Post-acute cardiac complications following SARS-CoV-2 infection in partial lipodystrophy due to LMNA gene p.R349W mutation.

PURPOSE: SARS-CoV-2 infection may cause varying degrees of cardiac injury and the presence of underlying cardiovascular morbidities contributes to the frequency and severity of occurrence of this complication. Lipodystrophy syndromes are frequently characterized by severe metabolic derangements that represent relevant cardiovascular risk factors. Besides causing lipodystrophy, mutations in the lamin A/C (LMNA) gene can lead to a wide spectrum of tissue-specific disorders including cardiac involvement. METHODS AND RESULTS: We herein examine the case of two patients affected by atypical progero

Neutrophil extracellular traps and cardiovascular disease: Associations and potential therapeutic approaches.

Cardiovascular disease (CVD) is a significant global health concern, ranking among the top five causes of disability-adjusted life-years (DALY) in 190 countries and territories. Neutrophils, key players in the innate immune system, combat infections by releasing neutrophil extracellular traps (NETs) composed of DNA, histones, elastase, myeloperoxidase, and antimicrobial peptides. This paper explores the relationship between NETs and cardiovascular diseases, focusing on conditions such as heart failure, pulmonary hypertension, atrial fibrillation, and ischemia-reperfusion injury. Particularly,

[Effectiveness of Prophylactic Negative Pressure Wound Therapy after Cardiovascular Surgery through Median Sternotomy].

Negative pressure wound therapy( NPWT) is used primarily for tissue defects. In recent years, cardiovascular surgery via full sternotomy is increasingly performed through small incisions, but the rate of cardiovascular surgery through median sternotomy remains high in elderly patients, who frequently have complicated cardiovascular diseases. Mediastinitis, among other surgical site infections( SSIs), is a serious complication after cardiovascular surgery that must be resolved. Mediastinitis has a high mortality rate once it occurs, and cost of treatment and a negative impact on a patient are s

Treatments for COVID-19 and acute respiratory infections are associated with gender and comorbidities in an Italian online survey.

Ministries of health need to know and understand factors affecting medical treatment choices of population subgroups, to tailor official recommendations. This study aimed to identify, quantify and compare treatments used by patients with an acute respiratory infection with and without COVID-19 during the first year of the pandemic by accounting for important factors potentially affecting treatment outcomes. With an online questionnaire, retrospective information on treatments used during events of acute respiratory infections were gathered in Italy. Logistic regression was used to detect signi

[Periodontal disease, a risk factor for atherosclerotic cardiovascular disease].

Cardiovascular disease is recognized as the leading cause of death and disability in the world. The majority of these deaths can be attributed to atherosclerotic disease and thromboembolic events leading to ischemic heart disease and stroke. The role of inflammation is well recognized in the pathogenesis of atherosclerosis and atherothrombosis. Increasing number of studies support the hypothesis that periodontal disease, specifically periodontitis, is a potential risk factor for atherosclerosis and thus cardiovascular disease. Chronic infections of periodontal pockets act as reservoirs for pat

Effectiveness of nirmatrelvir/ritonavir and molnupiravir in reducing the risk of short-term and long-term cardiovascular complications of COVID-19: a target trial emulation study.

While treatment with nirmatrelvir/ritonavir or molnupiravir is effective in lowering the rate of severe COVID-19, the effectiveness of these antivirals in reducing the risk of cardiovascular outcomes, especially among the hospitalized population, remains largely unknown. In this study, we assessed the real-world effectiveness of nirmatrelvir/ritonavir and molnupiravir on short- and long-term cardiovascular complications of COVID-19 using a target trial emulation design. Two target trials of COVID-19 antivirals were emulated by using a territory-wide, population-based, retrospective cohort of h

[Cardiovascular risk factors associated with antiretroviral therapy].

HIV-infected patients share traditional cardiovascular risk factors with the general population. Moreover, HIV per se significantly increases the risk of cardiovascular disease. Consequently, control of HIV infection with highly-active antiretroviral therapy (HAART) helps to reduce vascular risk in HIV-infected patients. However, there is evidence that HAART increases cardiovascular risk and the use of abacavir, didanosine, lopinavir/ritonavir and indinavir has been associated with the development of acute myocardial infarction (AMI). However, the absolute risk of AMI in patients receiving HAA

Comparative Effectiveness of Sodium-Glucose Cotransporter-2 (SGLT2) Inhibitors Versus Glucagon-Like Peptide-1 (GLP-1) Agonists on Cardiovascular and Renal Outcomes in Type 2 Diabetes: A Systematic Review and Network Meta-Analysis.

Sodium-glucose cotransporter-2 (SGLT2) inhibitors and glucagon-like peptide-1 (GLP-1) receptor agonists reduce cardiovascular and renal risks in type 2 diabetes mellitus (T2DM), but their relative efficacy remains uncertain due to the absence of direct comparative trials. This systematic review and network meta-analysis aimed to evaluate the efficacy and safety of interventions concerning major adverse cardiovascular events (MACE), heart failure, and renal outcomes. A systematic review and network meta-analysis of large-scale, placebo-controlled cardiovascular outcome trials was conducted. Pub

N-terminal pro-B-type natriuretic peptide (NT-proBNP) in HIV-1 infected individuals on HAART.

BACKGROUND: Studies suggest that highly active anti-retroviral therapy (HAART) prolongs life in HIV infected individuals and that HIV infected individuals increasingly suffer from cardiovascular complications. NT-proBNP has been shown to represent an indicator of cardiac function. METHODS: 495 HIV infected individuals under HAART and 1980 blood donors (BD) were tested for N-terminal pro-B-type natriuretic peptide (NT-proBNP). NT-proBNP was performed by an automated electrochemiluminescence immunoassay (ECLIA) method. RESULTS: HIV infected individuals had significantly higher NT-proBNP levels

Prognostic value of C-reactive protein for heart disease in dialysis patients.

C-reactive protein (CRP) is considered to be the prototype marker of inflammation. In the general population, there are ample clinical and epidemiological data that indicate its usefulness both in predicting the prognosis for various forms of cardiovascular disease, and in monitoring response to treatment. There is also evolving evidence that CRP may be directly involved in the pathological disease process itself. In end-stage renal disease (ESRD) patients, cardiovascular disease remains the leading cause of morbidity and mortality, and is accounted for by a clustering of both traditional and

A care pathway for the cardiovascular complications of COVID-19: Insights from an institutional response.

The infection caused by severe acute respiratory syndrome coronavirus-2, or COVID-19, can result in myocardial injury, heart failure, and arrhythmias. In addition to the viral infection itself, investigational therapies for the infection can interact with the cardiovascular system. As cardiologists and cardiovascular service lines will be heavily involved in the care of patients with COVID-19, our division organized an approach to manage these complications, attempting to balance resource utilization and risk to personnel with optimal cardiovascular care. The model presented can provide a fram

Cardiovascular damage and comorbidities related to long COVID: pathomechanisms, prevention, and therapy.

Long COVID (LC) is a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection-associated chronic condition and is present for at least 3 months as a continuous, relapsing and remitting, or progressive disease state that affects one or more organ systems, including cardiovascular. Extensive literature supports an association between SARS-CoV-2 infection and cardiovascular complications and increased cardiovascular risk after infection. The cardiovascular sequelae after SARS-CoV-2 infection have not yet been comprehensively characterized. A growing body of evidence suggests that en

Familial hypercholesterolemia: The nexus of endothelial dysfunction and lipoprotein metabolism in COVID-19.

PURPOSE OF REVIEW: Patients with heterozygous familial hypercholesterolemia (HeFH) are at increased risk for COVID-19 cardiovascular complications in the acute phase of the infection. Elevated levels of LDL-C and often lipoprotein(a) are present from birth and lead to endothelial dysfunction, which is aggravated by a direct viral attack of the endothelial cells and their exposure to the toxic levels of circulating proinflammatory and prothrombotic mediators during the hyperinflammatory reaction typical of COVID-19. RECENT FINDINGS: Evidence to date shows the benefit of lipid-lowering therapy

Cardiovascular Complications of Seasonal Influenza in the Pre- and Post-COVID-19 Era: Epidemiology, Mechanisms, and Clinical Implications.

Influenza has long been a well-documented contributor to cardiovascular morbidity and mortality, particularly among high-risk groups. COVID-19 has notably altered the seasonality and natural history of pandemic influenza, with broad implications for related cardiac complications. This review examines the interaction between influenza and cardiovascular illness, especially myocardial infarction, congestive heart failure, stroke, and other acute cardiac events. We review the impact of the COVID-19 pandemic on influenza transmission dynamics, public health policy, and the evolving burden of cardi

Eleven anthropometric indicators are associated with Framingham risk score in men but not in women living with HIV.

Metabolic and morphological changes caused in people living with HIV (PLHIV) by infections and antiretroviral therapy increase cardiovascular risk. Anthropometric indicators are low-cost tools for cardiovascular risk assessment. This cross-sectional study evaluated the association between 11 anthropometric indicators and cardiovascular risk in 354 cis-gender PLHIV of both sexes ≥18 years, attending a public outpatient clinic in northeastern Brazil. Sociodemographic, clinical, personal, and anthropometric data were collected. Cardiovascular risk was determined using the Framingham risk score, a

Fluoroquinolones: Neurological Complications and Side Effects in Clinical Practice.

Fluoroquinolones, a popular antibiotic class that inhibits nucleic acid synthesis of bacteria by disrupting the activity of the enzyme's topoisomerase IV and DNA gyrase, are used to treat bacterial infections. However, the widespread use of these drugs has allowed for the development of microbial resistance in recent years. Quinolones also have many clinically relevant side effects, including psychosis, confusion, seizures, headaches, dizziness, and nausea. Common side effects include tendinitis, myopathy, depression, and fatigue. Cardiovascular side effects include increased risk of aortic an

A complete review of empagliflozin: Most specific and potent SGLT2 inhibitor used for the treatment of type 2 diabetes mellitus.

Sodium-glucose co-transporter 2 (SGLT2) inhibitors are the latest class of drugs to be introduced for the treatment of type 2 diabetes mellitus (T2DM). They reduce hyperglycemia by increasing urinary glucose excretion and exert favorable effects beyond glucose control with consistent body weight, blood pressure, and serum uric acid reductions. Empagliflozin is a potent SGLT2 inhibitor used to improve glycemic control in adults with T2DM. It has the highest SGLT2 specificity among all the clinically used or currently tested SGLT2 inhibitors. Low risk of hypoglycemia, absence of weight gain and
