[Contribution to the study of cholecystoses. Clinical, radiological and pathological analysis (author's transl)].

Cholecystoses is a generic term introduced by Colesson and Jutras for a group of abnormalities of the gallbladder that appear to be separate from inflammatory diseases. Cholecystoses imply a benign proliferation of normal tissue elements, whereas cholecystosis indicates a pathologic process that is distinct from inflammation. 1 072 cholecystectomies were examined. 180 cholecystoses were diagnosed, 162 were associated with gallstones. There was no difference concerning the clinical symptom between cholecystoses and cholelithiasis. Radiological study confirms and clarifies the nature and charact

[Clinical experiences in rapid infusion of jotroxamide in cholegraphie (author's transl)].

Jotroxamid has been infused intravenously in 60 patients within 10 minutes for cholegraphy. At the evaluation, contrast medium side effects, heterotopie excretion, quality of contrast and length of examination time have been considered. Reactions to the contrast medium were low compared with the results of other authors, an increase of cases with roentgenographie demonstrable heterotopic excretion could not be observed. Opacification of the biliary duct system was except in 2 cases very good. The total examination time was compared with the conventional infusion rate 30 minutes less.

Cholecystostomy.

The role of cholecystostomy in the sugical treatment of gallstones has been considered. In a consecutive series of 558 patients who underwent surgery for gallstones, 30 (5-4 per cent) had cholecystostomy alone and a further 17 (3-1 per cent) cholecystostomy combined with exploration of the bile ducts. Cholecystostomy was done because cholecystectomy was technically very hazardous in 15 patients. In the remaining patients cholecystostomy was preferred to cholecystectomy because o f the poor general condition of the patient or the presence of pancreatitis. Of the 47 patients submitted to cholecy

The biliary system in primary biliary cirrhosis. A study by endoscopic retrograde cholangiopancreatography.

Endoscopic retrograde cholangiograms from 23 patients with primary biliary cirrhosis (PBC), 10 controls with either normal livers or hepatocellular disease, and 4 patients with sclerosing cholangitis, were compared. Of the PBC group, 39% had gallstones. The calibers of the common bile duct and left and right intrahepatic ducts were comparable in the PBC and control groups. The small intrahepatic ducts, while normal in the control group were abnormal in 7 of the 23 PBC patients. These small ducts were irregular in caliber and had a tortuous course. The changes were not related to the presence o

Gallbladder Fossa Abscess Masquerading as Cholecystitis After Cholecystectomy.

We present a case of a 59-y-old woman who had undergone cholecystectomy and was subsequently found to have an abscess within the gallbladder fossa. A hepatobiliary scan using (99m)Tc-diisopropyliminodiacetic acid demonstrated the characteristic rim sign, a photopenic defect surrounded by a rim of mildly increased activity immediately adjacent to the gallbladder fossa. The rim sign was thought to be the result of reactive inflammation in the hepatic tissue adjacent to a postoperative abscess within the gallbladder fossa.

Contemporary minimally invasive approaches to the management of acute cholecystitis: a review and appraisal.

Acute cholecystitis is one of the most common emergency admissions in surgical practice. This review appraises the available evidence from the English-language literature regarding the minimally invasive approaches to the management of this condition. The following aspects of care are reviewed and appraised: (1) the diagnostic criteria for acute cholecystitis, (2) the optimal timing for cholecystectomy (early, delayed, or interval surgery), (3) the optimal approach to cholecystectomy (laparoscopic versus open), (4) the role of intraoperative cholangiography, and (5) the management of patients
