Brachial plexus traction injuries most frequently occur following acute flexion or extension of the neck. The symptomatology following this injury may be defined clearly into the anatomic patterns of upper trunk, lower trunk, posterior cord, medial cord, and lateral cord radiations. This injury is the most common cause of the neurogenic form of thoracic outlet syndrome and is frequently seen in conjunction with cervical spine disease as well as peripheral entrapment syndromes of the ulnar, radial, and medial nerves.

We performed C8 nerve root stimulation in addition to other electromyographic (EMG) studies in a surgically proven case of classic thoracic outlet syndrome (TOS). The patient was a 19-year-old woman with a 2-year history of right hand cramps and progressive weakness and atrophy of hand muscles, especially the thenar eminence. Routine EMG studies showed evidence for an axon-loss lower trunk brachial plexopathy. Stimulation studies of the C8 nerve roots demonstrated proximal conduction block on the affected side only. The diagnosis was further supported by cervical spine radiographs, which demon

Parsonage-Turner syndrome (PTS) is an inflammatory, immune-mediated neurogenic condition characterized by acute-onset, severe unilateral shoulder/upper arm pain followed by patchy weakness and atrophy. Often described as an idiopathic brachial plexitis, most cases of PTS involve the brachial plexus or its terminal branches, and root-level or paraspinal muscle involvement is rare. Among the differential diagnoses for proximal neuropathic root involvement, cervical radiculopathy by mechanical compression of the exiting nerve roots and diabetic cervical radiculoplexus neuropathy by way of immune-

Hirayama disease is a rare neuromuscular disease, which classically presents as lower motor neuron weakness and atrophy in the upper limbs and specifically the C7-T1 myotomes. Proposed pathogenesis relates to microcirculatory dysfunction in the territory of the anterior spinal artery caused by epidural venous plexus engorgement with forward displacement of the posterior dura and spinal cord during neck flexion, leading to chronic ischemic changes in the lower cervical anterior horn cells. Diagnosis hinges upon clinical and radiographic findings, and treatment is generally conservative given th

BACKGROUND: The traditional approach for occipital migraine surgery encompasses three separate surgical incisions in the posterior neck to decompress the greater occipital nerves (GON), lesser occipital nerves (LON), and third occipital nerves (TON). Other incisions have been investigated, including singular transverse incisions. We sought to evaluate a single, vertical midline incision approach for decompression of all six occipital nerves.
METHODS: Using 10 cadaveric hemi-sides (5 fresh cadaver head and necks). Anatomic landmarks and the location of the bilateral GON, LON, and TON were mark

The ansa subclavia, subclavian loop, Vieussens' ansa or Vieussens' loop is a nerve cord that connects the middle cervical and inferior cervical sympathetic ganglia, forming a loop around the subclavian artery. The structure of the ansa subclavia is evolutionarily conserved from rats, guinea pigs, the porcine species and dogs to humans. A common application in physiological studies is to electrically stimulate the ansa subclavia in animal models as a robust protocol to modulate stimulatory cardiac sympathetic input. Despite a large number of physiological studies utilizing the ansa subclavia, o

Neurogenic tumors in the neck region are relatively infrequent with the exception of those involving the cranial nerves. A case of a ganglioneuroma or neurofibroma involving the nerve root of C4, C5 interspace is reported. The tumor appeared as a mass within the neck with a slight amount of pain in the shoulder and upper arm upon palpation. Surgical exploration revealed a mass superficial to the foramen and erosion of the foramen. Roentgenograms taken in the region demonstrated an extradural mass that was subsequently removed by laminectomy. Postoperatively, the patient did quite well. Surgica

The clinical picture of the so-called subforaminal stenosis headache (Gutmann) and its surgical treatment (Roesner) have been presented. We have analyzed our findings in 119 patients and 55 operations. The clinical picture is characterized by increasing pain in the upper cervical and occipital region which in later stages may be accompanied also by other symptoms like disturbances of concentration, memory, libido, and potency, as well in some cases by symptoms similar to those of a secondary chronic myelopathy. The syndrome is caused by a subforaminal stenosis of the dural sac which regularly

It is difficult to determine surgical approaches for treating a tumor arising at the thoracic outlet. Here we report a case of benign schwannoma arising at the thoracic apex, in which both pleural and cervical approaches were adopted. Chest radiographs of a 58-year-old male at the medical checkup in 2009 showed a tumor shadow in the right apical region that was increasing since 2003. He had no symptoms. Computed tomography(CT)and magnetic resonance imaging (MRI) showed a 35-mm tumor, which had a smooth surface and was suspected as a neurogenic tumor arising at the right thoracic outlet. Surger

OBJECTIVE. In this article, we will review the normal anatomy and imaging features of various neuromuscular abnormalities related to suprascapular neuropathy. CONCLUSION. Suprascapular neuropathy can be difficult to distinguish from rotator cuff pathology, plexopathy, and radiculopathy. Electrodiagnostic studies are considered the reference standard for diagnosis; however, high-resolution 3-T MR neurography (MRN) can play an important role. MRN enables direct visualization of the nerve and simultaneous assessment of the cervical spine, brachial plexus, and rotator cuff.

We report a case of bilateral neurogenic thoracic outlet syndrome (TOS). Electrophysiological examination suggested the presence of bilateral lower brachial plexus neuropathy. Radiography showed rudimentary bilateral cervical ribs. In the cases reported in the literature to date, the clinical findings are typically unilateral despite the common presence of bilateral bony abnormalities. Neurogenic TOS should be considered in young women, even if they present with bilateral symptoms, when they have occupations requiring strenuous activity of the upper limbs.

Neurovegetative system consist of preganglionar fibre from neural tube and postganglionar from neural crest. We examined 13 patients with Fuchs' syndrome. The first sign was heterochromia, followed by keratic precipitates, pathological cataract and finally secondary glaucoma. The cause of Fuchs' syndrome was the damage of superior cervical ganglion. We studied 4 cases of Claude Bernard Horner's syndrome through damages of preganglionar sympathetic system, in 3 cases because of laterocervical neoplasia and in the last case through brachial plexus damages.

Nerve transfer procedures are increasingly performed for repair of severe brachial plexus injury (BPI), in which the proximal spinal nerve roots have been avulsed from the spinal cord. The procedure essentially involves the coaption of a proximal foreign nerve to the distal denervated nerve to reinnervate the latter by the donated axons. Cortical plasticity appears to play an important physiological role in the functional recovery of the reinnervated muscles. The author describes the general principles governing the successful use of nerve transfers. One major goal of this literature review is
