Do Direct Bypasses Really Resolve Capillary Vessels in Moyamoya Disease? Delayed Hemorrhage Potentially Relevant to Direct Bypasses: A Pathological Investigation.

OBJECTIVE: Moyamoya disease is a chronic but progressive obliterative cerebrovascular disease of bilateral internal carotid arteries (ICAs) causing hemorrhagic or ischemic cerebral strokes. Surgical revascularization has the potential for resolving the capillary vessels, but the effect on the occlusive ICA and the moyamoya vessels after a direct bypass remains unclear. PATIENT: A 2-year-old girl with a history of repeated transient ischemic attacks and direct bypasses but demonstrating improvement and associated anomaly is reported. A year and a half later, after a bilateral revascularization

Intraoperative local hemodynamic quantitative analysis of direct revascularization in patients with moyamoya disease.

Direct bypass surgery improves blood flow in patients with moyamoya disease (MMD) as well as causes local flow relocation and disturbance. This study aimed to describe the characteristics and changes of quantitative blood flow in MMD patients. Sixty-four consecutive MMD patients (67 hemispheres) who underwent STA-MCA anastomosis between August 2013 and September 2017 were included in this study. Intraoperative blood flow of superficial temporal artery and middle cerebral artery was measured with a noninvasive, ultrasound flowprobe, before and after the bypass. All data were collected prospecti

Flow-augmentation bypass for moyamoya disease.

Moyamoya disease (MMD) is a chronic, occlusive cerebrovascular disease with unknown etiology, which is characterized by progressive steno-occlusive changes at the terminal portion of the internal carotid artery and an abnormal vascular network formation at the base of the brain. MMD has an intrinsic temporal nature to attempt a gradual conversion of the vascular supply for the brain from intracranial/internal carotid (IC) system to extracranial/external carotid (EC) system, so called "IC-EC conversion." Compatible cerebrovascular angio-architecture could be found in association with a variety

Maxillary Artery to Middle Cerebral Artery Bypass: A Novel Technique for Exposure of the Maxillary Artery.

OBJECTIVE: To define the maxillary artery (MaxA) anatomy and present a novel technique for exposing and preparing this vessel as a bypass donor. METHODS: Cadaveric and radiologic studies were used to define the MaxA anatomy and show a novel method for harvesting and preparing it for extracranial to intracranial bypass. RESULTS: The MaxA runs parallel to the frontal branch of the superficial temporal artery and is located on average 24.8 ± 3.8 mm inferior to the midpoint of the zygomatic arch. The pterygoid segment of the MaxA is most appropriate for bypass with a maximal diameter of 2.5 ± 0.

Clinical and angiographic outcomes from indirect revascularization surgery for Moyamoya disease in adults and children: a review of 63 procedures.

BACKGROUND: Several forms of indirect cerebral revascularization have been proposed to promote neovascularity to the ischemic brain. OBJECTIVE: To present clinical and angiographic outcomes of indirect revascularization by encephaloduroarteriosynangiosis and burr holes for the treatment of Moyamoya disease in adults and children. METHODS: Data from 63 hemispheres treated in 42 patients (average age, 30 years; 33 adults; 30 female patients; median follow-up, 14 months) were reviewed. In hemispheres with preoperative and postoperative (6- to 12-month) angiograms available, superficial temporal…

Ribbon enchephalo-duro-arterio-myo-synangiosis for moyamoya disease.

In patients with advanced moyamoya disease, reconstructive surgery alone may not prevent the deterioration of blood flow in the territory of the anterior cerebral artery. These types of surgery include superficial temporal artery-to-middle cerebral artery anastomosis and encephalo-duro-arterio-myo-synangiosis (EDAMS). Bilateral encephalo-duro-arterio-synangiosis (EDAS) gradually reduced the transient ischemic attacks in one of our patients who experienced motor weakness in the left extremities. After surgery, however, persistent bilateral attacks still occurred in the patient's legs. In a subs

[Anesthetic experience of a child with moyamoya disease].

The authors report four anesthetic experiences of a child with moyamoya disease for two occasions of angiography and bilateral encephalo-duro-arterio-synangiosis (EDAS). For angiography anesthesia was maintained with inhalation of halothane-nitrous oxide-oxygen under spontaneous respiration. During the first angiography PaCO2 was 59.5mmHg and 55.2mmHg and it was 56.0mmHg during the second angiography. For EDAS, anesthesia was managed under controlled ventilation and avoiding hyperventilation. Blood pressure was stable. The patient awoke well. Under anesthetic management of a child with moyamoy

[A case of persistent trigeminal artery aneurysm treated by proximal obliteration of feeding vessels in both intra- and extracranial compartments].

The authors described a case with a saccular aneurysm of the persistent trigeminal artery (PTA) treated with intra- and extracranial proximal obliteration of the feeding arteries. PTA aneurysm is a rare disorder found only 14 cases in the literature. There have been only 4 cases treated by surgical procedure; 2 underwent direct surgery and the remaining 2 had carotid ligation. Both of the cases treated by direct obliteration of the aneurysm sustained diplopia and sensory disturbance around the orbit as postoperative neurological deficits. We thought another choice of treatment to reduce the ri

Revascularization surgery for 28 patients with moyamoya disease--operative methods, outcome and postoperative angiography.

51 times (sides) of reconstructive surgery using 28 patients with moyamoya diseases were evaluated concerning operative methods, outcome and postoperative angiographic findings. Twenty patients (72%) showed excellent results, 7 patients (25%) good and 1 patient (3%) poor. Development of collaterals shown on postoperative angiograms was excellent on 20 sides, good on 9 sides, and fair on 8. Abundant neovascularization was most commonly noted on angiogram in superficial temporal artery-middle cerebral artery bypass with encephalo-myo-synangiosis. Encephalo-myo-arterio-synangiosis, devised by the

Superficial temporal-middle cerebral artery anastomosis in Moyamoya disease.

A 41-year-old Libyan woman with Moyamoya disease and persisting post-stroke neurological deficits was treated by a superficial temporal-middle cerebral artery (STA-MCA) anastomosis. The postoperative angiograms revealed that the STA was rapidly irrigating the territory of the middle cerebral artery including those regions that had been filled preoperatively via different networks of collaterals. Furthermore prompt venous drainage was seen to occur postoperatively. Following surgery EEG analyses revealed considerable increase in the electrical brain activity, and neurological examinations showe

[Perioperative Management and Complications of Moyamoya Disease].

Moyamoya disease is a chronic cerebrovascular disorder characterized by progressive stenosis or occlusion of the terminal portion of the internal carotid artery around the circle of Willis, thereby forming fragile collateral vessels(moyamoya vessels). Although ischemic symptoms predominate in pediatric cases, adult cases often involve ischemic and hemorrhagic events. Surgical revascularization effectively improves long-term outcomes; however, perioperative complications, such as ischemic events, hyperperfusion syndrome, and hemorrhagic complications, remain challenging. Perioperative managemen

An Orientational Harvesting Occipital Artery Technique via far Lateral Approach: A Cadaveric Study.

OBJECTIVE: To provide further information on the identification of the occipital artery (OA) and suggest an improved approach to its anterograde dissection technique for harvesting. METHODS: Six cadaveric specimens were prepared for surgical simulation, and the anterograde approach was used to harvest the OA; a hockey stick incision was made from the C2 spinous process through the nuchal ligament to the mastoid tip. By retracting the scalp flap from the posterior arch of C2 to the transverse process, the suboccipital triangle was reflected by a single myocutaneous flap from the inferior nucha
