infarction, hypertrophic cardiomyopathy and abnormality of the thorax or intrathoracic tissues which might cause marked counterclockwise rotation around the longitudinal axis of the heart, should b

Left bundle branch pacing vs biventricular pacing in heart failure patients undergoing cardiac resynchronization therapy: an updated systematic review and meta-analysis.

INTRODUCTION: To compare the efficacy and safety of left bundle branch pacing (LBBP) versus biventricular pacing (BVP) in patients with heart failure undergoing cardiac resynchronization therapy (CRT), we conducted an updated systematic review and meta-analysis. METHODS: PubMed, Cochrane Library, ScienceDirect, and Embase, were searched through February 2026. Randomized controlled trials and cohort studies comparing LBBP with BVP were included. Risk of bias was assessed using RoB 2.0 for randomized trials and ROBINS-I for observational studies. Data were pooled using fixed- or random-effects

[The left-fascicular blocks ("hemiblocks")].

The left ventricular impulse conduction system is composed of two, three or more fasciculi. The blocking of the impulse in one fasciculus is termed a left fascicular block or "hemiblock" for short. The anatomical variations of the left ventricular impulse conduction system, and the possibility of partial blocking, makes the electrocardiographic diagnosis more difficult. The ECG criteria of left fascicular blocking - with special regard to the left anterior fascicular block - and their combinations with the right block (bilateral bifascicular block) are discussed. A bilateral trifascicular bloc
