INTRODUCTION: Heterotopic pancreas is defined as pancreatic tissue without a real anatomical or vascular connection to the pancreas. It can be found in the stomach, duodenum, jejunum, ileum, Meckel's diverticulum, colon gall bladder, umbilicus, fallopian tube, mediastinum, spleen and liver. Complications of heterotopic pancreas are inflammation, bleeding, obstruction, malignant transformation, carcinoid syndrome, jejunojejunal intussusception and ileus, but it is usually asymptomatic and diagnosed only during examinations for other diseases.

A 55 -year-old Asian man was seen in the emergency department with bleeding per rectum. He was a teetotaller and had no previous abdominal surgery. He did, however, report a change in bowel habit towards constipation. He underwent colonoscopy which revealed a lesion, highly suspicious of malignancy, in the caecum. Two patients with chronic gastrointestinal bleeding are reported. One patient had chronic occult bleeding with iron deficiency. Extensive evaluations performed during a period of two years in three different medical centres did not reveal the cause of the bleeding. Four patients with chronic gastrointestinal blood loss of obscure aetiology were investigated by visceral angiography. A case of ectopic human Fasciola spp. infection in the cecal wall is reported. The patient, a 27-year-old Korean woman, resident in Seoul, Korea, presented with nausea, vomiting, and epigastric tenderness. One week later a palpable mass was discovered in the right iliac fossa. A clinical diagnosis of a carcinoma of the colon was made and the patient underwent a cecal resection. The mass proved to be an inflammatory reaction containing numerous tracts made by the migrating fluke, Fasciola sp.

A laparotomy was performed on a 44-year-old male patient with an acute abdomen who had findings suggestive of acute appendicitis and a periappendicular abscess. A histopathological examination revealed a carcinoid tumor infiltrating the intestinal serosa. A hospital-based study was performed in order to examine the subsite distribution of colorectal cancer diagnosed in 1992-93 in the Zaanstreek region. The localization was compared with tumours diagnosed in the years 1972-73 and 1982-83. The number of colorectal cancers has markedly increased, and a significant rise in localization of tumours proximal to the splenic flexure has occurred (32% in 1972-73 to 43% in 1992-93). Apart from their association in familial and hereditary cancer syndromes, sporadic double malignancies of the gastrointestinal tract involving the ampulla of Vater and colon are extremely rare. Although synchronous resection of the two by adding a colectomy to a pancreaticoduodenctomy can be accomplished with minimal increase in the surgical morbidity, a few patients, however, are best managed by a staged resection.
