Cost-Effectiveness of Internet of Things-Based Management of Home Noninvasive Positive Pressure Ventilation in Patients With Chronic Obstructive Pulmonary Disease and Hypercapnic Chronic Respiratory Failure: Trial-Based Economic Evaluation.

BACKGROUND: The management of chronic obstructive pulmonary disease (COPD) places a significant burden on health care systems worldwide. Home noninvasive positive pressure ventilation (NPPV) is an established treatment option associated with significant benefits for patients with COPD and hypercapnic chronic respiratory failure. Internet of Things (IoT)-based management may improve communication between patients and physicians and strengthen the integration and comprehensiveness of home NPPV telemonitoring. However, the economic value of such systems remains insufficiently understood. OBJECTI

Overview of methodological issues in the study of chronic care populations.

The intent of the methodology section of this volume was to provide an overview of recent thinking about analytic techniques that can be used to study chronic care populations, particularly dementia SCUs. As discussed earlier in this article, although not unique to these populations, longitudinal analysis of chronic care populations frequently involves several problems: nonequivalent comparison groups, unbalanced designs, censoring and attrition, autocorrelation (correlated repeated measures) and heterogeneous correlations across repeated measures. The last 5 years has witnessed considerable c

Number of chronic diseases and cognitive function among the elderly in China: a moderated mediation model.

PURPOSE: Despite the wealth of data on the role of chronic disease comorbidity in shaping cognitive dysfunction in older adults, a comprehensive view of this dynamic interplay remains a frontier. This study will reveal the intricate interactions between the number of chronic diseases and cognitive function in the elderly, based on the perspective of cognitive function in patients with multiple chronic diseases. METHODS: Our study was based on the data from the 2023 China Psychological Care for the Elderly Action Survey, and the SPSS 26.0 (IBM Corp., Armonk, NY, United States) software package

Chronic Lymphocytic Leukemia Care and Beyond: Navigating the Needs of Long-Term Survivors.

Chronic lymphocytic leukemia (CLL) treatment has undergone a significant evolution with a shift from historical chemotherapeutic regimens to targeted therapies such as Bruton tyrosine kinase (BTK) and BCL-2 inhibitors. These advancements have been associated with a notable improvement in survival rates with a transformation of CLL into a chronic and manageable condition for most persons with this disease. However, as a consequence of improved outcomes, long-term CLL survivors now face emergent challenges which include a risk of infections, cardiovascular complications, and secondary malignanci

Reproducing real-world clinical prediction models using the DIVE platform: A comparative validation study across three chronic diseases.

OBJECTIVES: The aim of this analysis is to evaluate the performance and reproducibility of the Python-based Data Insight Validation Engine (DIVE), a modular analytics interface implemented in Python to facilitate real-world evidence (RWE) generation from clinical (e.g. primary care) data. The platform was used to replicate three previously published studies focused on chronic kidney disease (CKD), chronic obstructive pulmonary disease (COPD), and severe asthma, each originally developed using conventional statistical environments. METHODS: Using a primary care data source, DIVE was employed t

[Patient navigation in severe mental illness-challenges, models, and perspectives].

Severe mental illness (SMI) is associated with chronic courses, significant functional impairments, and increased mortality. At the same time, care for this population is characterized by fragmentation, lack of continuity, and insufficient coordination. Against this background, patient navigation has gained importance as an approach to improving the quality and efficiency of care. The aim of this narrative review is to present key evidence-based models of care for individuals with SMI, to analyze structural challenges in service provision, and to derive principles for effective patient navigat

Risk Factors Associated With Transition From Acute to Chronic Low Back Pain in US Patients Seeking Primary Care.

IMPORTANCE: Acute low back pain (LBP) is highly prevalent, with a presumed favorable prognosis; however, once chronic, LBP becomes a disabling and expensive condition. Acute to chronic LBP transition rates vary widely owing to absence of standardized operational definitions, and it is unknown whether a standardized prognostic tool (ie, Subgroups for Targeted Treatment Back tool [SBT]) can estimate this transition or whether early non-guideline concordant treatment is associated with the transition to chronic LBP. OBJECTIVE: To assess the associations between the transition from acute to chron

Decentralization of hepatitis B care in sub-Saharan Africa: study protocol for a prospective cohort assessing three models of care in rural Ethiopia.

BACKGROUND: Improving treatment uptake among people with chronic hepatitis B (CHB) infection who live outside urban centres is crucial to achieving the World Health Organization (WHO) elimination targets. Furthermore, as the number of deaths due to hepatitis B in Africa is rising, there is a need for new models of decentralized care adapted to the local context. The main aim of this study is to compare three different models of decentralized care for CHB treatment in Ethiopia, both from a clinical perspective and an implementation perspective. METHODS: This will be prospective cohort study com

At the intersection of anger, chronic pain, and the brain: A mini-review.

Chronic pain remains one of the most persistent healthcare challenges in the world. To advance pain treatment, experts have recently introduced research-driven subtypes of chronic pain based on proposed underlying mechanisms. Nociplastic pain (e.g., nonspecific chronic low back or fibromyalgia) is one such subtype which may involve a greater etiologic role for brain plasticity, painful emotions induced by life stress and trauma, and unhealthy emotion regulation. In particular, correlational and behavioral data link anger and the ways anger is regulated with the presence and severity of nocipla

Vascular biomedicine in an era of chronic disease and multimorbidity.

It is increasingly common that patients present with more than one disease and that diseases are chronic in nature. Cardiovascular conditions such as hypertension, heart failure and stroke, renal diseases and cardiometabolic conditions such as diabetes are prime examples of chronic diseases which pose major challenges in contemporary healthcare provision. The complex features of multimorbidity call for precision medicine approaches that take comorbidity and chronicity into account. The research basis of chronic disease and multimorbidity, however, is currently in its infancy. This applies to a

Enhancing Large Language Models With AI Agents for Chronic Gastritis Management: Comprehensive Comparative Study.

BACKGROUND: The prevalence of chronic gastritis is high, and if not intervened in a timely manner, it may eventually lead to gastric cancer. Managing chronic gastritis essentially requires comprehensive lifestyle changes. However, the current health care environment does not support continuous follow-up by professional health care providers, making self-management a key component of postdiagnosis care. Increasingly, researchers are exploring the use of large language models (LLMs) for patient management. However, LLMs have limitations, including hallucinations, limited knowledge scope, and lac

Integrating Chronic Disease Management and Harm Reduction for Youth with Juvenile Idiopathic Arthritis Amid Canada's Overdose Crisis.

Juvenile idiopathic arthritis (JIA) is a chronic autoimmune condition in children that often requires long-term pain management, which can include opioid use. In the context of Canada's ongoing overdose crisis, youth with JIA face risks due to potential opioid dependency and exposure to toxic drug supplies. This commentary proposes an integrated approach combining chronic disease management with harm reduction strategies specifically tailored for JIA patients. By incorporating multidisciplinary care, opioid stewardship, and harm reduction measures, this approach aims to address the dual challe

Concepts for the Development of Person-Centered, Digitally Enabled, Artificial Intelligence-Assisted ARIA Care Pathways (ARIA 2024).

The traditional healthcare model is focused on diseases (medicine and natural science) and does not acknowledge patients' resources and abilities to be experts in their own lives based on their lived experiences. Improving healthcare safety, quality, and coordination, as well as quality of life, is an important aim in the care of patients with chronic conditions. Person-centered care needs to ensure that people's values and preferences guide clinical decisions. This paper reviews current knowledge to develop (1) digital care pathways for rhinitis and asthma multimorbidity and (2) digitally ena

Outcome of home care for technology-dependent children: success of an independent, community-based case management model.

Case management is important for successful home care of technology-dependent, respiratory-disabled children. Traditionally, the medical model of hospital-based home care and case management has been used for these children. The outcome may be different from when using independent, community-based home care and case management. We evaluated the results of 28 technology-dependent children [23 receiving mechanical ventilation and 5 receiving continuous positive airway pressure (CPAP)] from 8 hospitals, who utilized an independent, community-based, case management group to coordinate home care. Psychiatric morbidity in a gynaecology clinic in Nigeria.

Psychological disorders among 233 women attending a gynaecology out-patient clinic was assessed by a two-stage screening procedure, using the GHQ-30 and the PSE. The prevalence of psychiatric disorders was found to be 35.2%. Psychiatric morbidity was significantly associated with a history of induced abortion, previous marriages, having no children, complaints of menstrual abnormalities, chronic pelvic pain and having unsupportive husbands. It is suggested that more attention needs to be paid to the psychological health of patients with gynaecological disorders (in line with the biopsychosocia

Treatable traits models of care.

Treatable traits is a personalized approach to the management of respiratory disease. The approach involves a multidimensional assessment to understand the traits present in individual patients. Traits are phenotypic and endotypic characteristics that can be identified, are clinically relevant and can be successfully treated by therapy to improve clinical outcomes. Identification of traits is followed by individualized and targeted treatment to those traits. First proposed for the management of asthma and chronic obstructive pulmonary disease (COPD) the approach is recommended in many other ar

Primary care need and engagement by people with criminal legal involvement: Descriptive and associational analysis using retrospective data on the entire population ever detained in one southeastern U.S. county jail 2014-2020.

More than 7 million people are released each year from U.S. jails or prisons, many with chronic diseases that would benefit from primary care in their returning communities. The objective of this study was to provide an in-depth, payer-agnostic description and associational analysis of primary care need and utilization by all individuals ever detained in one county detention facility over a 7-year period. Detention records 2014-2020 were merged with data from an electronic health record with excellent coverage of local primary care, emergency, and hospital services. We found low primary care p

(Un)doing gender in a rehabilitation context: a narrative analysis of gender and self in stories of chronic muscle pain.

PURPOSE: To explore how gender appears in the stories of self-told by men and women undergoing rehabilitation for chronic muscle pain. METHOD: The material, which consists of qualitative interviews with 10 men and 6 women with chronic neck pain, was analyzed from a gender sensitive perspective using narrative method. The analysis was inspired by Arthur Frank's typologies of illness narratives (restitution, chaos and quest). FINDINGS: The women's stories displayed selves that were actively trying to transcend their former identity and life conditions, in which their pain was embedded. Their s

Cost-effectiveness of tapentadol in severe chronic pain in Spain: a cost analysis of data from RCTs.

BACKGROUND: Chronic pain is known to be a significant and common health problem. Tapentadol, a recently developed centrally active, oral analgesic agent is used to treat adults with severe chronic pain that can be adequately managed only with opioid analgesics. Tapentadol has been reported to provide an improved adverse-events (AE) profile compared with other potent opioid analgesics at similar levels of analgesia. OBJECTIVES: The aim of this study was to compare the cost-effectiveness of tapentadol to that of opioids commonly used as first-line treatment of severe, chronic, nonmalignant pain
