Pralatrexate : evaluation of clinical efficacy and toxicity in T-cell lymphoma.

INTRODUCTION: Pralatrexate is a novel antifolate agent that belongs to the class of 10-deazaaminopterins. Its clinical efficacy as a single agent in relapsed or refractory peripheral T-cell lymphoma (PTCL) has been established in randomized trials. Treatment with this agent is generally safe. AREAS COVERED: This paper discusses the pharmacokinetics and efficacy of pralatrexate in T-cell lymphoma in clinical trials. In addition, the authors highlight pralatrexate-associated adverse effects and safety concerns. EXPERT OPINION: Although established as a second-line therapy, pralatrexate offers

Problem-solving versus cognitive restructuring of medically ill seniors with depression (PROMISE-D trial): study protocol and design.

BACKGROUND: With an ageing population in most Western countries, people are living longer but often with one or more chronic physical health problems. Older people in physically poor health are at greater risk of developing clinical depression. Cognitive Behavioural Therapy (CBT) and Problem Solving Therapy (PST) have both been found to be efficacious in treating late-life depression, however patients with "multi-morbidity" (i.e. more than one chronic condition) are often excluded from these trials. The aim of this study is to compare the efficacy of CBT and PST in treating older adults who ha

Bendamustine in chronic lymphocytic leukemia and non-Hodgkin's lymphoma.

Bendamustine (Treanda(®); Pharmachemie BV, The Netherlands for Cephalon, Inc., PA, USA) is a unique cytotoxic agent with both alkylating and antimetabolite properties. A growing body of evidence demonstrates its efficacy in a number of hematologic malignancies, and as such, it has been US FDA approved for the treatment of chronic lymphocytic leukemia and non-Hodgkin's lymphoma that has not responded to, or progressed within 6 months of, a rituximab-based regimen. Bendamustine has efficacy both as a single agent as well as in combination with other chemotherapeutics and immunotherapeutics. Here

Bone and bone turnover.

Children with cancer are exposed to multiple influences that may adversely affect bone health. Some treatments have direct deleterious effects on bone whilst others may have indirect effects mediated through various endocrine abnormalities. Most clinical outcome studies have concentrated on survivors of acute lymphoblastic leukaemia (ALL). There is now good evidence that earlier treatment protocols that included cranial irradiation with doses of 24 Gy or greater may result in growth hormone deficiency and low bone mineral density (BMD) in the lumbar spine and femoral neck. Under current protoc

Hepatoblastoma: an oncological review.

The treatment of hepatoblastoma is an example of the great advances made in paediatric oncology over the past few decades. Formerly a disease with a dreadful prognosis, children now have a 5-year survival rate of over 70%. This has been achieved by advances in surgical techniques and the use of chemotherapy. In future, we hope to be able to improve survival for children with high-risk tumours, and decrease the morbidity associated with treatment in the others.

Thalidomide in multiple myeloma.

Recent evidence suggests that angiogenesis is increased in multiple myeloma and has prognostic value in the disease. Based on the increased angiogenesis observed in myeloma, thalidomide (Thalomid) has been studied as antiangiogenic therapy. Although its mechanism of action in myeloma is unclear, several trials show that thalidomide is active in 25% to 35% of patients with relapsed myeloma. Since many patients who respond have failed other active regimens, including transplantation, these results are impressive. Major toxicities include constipation, sedation, skin rash, fatigue, and peripheral

Treatment of disseminated malignant melanoma with cisplatin in combination with whole-body hyperthermia and doxorubicin.

Twenty-three patients with disseminated malignant melanoma were entered in a pilot study conducted from January 1983 to January 1988. There were 14 men and nine women. Fifteen patients were available for final evaluation. The criterion for evaluation was the completion of the two thermochemotherapy cycles. Patients received multiple-drug chemotherapy consisting of cisplatin (80 mg/m2, i.v., day 1) and doxorubicin (50 mg/m2, i.v., day 2); applications were repeated on days 22-29. Cisplatin was administered concurrently with whole-body hyperthermia (WBH) on day 1 (body core temperature 41 +/- 0.

2'-Deoxycoformycin (pentostatin) for lymphoid malignancies. Rational development of an active new drug.

A new antimetabolite, 2'-deoxycoformycin (pentostatin), has striking antitumor activity in several lymphoid neoplasms. Isolated from cultured soil organisms, this purine analogue is a potent inhibitor of adenosine deaminase (ADA), and is thus selectively toxic to lymphocytes. Early clinical trials showed that high doses of pentostatin caused severe and unpredictable toxicity, but responses in refractory lymphoid malignancies were encouraging. Careful pharmacologic studies led to the definition of a safe and effective low weekly dose, at which protracted ADA inhibition occurs in neoplastic cell

The effects of delay in standard treatment due to induction chemotherapy in two randomized prospective studies.

It is often suggested that tumors will respond to induction chemotherapy and result in improved survival for patients with squamous cell carcinoma of the head and neck. Two regimens of induction chemotherapy were studied in separate randomized, prospective trials over the last 6 years. Eighty-three patients with advanced disease were entered into the first study (43/chemotherapy; 40/control), and 60 into the second (27/chemotherapy; 33/control). Patient randomization was stratified by stage (III/IV) and site (oral cavity, oropharynx, nasopharynx, hypopharynx, larynx, paranasal sinuses). The fi

Amelioration of cytotoxic-induced emesis with high-dose metoclopramide, dexamethasone and lorazepam.

A double-blind randomised controlled trial comparing the antiemetic effects of sublingual lorazepam combined with high-dose, short course metoclopramide (3 mg/kg) infused twice 3 h apart with or without i.v. dexamethasone is reported. Sixty patients receiving a total of 209 cycles of potentially severely emetic cytotoxic chemotherapy were randomised to receive one or other antiemetic regimen. In those receiving platinum-based chemotherapy the addition of dexamethasone was associated with an improvement in freedom from nausea (P less than 0.01) and freedom from vomiting (P less than 0.05). Tamoxifen therapy of metastatic breast cancer.

Inhibition of estrogen action at the target tissue level with the antiestrogen, tamoxifen, has proved highly successful in the treatment of hormone-responsive breast cancer. In randomized clinical trials involving postmenopausal patients, tamoxifen has been found to be as effective as high-dose estrogens, androgens, progestins, and the aromatase inhibitor, aminoglutethimide. Because of its remarkable lack of significant toxicity, tamoxifen is now considered the first endocrine treatment of choice of hormone-responsive postmenopausal breast cancer. Although effective in a significant fraction o

Sequential combination of systemic high-dose ara-C and asparaginase for the treatment of central nervous system leukemia and lymphoma.

Eight patients with overt central nervous system (CNS) leukemia and lymphoma were treated with sequential administration of systemic high-dose cytosine arabinoside (HiDAC) and asparaginase (ASP) with no direct CNS therapy. Complete clearing of the cerebrospinal fluid (CSF) was achieved in six (86%) of seven patients with meningeal disease, generally after the first course of therapy. Two patients presented with evidence of extensive intracerebral disease; both responded with a greater than 50% regression of the tumor infiltrates. Concomitant extraneurologic localizations responded equally well

From Biology to Bedside: Challenges and Strategies in Developing Combination Cancer Therapies in Early-Phase Trials.

Recent advances in next-generation sequencing technology and molecular profiling have significantly expanded our understanding of the tumor mutational landscape, leading to a substantial increase in novel targeted agents. Small molecules have transformed the standard of care for many patients with oncogene-driven cancers. However, single-agent therapies have several limitations, including the development of primary (intrinsic) or secondary (acquired) resistance, which often results in nondurable antitumor responses. Additionally, targeting a single alteration can lead to the upregulation of ad

Preclinical Development of Tuspetinib for the Treatment of Acute Myeloid Leukemia.

ABSTRACT: Tuspetinib (TUS) is a well-tolerated, once daily, oral kinase inhibitor in clinical development for treatment of acute myeloid leukemia (AML). Nonclinical studies show that TUS targets key prosurvival kinases with IC50 values in the low nmol/L range, including SYK, wild-type (WT) and mutant forms of FLT3, mutant but not WT forms of KIT, RSK2, and TAK1–TAB1 kinases, and indirectly suppresses expression of MCL1. Oral TUS markedly extended survival in subcutaneously and orthotopically inoculated xenograft models of FLT3-mutant human AML, was well tolerated, and delivered enhanced activi

A phase 2 study of sacituzumab govitecan with or without atezolizumab in rare genitourinary tumors (SMART) - design and rationale.

Sacituzumab govitecan (SG) is a TROP2-directed antibody-drug conjugate with a payload of SN-38, a topoisomerase inhibitor. SG monotherapy has demonstrated activity in multiple solid tumors, including metastatic breast and bladder cancer. Rare tumors of the genitourinary tract often exhibit aggressive clinical behavior yet lack effective treatment options. This article describes the design and rationale for an open-label, non-randomized, phase 2 trial of SG monotherapy or concomitant SG plus atezolizumab in locally advanced/metastatic bladder/urinary tract adenocarcinoma/urachal adenocarcinoma,

Response to therapy in Richter syndrome: a systematic review with meta-analysis of early clinical trials.

INTRODUCTION AND AIMS: Richter syndrome (RS) represents the clonal evolution of chronic lymphocytic leukemia with histological transformation into a high-grade B cell lymphoma (diffuse large B cell lymphoma - DLBCL) or Hodgkin lymphoma. Considering that RS is an uncommon condition with poor prognosis, few high-quality evidence is available. To overcome this unmet need, this meta-analysis aimed to pool efficacy of early clinical trials in Richter syndrome (DLBCL subtype). METHODS: MEDLINE, Scopus and Web of Science were searched up to May of 2023 to identify clinical trials decoying efficacy.
