Aftereffects of a tainted meal.

A 58-year-old man presented with profuse diarrhea, abdominal distention, flatulence, weakness, and a 15-lb weight loss. The initial symptoms of nausea and vomiting had begun a week earlier, within hours of eating chicken that had not been well wrapped when purchased. Two days after symptom onset, he received intravenous rehydration therapy at another hospital. With resolution of the nausea and vomiting, he felt slightly better and went home against medical advice. He had not had fever, chills, hematochezia, melena, hematemesis, or dysuria.

Drug-induced fever: a clinical report and challenge test with calcium dobesilate.

A 54-year-old man was receiving calcium dobesilate for retinopathy and after 8 days he presented fever of more than 39 degrees C, generalized myalgia, chills and headache. Other causes of fever were ruled out. A challenge test was done with a single therapeutic oral dose of calcium dobesilate and fever appeared 20 h later and lasted 8 h. Our patient fulfils Young's stringent criteria for drug fever. To our knowledge drug-induced fever due to calcium dobesilate has not been reported previously.

Surgical implications of stone impaction in the gall-bladder neck with compression of the common hepatic duct (Mirizzi's syndrome).

Four surgically proven cases of Mirizzi's syndrome are reported and the literature reviewed. The clinical presentation and radiological investigations are described and discussed. All four patients presented with right upper quadrant pain and progressive jaundice. Three patients reported intermittent chills and fever. The percutaneous transhepatic cholangiographic appearances in three cases were diagnostic of this condition. The diagnosis was not made in the fourth case. The important role of the cholangiographer in entertaining the diagnosis and alerting the surgeon to a difficult cholecystec…

Cryopreservation of organs by vitrification: perspectives and recent advances.

The cryopreservation of organs became an active area of research in the 1950s as a result of the rediscovery of the cryoprotective properties of glycerol by Polge, Smith, and Parkes in 1949. Over the ensuing four decades of research in this area, the advantages of vitrification, or ice-free cryopreservation, have become apparent. To date, experimental attempts to apply vitrification methods to vascularized whole organs have been confined almost entirely to the rabbit kidney. Using techniques available as of 1997, it was possible to vitrify blood vessels and smaller systems with reasonable succ

[Chronic abdominal pain in elderly patient].

A 75-year-old female patient with autosomal-dominant polycystic kidney disease and a history of kidney transplantation presented to the emergency department with dysuria, chills and abdominal pain. A detailed review of her medical history revealed that her acute pyelonephritis was masking a chronic abdominal pain issue accompanied by significant bilirubinemia. MRCP showed a complete cystic transformation of the left liver lobe and liver fibrosis. Consequently, the diagnosis of Caroli syndrome, a rare cystic liver disease with fibrosis, was established despite her advanced age.

Renal actinomycosis with concomitant renal vein thrombosis.

Renal actinomycosis is a rare infection caused by fungi of the genus Actinomyces. A 74-year-old male was admitted to our hospital because of gross hematuria with urinary symptoms and intermittent chills. Computed tomography of the abdomen showed thrombosis in the left renal vein and diffuse, heterogeneous enlargement of the left kidney. After nephrectomy, sulfur granules with chronic suppurative inflammation were seen microscopically, and the histopathological diagnosis was renal actinomycosis. Our case is the first report of renal actinomycosis with renal vein thrombosis.

A case of endobronchial aspergilloma presenting as a broncholith.

A 75-year-old woman with no history of pulmonary disease was admitted to the hospital complaining of fever and chills. Chest computed tomography and bronchoscopy suggested a broncholith-like calcified endobronchial lesion and postobstructive pneumopathy in the left lower bronchus. The mass obstructing the airway was removed using grasping forceps and it was diagnosed pathologically as endobronchial aspergillosis. Endobronchial aspergilloma presenting radiographically as a calcification without adjacent calcified lymph nodes is a rare condition, even though pulmonary fungal diseases including e…

Sternoclavicular joint infection: a case report.

Infection of the sternoclavicular joint (SJI) is a rare problem accounting for approximately 1 % of cases of septic arthritis. Patients typically present with symptoms of localized pain lasting a period of several weeks with or without systemic signs of fever and chills. Confirmation is made by aspirating the joint, and broad spectrum antibiotics should be tailored to treat the identified organisms. SJI can be treated conservatively with intravenous antibiotics and repeat imaging, but surgical intervention is required if patients present with an abscess, osteomyelitis or mediastinitis.

[Pain in the throat due to acute suppurative thyroiditis caused by Salmonella].

A 53-year-old woman presented with fever accompanied by chills and an extremely painful swelling of her right thyroid lobe. She was initially diagnosed as having subacute thyroiditis, but after 14 days her disease appeared to be caused by a destructive suppurative thyroiditis due to Salmonella group C. A pre-existing hyperplastic nodule in the right thyroid lobe was the predisposing factor. Antibiotics were given for several weeks and surgical drainage was performed. Finally a hemithyroidectomy was done to eliminate the predisposing factor.

Babesiosis.

A case of human babesiosis is presented. This case emphasizes the need to consider tick-borne disease in anyone who presents with prolonged and undulating fevers, chills, headache, myalgias, and arthralgias. This holds true particularly in areas endemic for tick-borne diseases, even in the absence of a history of tick bite. These symptoms, associated with signs of intravascular hemolysis, thrombocytopenia, and renal insufficiency in a patient who resides in, or with recent travel to, the Northeastern United States, strongly suggest a diagnosis of babesiosis.

Update: outbreak of acute febrile respiratory illness among college students--Acapulco, Mexico, March 2001.

On March 30, CDC was notified by the Pennsylvania Department of Health of an acute febrile respiratory illness characterized by fever, chills, dry cough, chest pain, and headache among college students who traveled to Acapulco during March 2001. Initial laboratory testing indicated that most students had histoplasmosis, an infection caused by the soil-inhabiting fungus, Histoplasma capsulatum. While in Acapulco, most ill students had stayed at the Calinda Beach Hotel. This report updates the investigation of the outbreak and presents possible evidence of ongoing transmissions.

Respiratory distress, weakness, and electrolyte abnormalities.

A 56-year old man was admitted to the hospital with malaise, weakness, and fatigue. He was short of breath and had bilateral foot edema. Even though he had been very active a month earlier, he could no longer climb stairs. For the last two weeks, he had had a cough producing green sputum, a "tight feeling" in his chest, polyuria, and polydipsia. He had not had radiating chest pain, palpitations, leg pain or erythema, hemoptysis, diaphoresis, flushing, fever, chills, nausea, vomiting, diarrhea, or a loud snore.
