The authors report the case of a 59-year-old woman with progressive neck and arm pain that initially appeared in the neck and later extended to the shoulder and upper extremity. This pain was caused by compression of the cervical cord between the atlas and axis by the vertebral artery, and disappeared promptly following microvascular decompression.

A 77-year-old man was admitted with posterior cervical pain, lumbago, a low-grade fever, and anorexia after bowel cleaning with polyethylene glycol for colonoscopy. Computed tomography of the neck showed calcification of the transverse ligament of the atlas. He was diagnosed with crowned dens syndrome (CDS). His condition improved following treatment with corticosteroids. This represents a rare case of CDS after endoscopy with polypectomy. We should consider CDS in the differential diagnosis of patients with unexplained fever and posterior cervical pain after bowel preparation for colonoscopy.

The authors present an unusual case of massive ante partum haemorrhage following removal of a cervical suture. This patient was admitted for routine removal of a McDonald's cervical suture at 37 weeks gestation. The suture was densely adherent and after removal she suffered a haemorrhage of 2500 ml from where the suture was removed which led to fetal distress and an emergency caesarean section. The cervix was sutured and a vaginal pack inserted. During the procedure she developed hypovolaemic shock requiring 7 units of blood, 4 units of fresh frozen plasma, 1 unit of platelets and 1 unit of cr

The selection of the appropriate surgical approach in the management of an unstable cervical spine injury is predicated on the biomechanic deficiencies of the bony and ligamentous structures, the age of the patient, the level of experience of the surgeon, and the concomitant medical comorbidities. The optimal approach ideally is the least invasive, provides the greatest benefit-to-risk ratio in terms of potential injury to contiguous neurovascular structures, and provides adequate stabilization to avoid cumbersome external immobilization and allows early rehabilitation. This article discusses

Symptomatic atlantoaxial dislocation occurs rarely in psoriatic arthropathy and has previously been reported only as a late complication in this disorder. We report severe upward axial dislocation and acquired basilar impression as a presenting manifestation of psoriatic arthropathy. Magnetic resonance imaging is useful in evaluating this condition.

Acquired non-traumatic torticollis may be due to a variety of causes. Presumably it may arise from inflammatory diseases and tumours of the neck as well as the central nervous system. Three cases of acquired torticollis due to osteomyelitis, retropharyngeal carcinoma and subluxation of the spine are presented and the variety of possible underlying diseases is demonstrated. Persistent torticollis should provoke an aggressive search for a specific aetiology by all means, especially CT scans or even biopsy.

Occipitoatlantoaxial malformation was diagnosed in a 19-month-old mare of Appaloosa breeding and in a 3-year-old Quarter Horse gelding. Both horses had abnormal head and neck carriage since birth, but neurologic deficits did not become evident clinically until the horses reached 2 and 3 years of age, respectively. Palpation and manipulation of the base of the skull and cervical area proved to be useful diagnostically. Movement could not be elicited at the atlantooccipital joint but, in comparison with clinically normal horses, the range of dorsoventral motion at the atlantoaxial joint was incr

An 18-month-old child was rendered tetraplegic below C6 in a road traffic accident. There was an associated fracture-dislocation of the atlanto-axial joint. Early management, and the probable role of autonomic dysreflexia in increasing the neurological deficit are considered.

The results of a closed, non-operative type of treatment in acute upper cervical injury have been reviewed, especially as they relate to the use of the halo cast or vest. The morbidity with the use of this device has been minimal and there has been no mortality. Early patient mobilization has been readily achieved and in the case of the Hangman's, Jefferson's, and odontoid fractures, stable solid bony healing has been achieved in nearly all cases. Similar results have been noted in the C-1 arch fracture, as well as the acute post-infectious subluxations of C-1/C-2. In a small number of cases i

Trauma of the cranio-vertebral junction does not only cause osseous and ligamentous injuries but also nervous and vascular damage. The peculiar structure of these joints and trauma by hyperextension as well as forced bending of the spinal cord provoke distorsion, dislocations and fractures of atlas and axis as well as occipital condyles. A radiographic examination of these lesions must be minute and tomography and functional tests employed. Post-traumatic, neurologic and vascular complications are often the cause for acute and chronic bulbo-medullary alterations.

Around a third of all cervical spine injuries occur in the upper cervical spine in the area between the occiput and the second cervical vertebra. The latter being the most common location of the injury with around 70%. But also atlas fractures, occipital condyle fractures, traumatic spondylolisthesis of C2, atypical fractures in the corpus area as well as atlantooccipital and atlantoaxial ligamentous lesions should be mentioned in connection with injuries in this area. In many cases, conservative therapy regimen is possible. In unstable or displaced injuries, however, surgical intervention is

STUDY DESIGN: Forty isolated specimens of the first cervical vertebra were tested by the application of pure axial force to failure. To exclude ligamentous side effects, transverse ligaments were dissected in all specimens.
OBJECTIVE: To investigate the biomechanical characteristics of the human atlas and to describe the influence of different speeds of force impact on the fracture types.
SUMMARY OF BACKGROUND DATA: Atlas fractures have been reproduced in some studies in the literature. However, the characteristics of isolated atlas fractures under pure axial loading at different speeds has

STUDY DESIGN: A case of combined odontoid and Jefferson fracture is reported.
OBJECTIVE: To alert spine physicians to the rare combination of an odontoid and Jefferson fracture in a child.
METHODS: A 5-year old boy presented with neck pain and torticollis after falling on his head from a four-wheeler that had rolled over. A computed tomography scan confirmed a combined odontoid and Jefferson fracture.
RESULTS: The child was successfully treated nonsurgically with a hard cervical orthosis. At this writing, the child clinically is asymptomatic 2 years after the injury.
DISCUSSION: The fall o

An in-depth understanding of the anatomy of the craniocervical junction (CCJ) is indispensable in skull base neurosurgery. In this paper, we discuss the osteology of the occipital bone, the atlas (C1) and axis (C2), the ligaments and the muscle anatomy of the CCJ region and their relationships with the vertebral artery. We will also discuss the trajectory of the vertebral artery and review the anatomy of the jugular foramen and lower cranial nerves (IX to XII). The most important surgical approaches to the CCJ, including the far lateral approach, the anterolateral approach of Bernard George an

In this study, the axial skeletons of two Early Paleocene marsupials, Mayulestes ferox and Pucadelphys andinus, were analyzed functionally and compared to that of six South American and three Australian species of extant marsupials. In the case of the South American opossums, myological data of the epaxial musculature were collected and analyzed and osteological-myological associations were related to locomotor behavior. Various features of the vertebral column that relate to diet or to locomotor or postural patterns were pointed out. These features include: the craniocaudal development of the

STUDY DESIGN: Biomechanical and clinical testing of an atlantoaxial posterior fixation device.
OBJECTIVES: The authors tested an internal fixation device to maintain the atlas and axis in an anatomic relationship while fusion occurs. The device should also facilitate intraoperative reduction of any residual anterior atlantoaxial subluxation. The device should allow the use of cancellous rather than cortical bone graft.
SUMMARY OF BACKGROUND DATA: Previous techniques of atlantoaxial fusion were not universally successful, and the quality of reduction was assessed infrequently.
METHODS: Biome

Hydatid disease (HD) of the cervical spine is rare. A case investigated by computed tomography (CT) and magnetic resonance imaging (MRI) is presented. While CT shows the bone lesion better, MRI is superior in demonstrating compression of neural structures. The complementary use of CT and MRI in such cases is suggested.
