Clinical trials for chimeric antigen receptor T-cell therapy: lessons learned and future directions.

PURPOSE OF REVIEW: The purpose of this review is to summarize the status and utilization of chimeric antigen receptor T-cell (CAR-T) therapy based on the most recent clinical trials in patients with leukemia and lymphoma. Additionally, this review will highlight limitations in current strategies, discuss efforts in toxicity mitigation, and outline future directions for investigation. RECENT FINDINGS: CD19 targeted CAR-T-cell therapy (CD19-CAR) is highly effective in patients with relapsed/refractory (r/r) B-cell hematologic malignancies. However, multiple challenges have arisen, particularly

Present and future avenues of cell-based therapy for brain injury: The enteric nervous system as a potential cell source.

Cell therapy is a promising strategy in the field of regenerative medicine; however, several concerns limit the effective clinical use, namely a valid cell source. The gastrointestinal tract, which contains a highly organized network of nerves called the enteric nervous system (ENS), is a valuable reservoir of nerve cells. Together with neurons and neuronal precursor cells, it contains glial cells with a well described neurotrophic potential and a newly identified neurogenic one. Recently, enteric glia is looked at as a candidate for cell therapy in intestinal neuropathies. Here, we present th

Chimeric antigen receptor T-cell therapy for melanoma.

INTRODUCTION: In recent years, chimeric antigen receptor (CAR) T cell therapy has emerged as a cancer treatment. After initial therapeutic success for hematologic malignancies, this approach has been extended for the treatment of solid tumors including melanoma. AREAS COVERED: T cells need to be reprogramed to recognize specific antigens expressed only in tumor cells, a difficult problem since cancer cells are simply transformed normal cells. Tumor antigens, namely, CSPG4, CD70, and GD2 have been targeted by CAR-T cells for melanoma. Moreover, different co-stimulatory signaling domains need t

Cell therapy for nonischemic dilated cardiomyopathy: A systematic review and meta-analysis of randomized controlled trials.

Cell therapy involves transplantation of human cells to promote repair of diseased or injured tissues and/or cells. Only a limited number of mostly small-scale trials have studied cell therapy in nonischemic cardiomyopathy (NICM). We performed a meta-analysis of randomized clinical trials (RCTs) to assess the safety and efficacy of cell therapy in NICM. Electronic databases were searched for relevant RCTs from inception until August 2020. Outcomes assessed were left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter or volume (LVEDD), quality of life (QoL) indices, a

Human herpesvirus 6 myelitis after chimeric antigen receptor T-cell therapy.

This article reports a fatal case of human herpesvirus 6 (HHV-6) myelitis following CD19-targeted chimeric antigen receptor T-cell therapy. Infection from HHV-6 reactivation after haematopoietic stem cell transplant is established, and outside of this population is limited to case reports. The patient developed cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome that responded to corticosteroids both clinically and on imaging. Subsequently, ascending flaccid paralysis developed, leading to neuromuscular respiratory failure and, ultimately, death. Disease progre

Cell therapy assisted autotransplantation of olfactory tract into the optic nerve: A potential treatment for optic neuropathy.

Optic neuropathy is an invaliding pathology with diverse clinical manifestation and varying causes. Current understanding of etiopathological aspects of optical neuropathy does not provide an effective treatment protocol. In this article we discuss existing treatment methods, and their effectiveness, evaluated depending on disease etiology. The olfactory tract is a source of olfactory ensheating cells, whose unique properties can have treatment potential in correction of nerve degeneration. Transplantation of an olfactory tract graft into the damaged optic nerve is a technically achievable int

Multimodality molecular imaging of stem cells therapy for stroke.

Stem cells have been proposed as a promising therapy for treating stroke. While several studies have demonstrated the therapeutic benefits of stem cells, the exact mechanism remains elusive. Molecular imaging provides the possibility of the visual representation of biological processes at the cellular and molecular level. In order to facilitate research efforts to understand the stem cells therapeutic mechanisms, we need to further develop means of monitoring these cells noninvasively, longitudinally and repeatedly. Because of tissue depth and the blood-brain barrier (BBB), in vivo imaging of

A 38-color spectral panel for cost efficient characterization of T cell therapy products.

Investigators must frequently apportion precious sample volumes between multiple assays to measure the viability, potency, and compositional phenotype of cell therapy products and clinical research samples. However, the use of disparate assay modalities may miss critical information which links the expression profile of cells with functional activity, especially when these activated cell populations are rare. To address this shortcoming, we describe in this short report the creation of a 38-color spectral flow cytometry panel for the phenotypic and functional characterization of antigen reacti

In silico optimization of regenerative cell therapy in the infarcted human ventricles to mitigate arrhythmic burden.

Myocardial infarction remains a frequent cause of heart failure and mortality. Cell therapy has shown promise in regenerating the damaged tissue, but delivered cells may beat spontaneously and produce ventricular arrhythmias, hindering clinical application. Here, we conducted multiscale computer simulations of the electrical activity of the infarcted human ventricles including the cardiac conduction system to identify and mitigate pro-arrhythmic mechanisms following cell delivery. Firstly, our simulations show how arrhythmic risk increases from before to after cell injection and further during

Clinical trial landscape of cell therapy for spinal cord injury: from integrated practices to future developments.

INTRODUCTION: Spinal cord injury (SCI) precipitates a multiphasic secondary injury cascade that establishes a hostile, inhibitory microenvironment, rendering the condition refractory to conventional surgical stabilization and rehabilitation. While cell-based therapies offer promise for neural reconstruction, their clinical translation is impeded by protocol heterogeneity and fragmented safety data. To address this, we mapped clinical trials for SCI to quantify patient demographic parameters, identify lineage-specific adverse-event patterns, evaluate objective motor and sensory efficacy outcome

Top advances of the year in autologous cellular therapy in melanoma and solid tumors.

The year 2025 marked significant advances in autologous cellular therapy for melanoma and solid tumors, building on landmark regulatory approvals in 2024. Lifileucel, the first Food and Drug Administration-approved tumor-infiltrating lymphocyte (TIL) therapy for advanced melanoma, demonstrated durable efficacy in the 5-year analysis of the C-144-01 trial, with an objective response rate (ORR) of 31.4% and prolonged responses in a heavily pretreated population. Real world data further supported its effectiveness, with higher ORR likely reflecting earlier lines of therapy and differences in pati

The contribution of probiotics to combined cellular therapy in skin wound healing in diabetic rats.

BACKGROUND: Diabetes-related wound care is still a major issue due to chronic and non-healing ulcers that are prone to infection and ultimately amputation. In recent years, cellular therapy (CT) products such as mesenchymal stem cells (MSC), platelet-rich plasma (PRP), and stromal vascular fraction (SVF) have been widely used. A combined cellular therapy (CCT) has not yet been tested as a triple combination, although its use alone and in dual combinations has been investigated. Probiotics (PB) accelerate healing by altering the intestinal microbiota. This study aims to examine the role of PB i

Comprehensive review and reflective evaluation of cell therapies for tuberculosis.

SUMMARYTuberculosis (TB) remains a major infectious disease threatening global health. An estimated 2 billion people worldwide are infected with Mycobacterium tuberculosis (Mtb), and 5-10% of those with latent infection will develop active TB. Despite the success of existing anti-TB drugs in controlling transmission, significant challenges remain, including long treatment durations, severe side effects, suboptimal treatment adherence, and inevitable drug resistance. Even newly developed drugs may fail to keep pace with the rising number of drug-resistant cases. Moreover, these treatments do no
