BACKGROUND: Clavicle (collarbone) fractures account for around 4% of all fractures; treatment of these fractures is usually non-surgical (conservative). Commonly used treatments are arm slings, strapping, figure-of-eight bandages and splints. This is an update of a Cochrane review first published in 2009.
OBJECTIVES: To evaluate the effects of different methods for conservative (non-operative) treatment for acute (treated soon after injury) middle third clavicle fractures in adolescents and adults.
SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Regist…

We report a case of open fracture of the clavicle with subclavian artery and vein laceration and perforation of the parietal pleural below the first rib that caused massive haemothorax. Emergency thoracotomy and exploration followed by repair of both vessels were able to salvage the patient and the extremity.

OBJECTIVE: An experimental cadaver model was used to assess the effects of a malunited fracture of the middle third of the clavicle on the functional anatomy of the shoulder joint.
METHOD: Anatomic samples were prepared with simulated shortening and axial malposition of the clavicle. From these, alterations in glenoid fossa position were measured and depicted graphically.
RESULTS: Healing of clavicle fractures with bony shortening leads to a ventromedialcaudal shift in glenoid fossa position. The following malpositions of the clavicle lead to the respective glenoid fossa positional changes: …

BACKGROUND: Concomitant ipsilateral clavicle and scapula fractures are uncommon injuries, and their optimal management remains controversial. While surgical treatment has been advocated for selected unstable patterns, the role of conservative treatment, particularly in minimally displaced fractures, has not been clearly established. The aim of this study was to evaluate the prevalence of concomitant clavicle and scapula fractures and to compare the clinical outcomes of conservatively treated patients with those of a matched cohort of patients with isolated clavicle fractures.
METHODS: We retr…

INTRODUCTION: Bipolar clavicle fractures are a simultaneous injury to both ends of the clavicle and represent an exceptionally rare entity with an incidence of <1% of all clavicle fractures. Despite the clavicle being one of the most commonly fractured bone, involvement of both ends is frequently missed or misdiagnosed, leading to complications like persistent pain and compromised function. The rarity of these fractures has also led to a significant gap in established treatment guidelines. We present a case of a bipolar clavicle fracture with sternoclavicular joint dislocation treated successf…

Displaced acromion fractures are commonly treated surgically; however, no implants have been specifically designed for this fracture type. Although various plates have been used, fixation failure and implant-related irritation remain concerns. We report a case of acromion fracture fixation using a proximal clavicle plate. A 77-year-old man presented with right shoulder pain after a fall. Computed tomography revealed an Ogawa type III acromion fracture extending to the spinoglenoid notch. Through a posterior approach along the scapular spine, the fracture was anatomically reduced and fixed with…

CASE: A 17-year-old adolescent boy sustained right lateral clavicle physeal separation with the epiphysis displaced superiorly in the acromioclavicular joint (ACJ). The ossified epiphysis formed a painful nonunion with the clavicular metaphysis. Resection of the epiphysis has achieved an excellent outcome.
CONCLUSION: Children and adolescents with lateral clavicle injuries often develop pseudodislocation of the ACJ in which the metaphysis is displaced due to physeal separation. By contrast, displacement of the unossified epiphysis is extremely difficult to diagnose both radiographically and c…

AIMS: Tobacco, in addition to being one of the greatest public health threats facing our world, is believed to have deleterious effects on bone metabolism and especially on bone healing. It has been described in the literature that patients who smoke are approximately twice as likely to develop a nonunion following a non-specific bone fracture. For clavicle fractures, this risk is unclear, as is the impact that such a complication might have on the initial management of these fractures.
METHODS: A systematic review and meta-analysis were performed for conservatively treated displaced midshaft…

This case study examines the medical complexity of managing a neglected clavicle in a young patient, resulting in a complicated interstitial non-union. Despite initial therapeutic efforts, the fracture was not treated, resulting in significant pain as well as functional and aesthetic disabilities. This case outlines accurate clinical presentation and diagnostic methods. In addition, malunion clavicle fractures require multifaceted therapeutic approaches including surgical interventions, rehabilitation programs, and psychological support. Through comprehensive research and long-term follow-up, …

BACKGROUND This report describes the case of a 9-year-old boy with unilateral fracture of the mandibular condyle and malocclusion managed using a function-generating bite (FGB) orthodontic appliance. The mandible is a frequent site of trauma in children, occurring in 12-56% of facial fractures. Condylar fractures are about 29-52% of mandibular fractures, with high-neck fractures represent 9-12%. Maxillofacial surgery during growth remains controversial, and treatment typically requires not only addressing the fracture but also any associated malocclusion. If a condylar fracture is inadequately

BACKGROUND: Congenital pseudoarthrosis of the clavicle (CPC) is an uncommon entity. Owing to its scarce presentation, treatment of this disorder has not been well established. This study aimed (1) to compare surgical treatment methods that included excision of pseudoarthrosis and iliac crest bone graft and fixate with either the elastic stable intramedullary nail (ESIN) or K-wires or plate and screws, and (2) to assess the clinical outcomes of two different surgical methods.
METHODS: A multi-central retrospective study was performed between 2013 and 2017 in four tertiary teaching hospitals. F…

Acromioclavicular dislocation combined with a midshaft clavicle fracture is a rare traumatic shoulder problem. Various treatment options have been described in literature. We describe a new technique using a hook plate and coracoclavicular sling to replace the ruptured coracoclavicular ligaments in combination with plate osteosynthesis of the clavicle fracture. Furthermore, we provide a short overview of the few cases described in literature and their treatment options.

AIM: In recent years there has been increasing evidence that comminuted and shortened clavicle fractures should be treated surgically. The aim of this study was to assess the outcome of patients who had their clavicle fracture fixed using a pre-contoured, clavicle specific plate from an anterior approach.
METHOD: A retrospective analysis was carried out of all patients treated with pre-contoured clavicle plates from an anterior approach between May 2014 and June 2016.Medical records were analysed, and a phone survey was carried out to assess patient function and note any complications.

BACKGROUND: Clavicle fractures are common, accounting for 2.6% to 4% of all fractures. Eighty per cent of clavicle fractures are located in the middle third of the clavicle. Although treatment of these fractures is usually non-surgical, displaced clavicle fractures may be considered for surgical treatment because of their greater risk of non-union. This is an update of a Cochrane Review first published in 2013.
OBJECTIVES: To assess the effects (benefits and harms) of surgical versus conservative interventions for treating middle third clavicle fractures.
SEARCH METHODS: We searched the Coch…

Acute osteomyelitis of the clavicle is rare in the paediatric age group. We treated a 5-year-old boy who presented initially with fever and left shoulder pain, and subsequently developed swelling in the region of the left clavicle. Group A Streptococcus (GAS) was isolated in blood culture. MRI of the clavicle showed osteomyelitis of the medial clavicle. The child had incision and drainage of his clavicular collection. The child received intravenous benzylpenicillin and oral cephalexin in the initial presentation; he was treated with 2 weeks of intravenous ceftriaxone and 4 weeks of oral penici…
