How does COVID-19 impact intrafamilial child sexual abuse? Comparison analysis of reports by practitioners in Israel and the US.

BACKGROUND: There is consensus in child sexual abuse (CSA) literature that intrafamilial child sexual abuse (IFCSA) has a tremendous impact on children and families while simultaneously creating challenges for practitioners. COVID-19 impacted countries worldwide and generated a global crisis resulting in impacts on daily life, however, it's effect on IFCSA is unknown. OBJECTIVE: This study aimed to compare professional perspectives and experiences working with IFCSA with respect to the context of the COVID-19 pandemic within the United States and Israel. PARTICIPANTS AND SETTING: Participant

Children at the Intersection of Pediatric Palliative Care and Child Maltreatment: A Vulnerable and Understudied Population.

CONTEXT: Concerns for child maltreatment can complicate the provision of pediatric palliative care (PPC). Little is known about the vulnerable population of children with life-threatening conditions involved with PPC and state Child Protective Services (CPS) or hospital Child Protection Teams (CPTs). More information is needed to inform and optimize collaborative care. OBJECTIVES: Define and describe the population of children with PPC involvement for whom there was concern for maltreatment. METHODS: Single-center retrospective chart review of children with PPC involvement for whom there was…

Effect of child abuse and neglect on risk behaviors in inner-city minority female adolescents and young adults.

BACKGROUND: Over six million children each year are referred to child protective services for child abuse (sexual, physical and emotional) and neglect (physical and emotional). OBJECTIVE: While the relationship between child sexual abuse and sexually transmitted infections has been documented, there has been little research regarding the effects of other forms of maltreatment. PARTICIPANTS AND SETTING: 882 inner-city females aged 12-20 years of age seen at a large adolescent and young adult (AYA) health center in New York City between 2012-2017. METHODS: History of maltreatment was assessed…

Three-Year Custody Outcomes Among Infants Investigated by Child Protection Systems for Prenatal Substance Exposure in California.

OBJECTIVE: Infants affected by prenatal alcohol and drug use are more likely to be removed from parental custody than those in the general population, although it is unclear whether their custody outcomes differ from infants investigated by child protection systems (CPS) for other reasons. This analysis seeks to compare trajectories of involvement and custody outcomes among infants investigated by CPS with and without documentation of prenatal substance exposure (PSE). METHOD: We used vital birth records linked to administrative CPS records to examine the timing of system involvement and 3-ye

Dyadic emotional and behavioral experiences in pediatric leukaemia: An interpretative phenomenological analysis of parent-child dynamics in Pakistan.

PURPOSE: Pediatric leukaemia disrupts children's emotional development and places sustained psychological demands on parents. In Pakistan, formal psychological rehabilitation services remain limited, and little is known about how emotional coping unfolds within parent-child relationships during illness. This study aims to explore the lived emotional and behavioral experiences of children with leukaemia and their parents using a dyadic interpretative framework. METHODS: An interpretative phenomenological analysis (IPA) was conducted with 15 parent-child dyads recruited from the pediatric oncol

Can audit and feedback improve health service readiness and delivery outcomes in a low-resource setting? Effectiveness results of the IDEAs strategy from central Mozambique.

Limited evidence exists on audit and feedback(A&F) in low-resource contexts. We tested the Integrated District Evidence-to-Action (IDEAs), a multicomponent A&F strategy in Mozambique's maternal and child health (MCH) services. IDEAs include A&F meetings, readiness assessments, and facility support. We report the effectiveness results. IDEAs were implemented in 2016-2020 across two provinces, 12 districts, and 154 primary health facilities in Mozambique. We assessed 1) ten service delivery outcomes across antenatal, maternity, postpartum, childcare, and reproductive health services and 2) five

Pneumatosis Intestinalis Due to Child Abuse.

Pneumatosis intestinalis (PI) and the presence of portal venous gas (PVG) are commonly considered pathognomonic for necrotizing enterocolitis in the neonatal period; however, these 2 radiographic findings have been documented in all age groups in a variety of clinical settings and medical conditions including respiratory, cardiac, rheumatologic, gastrointestinal disorders, and traumatic injury. In children, intramural dissection of intestinal gas in the absence of clinical symptoms suggestive of necrotizing enterocolitis should raise concern for a traumatic etiology, including injuries sustain

Factors influencing the implementation of health care reform: an examination of the Concurrent Care for Children provision.

In the wake of the 2013 Presidential reelection, states now face the reality of implementing the healthcare reform, including ACA section 2302 (Concurrent Care for Children). The purpose of our study was to examine the influence of economic, political, and legal factors on state implementation of ACA 2302. Using data from 2010 to 2012, our analysis revealed that for early implementers economic, political or legal factors did not influence implementation of ACA 2302. In 2011, states that were engaged in Medicaid cost containment efforts were more likely to implement ACA 2302 and in 2012, states

A natural language processing and deep learning approach to identify child abuse from pediatric electronic medical records.

Child physical abuse is a leading cause of traumatic injury and death in children. In 2017, child abuse was responsible for 1688 fatalities in the United States, of 3.5 million children referred to Child Protection Services and 674,000 substantiated victims. While large referral hospitals maintain teams trained in Child Abuse Pediatrics, smaller community hospitals often do not have such dedicated resources to evaluate patients for potential abuse. Moreover, identification of abuse has a low margin of error, as false positive identifications lead to unwarranted separations, while false negativ

Public Policy Approaches to Population Health.

The United States spends more per capita on health care than any other country and has worse health outcomes. Public policy can influence almost all aspects of health. Publicly funded insurance pays for health care for more than half of Americans. For individuals and employers that purchase insurance, the regulatory environment influences which insurance products can be purchased as well as by whom and for how much. Health policy levers at federal, state, and local levels can exert financial and regulatory influence over care. This influence determines what is health care, with increasing fede

Recognizing children's emotions in child abuse and neglect.

Past research has suggested that parents' ability to recognize their children's emotions is associated with an enhanced quality of parent-child interactions and appropriateness of parental caregiving behavior. Although this association has also been examined in abusive and neglectful parents, the results are mixed and do not adequately address child neglect. Based on the Social Information Processing model of child abuse and neglect, we examined the association between mothers' ability to recognize children's emotions and self- and professionals-reported child abuse and neglect. The ability to

Safeguarding Capabilities in Preventing Child Sexual Abuse: Exploratory Factor Analysis of a Scale Measuring Safeguarding Capabilities in Youth-Serving Organizations Workers.

Media reports and government enquiries have shone a spotlight on institutional child sexual abuse (CSA) globally. With youth-serving organizations seeking to identify how to improve policies and procedures developed to protect children, a gap exists in research and organizational quality assurance procedures. A new tool is needed to measure the capability of workers to implement and support effective child-safeguarding policies and practices. To address this, our aim was to develop the Safeguarding Capabilities in Preventing Child Sexual Abuse Scale. Participants (n = 345) from a range of yout

AF-CBT for Families Experiencing Physical Aggression or Abuse Served by the Mental Health or Child Welfare System: An Effectiveness Trial.

The Partnerships for Families project is a randomized clinical trial to evaluate the effectiveness of Alternatives for Families: A Cognitive Behavioral Therapy (AF-CBT), an evidence-based treatment (EBT) for families who are at risk of or have histories of child physical abuse. Across 10 agencies whose programs were supported by referrals from the mental health or child welfare system, individual providers were randomized to receive AF-CBT training ( n = 90) in a 6-month learning community or treatment as usual (TAU; n = 92) which provided trainings per agency routine. We recruited families se

Superior Mesenteric Artery Syndrome in Down Syndrome: A Case Report.

Down syndrome is a common chromosomal abnormality, which is associated with a wide spectrum of multiorgan complications, including gastrointestinal disorders. Superior mesenteric artery syndrome is a rare type of small intestinal obstruction caused by compression of the duodenum between the superior mesenteric artery anteriorly and the abdominal aorta posteriorly. A 12-year-old boy with Down syndrome came to the emergency department with a first seizure secondary to electrolyte disturbances. He had been having recurrent episodes of bilious vomiting. Computed tomography of the abdomen revealed

Child abuse followed by fatal systemic Pseudomonas aeruginosa infection.

Child abuse has become an increasingly serious diagnostic challenge for physicians. The clinical manifestations include malnutrition and sometimes infection. In fact, stress in children has been reported to increase corticosteroid levels. As a consequence, the thymus begins an involution process, producing a severe impairment in cellular and humoral immunity. Here, we report the case of a 7-year-old child who suffered a prolonged history of abuse and died from a systemic Pseudomonas aeruginosa infection. An initial local chronic infection propagated to the pelvic lymph nodes in an immunologica

Encountering children and child soldiers during military deployments: the impact and implications for moral injury.

Background: During a deployment, soldiers must make seemingly impossible decisions, including having to engage with child soldiers. Such moral conflicts may continue to affect service members and veterans in the aftermath of a deployment, sometimes leading to severe moral distress, anguish, and personal crises. Service providers have increasingly argued that as a diagnosis, Post-Traumatic Stress Disorder (PTSD) cannot account for these deeply personal and painful moral conflicts. In light of this, the concept of moral injury has been introduced to better capture the profound forms of guilt and
