Children's Surgery: The Missing Link in the Global Child Survival Agenda.

Global child health is at an inflection point. Under-five mortality has fallen substantially over recent decades, yet progress is slowing and preventable deaths and disability remain concentrated in low- and middle-income countries. As mortality from vaccine-preventable diseases and other infections declines, birth defects and other surgically treatable conditions account for an increasing share of the residual burden. For many children, access to timely, safe and affordable surgical, anesthesia and perioperative care is the difference between survival, disability and death. Yet these services

Health-seeking in fragile and conflict-affected settings: how armed violent conflict shapes maternal and child health-seeking behaviours.

BACKGROUND: Maternal and child mortality rates remain unacceptably high in fragile and conflict-affected settings, where insecurity, displacement, and systemic disruption hinder access to healthcare. Nigeria is one of the countries most severely affected, and continues to experience widespread maternal and neonatal deaths despite global and national efforts to achieve SDG 3.1. This study explored stakeholders’ perspectives on how armed conflicts shapes maternal and child health-seeking behaviours and outcomes in northeastern Nigeria. METHODS: A qualitative approach was employed, involving Key …

Portal hypertension: contraindication to liver surgery?

INTRODUCTION: In recent decades liver resection has become a safe procedure, mainly because of better patient selection. Despite this progress, however, outcomes of hepatectomy in cirrhotic patients with portal hypertension are still uncertain. The aim of this study was to elucidate early and long-term outcomes of liver resection in these patients. METHODS: Between 1985 and 2003, a total of 245 cirrhotic patients underwent hepatectomy for HCC. Altogether, 217 patients were eligible for this analysis and were divided into two groups according to the presence of portal hypertension at the time

Evaluation of a package of continuum of care interventions for improved maternal, newborn, and child health outcomes and service coverage in Ghana: A cluster-randomized trial.

BACKGROUND: In low- and middle-income countries (LMICs), the continuum of care (CoC) for maternal, newborn, and child health (MNCH) is not always complete. This study aimed to evaluate the effectiveness of an integrated package of CoC interventions on the CoC completion, morbidity, and mortality outcomes of woman-child pairs in Ghana. METHODS AND FINDINGS: This cluster-randomized controlled trial (ISRCTN: 90618993) was conducted at 3 Health and Demographic Surveillance System (HDSS) sites in Ghana. The primary outcome was CoC completion by a woman-child pair, defined as receiving antenatal ca

Geographical disparities in infant mortality in the rural areas of China: a descriptive study, 2010-2018.

BACKGROUND: The infant mortality rate (IMR) is considered a basic measure of public health for countries around the world. The specific aim of our study was to provide an updated description of infant mortality rate among different regions in rural China, and assess the trends and causes of the IMR geographical disparities. METHODS: Data were collected from China's Under-5 Child Mortality Surveillance System(U5CMSS). The annual number of deaths and causes of death were adjusted using a 3-year moving average underreporting rate based on annual national data quality control results. The average…

Is routine Vitamin A supplementation still justified for children in Nepal? Trial synthesis findings applied to Nepal national mortality estimates.

BACKGROUND: The World Health Organization has recommended Vitamin A supplementation for children in low- and middle-income countries for many years to reduce child mortality. Nepal still practices routine Vitamin A supplementation. We examined the potential current impact of these programs using national data in Nepal combined with an update of the mortality effect estimate from a meta-analysis of randomized controlled trials. METHODS: We used the 2017 Cochrane review as a template for an updated meta-analysis. We conducted fresh searches, re-applied the inclusion criteria, re-extracted the d…

Health of Aboriginal and Torres Strait Islander children and their grandparents: a Western Australian retrospective cohort study.

BACKGROUND: In Australian Aboriginal and Torres Strait Islander communities, childcare is traditionally shared by kin. Little is known about how grandparental care impacts Aboriginal child health and evidence from other countries is mixed. We explored relationships between grandparental health (a proxy for grandparental care) and health and health service use by Aboriginal children born in Western Australia from 2000 to 2013. METHODS: This is a retrospective cohort study using linked administrative health data. Outcomes were child mortality, hospital admissions, and emergency department (ED) …

Estimating the number of deaths averted from 2008 to 2020 within the Ethiopian CMAM programme.

The management of wasting in Ethiopia is heavily reliant on the Community-based Management of Acute Malnutrition (CMAM) programme that has been implemented in more than 18,000 service delivery points scattered across the country. Despite the full-scale implementation of the CMAM, the number of child death averted, and the cost per child death averted remains unknown. This study aimed to estimate the cost and the number of child death averted by the CMAM programme between 2008 and 2020. Using data from routine monitoring of the CMAM programme, we estimated the excess mortality averted by the pr

Enhancing the child survival agenda to promote, protect, and support early child development.

High rates of child mortality and lost developmental potential in children under 5 years of age remain important challenges and drivers of inequity in the developing world. Substantive progress has been made toward Millennium Development Goal (MDG) 4 to improve child survival, but as we move into the post-2015 sustainable development agenda, much more work is needed to ensure that all children can realize their full and holistic physical, cognitive, psychological, and socio-emotional development potential. This article presents child survival and development as a continuous and multifaceted pr…

Childhood diarrhoea: failing conventional measures, what next?

BACKGROUND: Diarrhoea is one of the leading causes of infant mortality. This article analyzes its contribution towards the realization of millennium development goal number 4 (MDG-4). METHODS: A PubMed search using keywords acute infant diarrhea together with prevalence, management, or prevention 23 of the 634 generated articles were reviewed for inclusion. RESULTS: WHO first expressed concern about diarrhoeal mortality in 1979. Two decades later it reported diarrhoea as the second leading cause of infant mortality worldwide. The annual death toll of 1.5 million is greater than AIDS, malaria

Effect of mass distribution of azithromycin for trachoma control on overall mortality in Ethiopian children: a randomized trial.

CONTEXT: Mass oral azithromycin distribution to affected communities is a cornerstone of the World Health Organization's trachoma elimination program. Antibiotics are provided to target the ocular strains of chlamydia that cause trachoma, but may also be efficacious against respiratory disease, diarrhea, and malaria--frequent causes of childhood mortality in trachoma-endemic areas. OBJECTIVE: To compare mortality rates of participants aged 1 to 9 years in treated communities with those in untreated communities. DESIGN, SETTING, AND PARTICIPANTS: We conducted a cluster-randomized clinical tri

Epidemiologic differences between sexual and physical child abuse.

Sexual and physical child abuse are assumed to differ; however, these differences have not been well characterized epidemiologically. Furthermore, despite assumed differences, these types of abuse are often analyzed as one entity. This can have significant effects on assessment of risk and recommendations for intervention. We compared 735 cases of sexual abuse and 3,486 cases of nonsexual physical abuse confirmed by the Georgia Department of Protective Services. Sexual and physical child abuse cases differed in age, sex, and relationship of perpetrators and victims; demographic and socioeconom

Effects of compulsory and free basic education policies and COVID-19-related educational disruptions on offspring mortality in low-income and middle-income countries: a quasi-experimental and modelling study.

BACKGROUND: Compulsory and free basic education policies (hereafter called education policies) are essential for enhancing educational attainment, which has been linked to improved offspring health outcomes. However, existing studies do not provide strong evidence with causal inference on how these policies influence offspring mortality in low-income and middle-income countries (LMICs). Furthermore, although COVID-19 has substantially disrupted education systems in LMICs, its long-term consequences for offspring mortality remain unclear. METHODS: We extracted individual-level data from the na

Sex differences and sustained excess in mortality among discordant twins in Matlab, Bangladesh: 1977-1985.

This paper reports findings from a study that compared survival of 204 pairs of liveborn twins discordant for sex and a random sample of 2371 singleton live births from the same population in Matlab, Bangladesh during the period 1977-1985. The discordant twins showed no evidence of sex differential in neonatal survival. They had neonatal mortality of 287 and infant mortality of 468, both per 1000 live births. These rates were respectively five and four times the rate for singletons in the study. Survival from 12 months through age nine years was similar overall for both discordant twins and si

Impact of Cirrhosis Etiology and Intensive Care Accessibility on Upper Gastrointestinal Bleeding: A Prospective Observational Study.

BACKGROUND: Upper gastrointestinal bleeding (UGIB) is a significant cause of cirrhosis decompensation; however, there is still controversy on risk factors for poor outcomes. This study aims to explore the impact of additional variables, such as liver disease etiology and intensive care unit (ICU) accessibility, on mortality, rebleeding, and infection rates. METHODS: Cox regression analysis was employed to identify predictors associated with mortality and rebleeding, while Poisson regression analysis was utilized to assess associations with infections. RESULTS: A total of 228 patients were in

Group B streptococcal bacteremia in non-pregnant adults: results from two Korean centers.

Patients with liver cirrhosis (LC) have impaired immunity and are, thus, predisposed to infection. Few studies have attempted to evaluate group B streptococcal (GBS) bacteremia in LC patients. A retrospective study of patients with GBS bacteremia was performed at the Dongguk University Ilsan Hospital and National Health Insurance Service Ilsan Hospital over a 13-year period (October 2000 to July 2013). During the study period, 97 patients with GBS bacteremia were enrolled. The median age of the patients was 67 years and 54 % were men. Among them, 23 (24 %) patients were classified as LC patien
