XMP-Net: An XAI-Based Modified Xception Model for Recognizing Monkeypox and Other Skin Diseases.

This research introduces "XMP-Net," a modified Xception-based deep learning architecture constructed for the categorization of skin conditions, with a particular focus on identifying monkeypox. The study recognizes skin images of four categories: normal, chickenpox, measles, and monkeypox. To enhance interpretability and foster confidence in the model's predictions, Grad-CAM (gradient-weighted class activation mapping) and LIME (local interpretable model-agnostic explanations) were employed to illustrate the model's thinking manner. The model demonstrated impressive classification performance,…

Prevalence and progression of pneumonia in immunocompetent adults with varicella.

Pneumonia is the most common complication of varicella infections. Although previous studies have tended to focus mainly on immunocompromised patients, varicella pneumonia can also occur in healthy adults. Therefore, in this study, we aimed to assess the progression of varicella pneumonia in immunocompetent hosts. This retrospective study involved immunocompetent adult outpatients with varicella who attended the adult Fever Emergency facility of Peking University Third Hospital from April 1, 2020, to October 31, 2022. Varicella pneumonia was defined as a classic chickenpox-type rash in patient…

Adult Varicella Complicated by Deep Venous Thrombosis and Pulmonary Embolism: A Case Report and a Literature Review.

Varicella-zoster virus (VZV) infection can rarely present with severe vascular pathologies, such as deep vein thrombosis (DVT) and pulmonary embolism (PE). These events are seldom documented in the literature as complications of primary VZV infection in adults. We present the case of a 52-year-old Caucasian male patient with chickenpox complicated by DVT and PE, which developed despite thrombectomy and anticoagulation. Laboratory analysis revealed elevated antiphospholipid antibodies. Although the patient was discharged home after clinical improvement, antiphospholipid antibodies remained elev…

Visceral Varicella-Zoster Virus Infection in An Immunocompromised Patient: Uncommon Endoscopic Differential Diagnosis.

Varicella-Zoster virus (VZV) infection is well-known for its dermatological appearances, such as chickenpox and shingles. In addition to its dermal disease, we report a case of visceral dissemination of VZV in immunocompromised patients. A 38-year-old male Caucasian with a history of bone marrow transplantation due to diffuse large B-cell-lymphoma presented himself with severe abdominal pain. Immediate endoscopy revealed numerous gastric and duodenal ulcerations, while histological and molecular evaluation on tissue biopsies and blood samples confirmed VZV infection. Positive serological findi…

Childhood infections as risk factors for multiple sclerosis: Belgrade case-control study.

The aim of this case-control study was to analyze the role of childhood infections and vaccinations in patients with MS in the Belgrade population. The study group comprised 110 cases with definite MS according to Poser's criteria, in whom onset symptoms occurred up to 2 years prior to the interview. An equal number of controls, individually matched by sex, age and area of residence, was recruited from patients with various nonautoimmune neurological disorders. Measles (OR = 2.6, 95%CI 1.4-5.0), chickenpox (OR = 3.0, 95%CI 1.5-6.0), rubella (OR = 2.4, 95%CI 1.2-4.7), whooping cough (OR = 1.9, …

Immunity to varicella zoster virus in children with leukaemia.

A total of 46% (32/70) of children with acute lymphoblastic leukaemia presenting to the Hospital for Sick Children had a past history of chickenpox. When their immunity to varicella zoster virus (VZV) was assessed, 75% (24/32) had a positive lymphocyte transformation response to VZV antigen in tissue culture while 78% (25/32) possessed IgG antibody against VZV. Therefore, at least 22% of the children were probably susceptible to infection with VZV inspite of having a history suggestive of chickenpox. Of those with a negative or uncertain history 34% had a positive lymphocyte transformation res…

New varicella vaccines.

The live attenuated varicella vaccine, which is available for the prevention of chickenpox, was produced by a classic technology that also has been used for polio, measles, mumps, and rubella vaccines. There are many newer technologies that have been applied to the research and development of other vaccines. Each of these other approaches offers potential advantages and disadvantages relative to the current varicella vaccine.

Ambispective epidemiological observational study of varicella-zoster virus infection: An 18 year-single-center Bulgarian experience.

BACKGROUND: Varicella (chickenpox) and herpes zoster (shingles) are outcomes of varicella-zoster virus (VZV) infection, and understanding their incidence trends is vital for public health planning. AIM: To conduct an ambispective epidemiological study by analyzing the main epidemiological characteristics of VZV infection during an 18 year-period (2000-2018). METHODS: We used descriptive and epidemiological methods to characterize chickenpox in Bulgaria, the city of Plovdiv and the region for a period of 18 years (2000-2018). RESULTS: The average incidence of varicella-zoster infection for t…

[The significance of rare viral infections and the etiology of fetal damage].

Viral infections of the mother during pregnancy are responsible for approximately 10% of birth defects. Infections with Rubella-, Cytomegalo-, Mumps-, Measles- and Chickenpox-Virus in early pregnancy are associated with abortion or abnormal development of the fetus. While infection with Chickenpox, Hepatitis B, Herpes simplex and Enterovirusinfection at the end of pregnancy may affect the newborn. - It is estimated that birth defects due to viral infections in the pregnancy are accounting for ca. 2500 clinically affected newborns among 100,000 deliveries. The number of children well at birth b…

Risk factors for some tropical diseases in an African country.

BACKGROUND: Often, non-clinical risk factors could affect the predisposition of an individual to diseases. Understanding these factors and their impacts helps in disease prevention and control. This study identified risk factors for malaria, yellow fever, typhoid, chickenpox, measles, hepatitis B, and urinary tract infection in a population in an African country. METHODS: Our study was an observational, correlational, and quantitative one that explored relationships among risk variables and disease prevalence - without modifying or controlling the variables. Data for this study was obtained t…

Leishmania scar: a risk factor for the development of basal cell carcinomas.

Basal cell carcinoma is a malignant epithelial neoplasm of skin that usually arises in areas of chronic sun exposure. It rarely originates from smallpox vaccination scars, chickenpox scars, previous surgical scars, or burn scars. We present a case of basal cell carcinoma arising on a leishmania scar on the nasal dorsum 30 years after the primary lesion.

Varicella-zoster virus infections in pregnancy.

Varicella-zoster virus can cause a distinct congenital syndrome, a potentially fatal neonatal infection and life-threatening maternal illness. Physicians can reduce morbidity from these conditions by advising nonimmune pregnant women to avoid exposure to chickenpox and herpes zoster and, when indicated, by promptly administering varicella-zoster immune globulin. When prevention fails, acyclovir may be effective therapy.

[Study on the economic burden of varicella in Suzhou City from 2017 to 2022].

This study investigated the medical expenses and vaccination records of varicella patients using medical insurance settlement for medical treatment in Suzhou city from 2017 to 2022, systematically assessing the impact of implementing a free childhood varicella vaccine (VarV) strategy on the economic burden of varicella in the region. The results showed that patients without an immunization history accounted for a relatively high proportion of medical visits (84.22%). Among hospitalized patients, those without immunization history incurred higher per capita medical costs, which exhibited a stat

[Progress of varicella prevalence and immunization strategies in adolescents and adults].

Varicella, often referred to as chickenpox, is a widespread acute infectious condition triggered by the varicella-zoster virus (VZV). It manifests with systemic symptoms and distinct skin and mucosal eruptions, including macules, papules, and vesicles. Although it mainly affects children, the disease is typically more severe in teenagers and adults. Following the adoption of vaccine-based control measures in China, there has been a noticeable trend of varicella affecting older demographics, leading to an uptick in cases among teenagers and adults. This review synthesizes the latest research on…

[Universal vaccination against varicella in Italy: the same opportunity for all children].

Varicella is an infectious disease still frequent in Italy, where 8 out 20 Regions have adopted universal vaccination programs starting from 2003. Accordingly to National Vaccination Plan, all Regions should introduce universal varicella vaccination in 2015. An independent multidisciplinary group of experts met to discuss some debated questions. The available evidence of varicella vaccine efficacy in the 8 Regions was evaluated and the evidence of safety of monovalent and combined varicella vaccines are presented. The strategy for introducing universal varicella vaccine in the pediatric immuni

[Two cases of congenital varicella syndrome: plea for the varicella vaccine].

We describe two cases of congenital varicella. The first presented with cutaneous aplasia and scars; the second with skin abnormalities, limb atrophy, limb paresis, Horner's syndrome and liver calcifications: prognosis was poor in this case. After reviewing the published cases of congenital varicella, we advocate the use of varicella vaccine in seronegative women before pregnancy.
