Dihydropyrimidine-2-thiones as Eg5 inhibitors and L-type calcium channel blockers: potential antitumour dual agents.

The use of multitarget drugs has evolved as an alternative to "magic bullets" for the treatment of complex diseases such as cancer, in order to affect simultaneously several targets relevant to the disease. We have designed and synthesized a series of dual agents as both Eg5 inhibitors and calcium channel blockers, bearing a 4-aryldihydropyrimidine core. Compound 2 (aryl: 3-nitrophenyl) was selected as potential dual agent due to displaying both activities: it is a vasorelaxant agent (>90% relaxation at 10-5 M in KCl-precontracted aorta rings), it decreases the response to calcium and it is cy…

Synthesis, Docking Simulation, Biological Evaluations and 3D-QSAR Study of 1,4-Dihydropyridines as Calcium Channel Blockers.

Resurgence to target L-type voltage-dependent calcium channels has been applied by the synthesis of two series of nifedipine analogues where the ortho- or a meta-nitrophenyl ring is retained. A pre-synthetic molecular docking study with a receptor model followed by molecular alignment has been performed on 47 compounds to predict the most active member. The IC50 values revealed that some of the compounds are similar to or more active than nifedipine. Substitution of groups at the 3- and 5-positions of the dihydropyridine (DHP) ring gave 3k, which is more active than nifedipine. Our valid three

Calcium channel blockers and cancer: a risk analysis using the UK Clinical Practice Research Datalink (CPRD).

OBJECTIVE: The evidence of an association between calcium channel blockers (CCBs) and cancer is conflicting. The objective of the present study was to evaluate the risk of cancer (all, breast, prostate and colon cancers) in association with exposure to CCB. METHODS: This is a population-based cohort study in patients exposed to CCBs from across the UK, using two comparison cohorts: (1) patients with no exposure to CCB (non-CCB) matched on age and gender and (2) unmatched patients unexposed to CCB and at least one other antihypertensive (AHT) prescription. Cancer incidence rates computed in th…

Effect of T-type calcium channel blockers on spiral ganglion neurons of aged C57BL/6J mice.

To explore the expression levels of T-type calcium channel receptors in spiral ganglion neurons of C57BL/6J mice and the effect of T-type calcium channel blockers on the spiral ganglion neurons of 42-44-W C57BL/6J mice. We first quantified the subunits of T-type calcium channel blockers in the spiral ganglion neurons of C57BL/6J mice in three groups (6-8 W, 24-26 W, 42-44 W) according to age via RT-PCR. Next, we administered three drugs (zonisamide, felodipine, saline) to the 42-44-W C57BL/6J mice by gavage for four weeks. We observed the changes in the hearing threshold of 42-44-W C57BL/6J mi…

The pharmacological differences in anti-anginal effects of long-lasting calcium channel blockers: azelnidipine and amlodipine.

ABSTRACT: We examined anti-anginal effects of azelnidipine and amlodipine in an arginine vasopressin (AVP)-induced rat anginal model. Oral administration of azelnidipine or amlodipine produced long-lasting inhibition of AVP-induced ST-segment depression in ECG. The degrees of inhibition with azelnidipine at doses of 1 and 3 mg/kg were comparable to those with amlodipine at 3 and 10 mg/kg. Both drugs lowered mean blood pressure in a dose related manner, while only azelnidipine decreased heart rate. Azelnidipine at 3 mg/kg and amlodipine at 10 mg/kg produced a similar decrease in the rate pressu

Current use of calcium channel blockers (CCBs) is associated with an increased risk of gingival hyperplasia.

SUBJECTS: This is a nested case-control study within a cohort of all patients who were new users of either calcium channel blockers (CCBs) or drugs interacting with the renin-angiotensin system (RAS). The sample was obtained from the Integrated Primary Care Information (IPCI) database in the Netherlands and consisted of new users of either CCBs (dihydropyridine derivatives, phenylalkylamine derivatives, and benzothiazepine derivatives) or RAS drugs (angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers). The source population comprised all subjects 18 years and older who…

[Severe pulmonary complications of massive intoxication with calcium channel blockers and isosorbide mononitrate--a case report].

A 22-year-old man was admitted to the acute intoxication unit after suicidal intoxication with 100 tablets of verapamil, diltiazem and isoso … all components of the surfactant. Surfactant secretion in epithelial type II cells is inhibited by calcium channel blockers and it predisposes to develop ARDS. This case illustrates the possibility of ARDS in the verapamil and diltiazem intoxication and the need for a heightened awareness of this potential complication.

Calcium channel blockers reduce blood pressure in part by inhibiting vascular smooth muscle carbonic anhydrase I.

Calcium channel blockers are a group of drugs used for the treatment of hypertension. Carbonic anhydrase (CA) I detected in vascular smooth muscle and in other cells in the organism has a major role in the acid-base balance and in vascular processes. Our previous work has proven that verapamil inhibits CA activity by a direct mechanism of action. Starting from our results in this article we studied in vitro and in vivo the effect of calcium channel blockers (verapamil and amlodipine) on erythrocyte CA I, on vascular smooth muscles CA I, and on arterial blood pressure values in human and in ani…

Effect of pre-treatment of some calcium channel blockers on catalepsy and stereotypic behaviour in rats.

Effect of pretreatment of intraperitoneally administered Ca-channel blockers Nifedipine (5, 10, 20 mg/kg). Verapamil (5, 10, 20 mg/kg) and Diltiazem (5, 10, 20 mg/kg) was studied on Haloperidol-induced catalepsy and Methamphetamine-induced stereotypy in albino rats. All these drugs reduced the onset of catalepsy, significantly increased the cataleptic score and delayed the onset and inhibited the Methamphetamine-induced stereotypy. The possible involvement of dopaminergic and adrenergic mechanisms and modification by Ca-channel blockers are discussed.

Beneficial effects of calcium channel blockers and calmodulin binding drugs on in vitro renal cell anoxia.

Proximal tubules (PSTs) of the S1, S2 and S3 segments and cortical collecting tubules (CCTs) were microdissected individually from rabbit kidneys and cultured for 7 days in hormonally defined media. Anoxia was induced by incubation of cultures in normal medium for 45 min at 25 degrees C in an atmosphere of nitrogen and cell death was measured by nigrosine dye uptake. After 45 min of anoxia and a 4- to 6-hr incubation in normal Ca++-containing media, cells from all segments were dead. Addition of calcium channel blockers verapamil and nifedipine (5 X 10(-7) and 10(-6) M, respectively) for the f…

Age- and Sex-Related Differences in Efficacy With an Angiotensin II Receptor Blocker and a Calcium Channel Blocker in Asian Hypertensive Patients.

Overseas guidelines to manage hypertension recommend selecting different drugs depending on age, but no studies have investigated the relationship between drug selection and age- and sex-related differences, although such information may help to reduce the risk of cardiovascular mortality. The Azilsartan Circadian and Sleep Pressure--the First Study (ACS1) trial was a multicentered, randomized, open-label, two-parallel group study comparing the effects of an angiotensin II receptor blocker (azilsartan) and a calcium channel blocker (amlodipine). The present study is a post hoc analysis of ACS1

Rationale for triple fixed-dose combination therapy with an angiotensin II receptor blocker, a calcium channel blocker, and a thiazide diuretic.

Hypertension is a growing global health problem, and is predicted to affect 1.56 billion people by 2025. Treatment remains suboptimal, with control of blood pressure achieved in only 20%-35% of patients, and the majority requiring two or more antihypertensive drugs to achieve recommended blood pressure goals. To improve blood pressure control, the European hypertension guidelines recommend that angiotensin II receptor blockers (ARBs) or angiotensin-converting enzyme inhibitors (ACEIs) are combined with calcium channel blockers (CCBs) and/or thiazide diuretics. The rationale for this strategy i…

Unexpected synergy: Calcium channel blockers and residual chlorine cooperatively accelerate antibiotic resistance dissemination in water systems.

The dissemination of antibiotic resistance driven by non-antibiotic pharmaceuticals is an emerging concern warranting further investigation. Calcium channel blockers (CCBs), frequently detected in aquatic environments, have rarely been studied for their role in the spread of antibiotic resistance genes (ARGs). This study examines the impact of two representative CCBs, amlodipine (AML) and verapamil (VER), on conjugative horizontal gene transfer (HGT) under low-level chlorine conditions. A bacterial conjugation system was established using Escherichia coli harboring the conjugative plasmid RP4,…

Psychiatric Adverse Events Associated With Dihydropyridine Calcium Channel Blockers: A Pharmacovigilance Study Based on FAERS Database.

OBJECTIVES: Dihydropyridine calcium channel blockers (DHPCCBs) are widely employed in managing cardiovascular diseases. Although some studies suggest a potential link to psychiatric adverse events (PAEs), a systematic investigation is lacking. METHODS: Adverse event reports for seven DHPCCBs marketed in the United States were collected from the FDA Adverse Event Reporting System (FAERS). PAEs were extracted from these reports and then performed using disproportionality analysis. RESULTS: A total of 9164 PAEs associated with 7 DHPCCBs were identified, revealing 18 positive signal associations…

Inhibitory effects of calcium channel blockers nisoldipine and nimodipine on ivacaftor metabolism and their underlying mechanism.

Ivacaftor is the first potentiator of the cystic fibrosis transmembrane conductance regulator (CFTR) protein approved for use alone in the treatment of cystic fibrosis (CF). Ivacaftor is primarily metabolized by CYP3A4 and therefore may interact with drugs that are CYP3A4 substrates, resulting in changes in plasma exposure to ivacaftor. The study determined the levels of ivacaftor and its active metabolite M1 by ultra performance liquid chromatography tandem mass spectrometry (UPLC-MS/MS). We screened 79 drugs and 19 severely inhibited ivacaftor metabolism, particularly two cardiovascular drug
