Cauda equina syndrome after in situ arthrodesis for severe spondylolisthesis at the lumbosacral junction.

Relative stretching of the cauda equina over the posterosuperior border of the sacrum can be found in all patients who have Grade-III or IV spondylolisthesis at the lumbosacral junction. We identified twelve patients, all less than eighteen years old, who had cauda equina syndrome after in situ arthrodesis for Grade-III or IV lumbosacral spondylolisthesis. In all twelve patients, posterolateral arthrodesis had been done bilaterally through a midline or paraspinal muscle-splitting approach. Nothing in the operative reports suggested that the cauda equina had been directly injured during any of …

Cauda equina syndrome caused by isolated spinal extramedullary-intradural cauda equina metastasis is the primary symptom of small cell lung cancer: a case report and review of the literatrure.

OBJECTIVES: To describe a case of extramedullary-intradural metastases causing cauda equina syndrome (CES) as the primary syndrome of lung cancer, and to review the pertinent medical literature. SUMMARY OF BACKGROUND DATA: Intradural spinal metastasis is rare, accounting for 6% of all spinal metastases. Of all primary lesion types, lung carcinoma is about 40-85%. Extramedullary-intradural metastases of lung cancer causing CES is unusual. METHODS: A 55-year-old patient with symptoms of low back pain that radiated to bilateral lower legs with painful paresthesias and piecemeal sphincter distur…

Beyond Recovery: Long-Term Multidomain Disability and the Satisfaction Paradox in Cauda Equina Syndrome.

BACKGROUND CONTEXT: Long-term outcomes after cauda equina syndrome (CES) are commonly reported using isolated endpoints such as bladder recovery, which may underestimate the true burden of disease. CES affects multiple functional domains, yet its cumulative long-term impact remains poorly characterised. PURPOSE: To evaluate long-term outcomes after CES using a multidomain framework, quantify cumulative functional burden, examine discordance between residual deficits and patient-perceived recovery, and identify factors associated with adverse outcome. STUDY DESIGN/SETTING: Retrospective cohor…

Acute Aortic Occlusion Mimicking Cauda Equina Syndrome: Complete Neurologic Recovery After Emergent Endovascular Revascularization.

A 67-year-old man with a history of bilateral iliac stenting presented for acute right lower extremity weakness, numbness, and severe pain after exertion. Femoral and distal pulses were absent bilaterally. Initial suspicion during evaluation was for Cauda Equina Syndrome. However, computed tomography angiography of the abdomen and pelvis revealed an acute thrombotic occlusion of the distal abdominal aorta extending into both common iliac arteries with a 3.1-cm aneurysm. Emergency vascular intervention was performed, consisting of over-the-wire thrombectomy and endovascular stenting with adjunc…

Atypical presentation of L3 vertebral body osteochondroma mimicking cauda equina syndrome: a case report.

BACKGROUND: Osteochondromas are common benign bone tumors, but spinal involvement is rare (<4%), with vertebral body origin being exceptionally uncommon. Neurological symptoms are typically mild and unilateral, making presentations mimicking incomplete cauda equina syndrome (CES) exceedingly rare. CASE PRESENTATION: A 58-year-old Indian male presented with progressive lower back pain, followed by bilateral lower limb weakness and saddle anesthesia. There was no history of trauma, radiation, malignancy, or inflammatory disease. Neurological examination revealed flaccid paraplegia, areflexia, a…

Long-term rates of bladder dysfunction after decompression in patients with cauda equina syndrome.

BACKGROUND CONTEXT: Cauda equina syndrome (CES) occurs due to compression of the lumbar and sacral nerve roots and is considered a surgical emergency. Although the condition is relatively rare, the associated morbidity can be devastating to patients. While substantial research has been conducted on the timing of treatment, the literature regarding long-term rates of bladder dysfunction in CES patients is scarce. PURPOSE: The aim of this study was to identify long-term rates of bladder dysfunction in CES patients and to compare those rates to non-CES patients who underwent similar spinal decom…

Cauda equina syndrome without motor dysfunction following lumbar spinal stenosis surgery: A case report.

RATIONALE: Cauda equina syndrome (CES) refers to a group of symptoms that occur when the nerves in the cauda equina become compressed or damaged. The most common etiology of CES is lumbar intervertebral disc herniation, but CES following lumbar spinal surgery is rare, especially without motor dysfunction. Herein, we illustrate a case of CES that developed as a complication of spinal surgery and to deduce its possible underlying cause. PATIENT CONCERNS: A 46-year-old man experienced lumbago, bilateral shank pain, and numbness with neurogenic claudication for 3 years due to degenerative lumbar …

Bladder Scans and Postvoid Residual Volume Measurement Improve Diagnostic Accuracy of Cauda Equina Syndrome.

STUDY DESIGN: A prospective, observational cohort study. OBJECTIVE: The aim of this study was to determine the role of pre and postvoid bladder scan in predicting cauda equina syndrome (CES). The thesis was that bladder scanning [specifically postvoid residual (PVR) volume] would have higher diagnostic accuracy than physical examination alone. SUMMARY OF BACKGROUND DATA: CES is an ill-defined condition with a spectrum of presenting symptoms. There is neither a combination of clinical symptoms and/or signs that reliably predicts cauda equina compression nor single defining clinical criterion …
