Cervical pessary use and preterm birth.

Preterm birth remains a considerable public health concern and priority. Little headway has been made in the prevention of preterm birth despite considerable research in this area. New ideas and treatments are desperately needed. The pessary has emerged as a possible treatment for the prevention of preterm birth in both singleton and twin gestations. It appears to be low cost with minimal side effects. This review focuses on the available evidence for the use of cervical pessaries for the prevention of preterm birth, especially in a high-risk population with a shortened cervical length. Larger

Maternal and fetal ultrasonography, vulvar temperature and vaginal mucous impedance for the prediction of parturition in Saanen does.

The aim of this study was to evaluate and correlate modifications of vaginal mucous impedance, vulvar temperature and ultrasonographic measurements (echobiometric parameters) to parturition in pregnant Saanen does. 30 does were selected for the study and submitted to an estrus synchronization protocol and natural mating. The females were evaluated daily from Day 143 of pregnancy to parturition. For the sonographic evaluations, the following structures were measured: biparietal diameter, thoracic diameter, abdominal diameter, ocular orbit, kidney length, kidney height, cardiac area, placentome

The potential role of diffusion weighted imaging in the diagnosis of early carotid and vertebral artery dissection.

PURPOSE: To investigate the role of the diffusion weighted imaging (DWI) in the acute dissection of internal carotid artery (ICA) and vertebral artery (VA) and assessing the length of intramural hematoma (IMH), caused by dissection. METHODS: We analyzed 28 patients presenting with a dissection of the ICA and/or VA with respect to the presence of high signal intensity areas on DWI suggestive of dissection and 20 control subjects without arterial dissection, some with and some without atherosclerotic lesions. ICA or VA dissection was defined by clinical and imaging, computed tomography angiogra

Screening for women at risk of spontaneous preterm birth, including cervical incompetence.

Preterm births remain one of the biggest challenges in obstetrics worldwide. With the advancement of neonatal care, more premature neonates survive with long term consequences. Therefore, preventing or delaying preterm births starting from the preconceptional or antenatal periods are important. Among the numerous screening strategies described, not one can fit into all. Nonetheless, approaches including identifying women with modifiable risk factors for preterm births, genitourinary infections and short cervical length are the most useful. In this article, the current evidence is summarized an

Interventions to Try to Prevent Preterm Birth in Women With a History of Conization: A Systematic Review and Meta-analyses.

OBJECTIVES: The most effective preterm birth (PTB) intervention is unknown for women who are at risk of PTB due to a history of conization. The objective of this systematic review was to determine whether PTB interventions, progesterone, cerclage, and pessary decrease the risk of PTB compared to no treatment in singleton-and separately in twin-pregnancies of women with history of conization. METHODS: We searched Cochrane Central, MEDLINE, EMBASE, CINAHL, and ClinicalTrials.gov from January 1994 until May 2017. In duplicate, we reviewed titles, abstracts, full texts, extracted data, and assess

Combined treatment with cervical pessary and vaginal progesterone for the prevention of preterm birth: A randomized clinical trial.

AIM: We compared the effectiveness of a combined treatment involving cervical pessary plus vaginal progesterone to vaginal progesterone alone in decreasing the rate of preterm birth in women with short cervix in the second gestational trimester. METHODS: This prospective, open-label, randomized clinical trial was conducted on 144 pregnant women with singleton pregnancy who had a cervical length ≤ 25 mm, at 18-22 gestational weeks (GW). Seventy-three patients were assigned to receive 400 mg of daily vaginal progesterone (group A), and 73 to receive cervical pessary plus 400 mg of daily vaginal

Morphology and morphometry of Haemonchus contortus exposed to Gigantochloa apus crude aqueous extract.

AIM: Haemonchus contortus is the most pathogenic nematode infesting the digestive tract of goats and sheep worldwide leading to a tremendous loss in a variety of routes. Economic losses due to haemonchosis in subtropic and tropic areas are usually caused by poor weight gain, minimized growth, loss of production, and mortality. The prevalence of haemonchosis in Indonesia is 89.4% in goat, and annual loss achieved 1 million US dollars. This study evaluated in vitro effects of Gigantochloa apus crude aqueous extract as an anthelmintic on H. contortus morphology and morphometry. MATERIALS AND MET

Comparison between cervicovaginal fluid interleukin-6 and fetal fibronectin in combination with cervical ultrasound characteristics for the prediction of spontaneous preterm delivery in threatened preterm labor.

BACKGROUND: About 80% of patients presenting with threatened preterm labor do not deliver preterm. Currently, cervical length and cervicovaginal fluid fetal fibronectin are tools used to predict spontaneous preterm birth. However, fetal fibronectin testing has been discontinued worldwide. OBJECTIVES: This study aimed to compare the predictive performance of cervicovaginal fluid interleukin-6 (IL-6) and fetal fibronectin, combined with cervical characteristics (cervical length, sliding sign, and elastography) and other maternal characteristics, for predicting spontaneous preterm delivery in pa

Defining Cervical Elongation: A Prospective Observational Study.

OBJECTIVES: Our primary aim was to define cervical elongation (CE) using the following methods: (1) measurement of pathology specimen, (2) physician perception, (3) intraoperative estimate of anterior cervical length, and (4) office Pelvic Organ Prolapse Quantification (POP-Q) points C and D. Our secondary aim was to determine whether these definitions correlate with perioperative outcomes. METHODS: Women undergoing vaginal hysterectomy and prolapse repair were enrolled. Office POP-Q measurements were collected. Estimates of cervical length were made based on points C minus D of the POP-Q and

Relative position of cerclage - an ultrasound marker for prediction of preterm birth.

BACKGROUND: A transvaginal cerclage (TVC) identified high in the cervix is associated with prolongation of pregnancy. However, the relative position of the TVC (length above the cerclage/total cervical length) may be a better predictor of cerclage success as it is proportional of total cervical length. This study evaluates the relative position of TVC and its association with spontaneous preterm birth, comparing its predictive value against other post-cerclage ultrasound parameters. METHODS: A retrospective cohort study was conducted of singleton pregnancies with TVC. Demographic and ultrasoun

The predictive value of hemogram parameters for early preterm delivery in pregnant women undergoing cervical cerclage.

OBJECTIVE: This study aims to investigate the predictive value of hemogram parameters in early preterm delivery (32 gestational weeks and below) among pregnant women who have undergone cervical cerclage, based on cervical changes determined before the cerclage procedure. METHODS: Between 2010 and 2020, a total of 161 patients underwent cervical cerclage. The participants were divided into three groups. Group 1 (n=92) consisted of pregnant women who underwent prophylactic cerclage. Group 2 (n=31) included those with cervical shortening (<5 mm) and/or dilation (≤3 cm). Group 3 (n=38) comprised

Differences in linear and angular measurement of the lower facial third in lateral cephalometry and lateral photometry: A comparative study.

BACKGROUND: In clinical orthodontic practice, to evaluate aesthetics in dentistry, lateral cephalometry and lateral photometry are used to measure and assess craniofacial morphology. In contrast to cephalometry, photometry has no radiation hazard and is less expensive. There has been a paradigm shift in contemporary orthodontics that now emphasizes soft tissue aesthetics, rather than depending entirely on hard tissue evaluation from lateral cephalometry. Therefore, the key question is whether lateral photometry can be a substitute for conventional cephalometry for diagnostics, prognostic analy

Cervical Evaluation: From Ancient Medicine to Precision Medicine.

Since ancient times, cervical assessment for predicting timing of delivery has relied primarily on digital (subjective) assessment of dilatation, softening, and length. To date, transvaginal ultrasound cervical length is the only one of these parameters that meets criteria for a biomarker; no objective, quantitative measure of cervical dilatation or softening has gained clinical acceptance. This review discusses how the cervix has been assessed from ancient times to the present day and how a precision medicine approach could improve understanding of not only the cervix, but also parturition in
