Vaccine strategies in the treatment of low-grade non-Hodgkin lymphoma.

Recent years have witnessed the development of a variety of promising immunotherapies for treating patients with B-cell non-Hodgkin's lymphomas. Each B lymphocyte expresses an immunoglobulin molecule that is the product of a unique combination of gene segments. B cell malignancy arises from one original B lymphocyte, and therefore all the members of a given lymphoma tumor population have the same unique immunoglobulin, which can serve as a target for immune therapy. When the idiotype (Id), or unique portion, of each immunoglobulin is used as a vaccine, antibodies and T cells can be induced and

A platform trial design for preventive vaccines against Marburg virus and other emerging infectious disease threats.

BACKGROUND: The threat of a possible Marburg virus disease outbreak in Central and Western Africa is growing. While no Marburg virus vaccines are currently available for use, several candidates are in the pipeline. Building on knowledge and experiences in the designs of vaccine efficacy trials against other pathogens, including SARS-CoV-2, we develop designs of randomized Phase 3 vaccine efficacy trials for Marburg virus vaccines. METHODS: A core protocol approach will be used, allowing multiple vaccine candidates to be tested against controls. The primary objective of the trial will be to ev

Efficacy and tolerability of vilazodone for major depressive disorder: evidence from phase III/IV randomized controlled trials.

Vilazodone is a new molecule approved for major depressive disorder (MDD). This report focuses on the efficacy and tolerability of vilazodone for MDD. MEDLINE, EMBASE, and Cochrane Library were searched. A total of 1,930 patients from four trials were included. A significant improvement in the Montgomery-Asberg Depression Rating Scale (MADRS) total score was seen as early as week 2 (P<0.01) in vilazodone-treated patients. The results showed a higher rate of MADRS response with vilazodone compared with placebo (P<0.001). There were also greater improvements in the Hamilton Rating Scale for Anxi

Achieving consensus for clinical trials: the REiNS International Collaboration.

The neurofibromatoses (NF)--including neurofibromatosis 1 (NF1), neurofibromatosis 2 (NF2), and schwannomatosis--are related tumor-suppressor syndromes characterized by a predisposition to multiple tumor types and other disease manifestations, which often result in functional disability, reduced quality of life, pain, and, in some cases, malignancy. With increasing knowledge of the biology and pathogenesis of NF, clinical trials with targeted agents directed at NF tumors have become available. Most clinical trials for patients with NF have used designs and endpoints similar to oncology trials.

A narrative review of Phase III and IV clinical trials for the pharmacological treatment of Tourette's syndrome in children, adults, and older adults.

Tourette syndrome (TS) is a neurodevelopmental disorder marked by involuntary movements and vocalizations (tics), often accompanied by cognitive and conduct challenges. Despite existing treatments, many remain ineffective, prompting increased focus on Phase III and IV clinical trials for improved pharmacological options. These trials aim to refine symptom management and enhance the quality of life for individuals with TS. This review aims to identify the pharmacological treatments trialed in completed Phase III and IV clinical trials for managing TS in children, adults, and older adults. Phase

Insufficient diet management and monitoring of patients during phase 2 and 3 pharmaceutical clinical trials: A narrative review with a systematic approach.

Clinical trials play a vital role in developing new medications for nutrition-related diseases, such as phenylketonuria (PKU), obesity, and diabetes. Dietary management is often a critical component of these trials, as consistency in diet can significantly influence outcomes and the evaluation of drug efficacy. While the Federal Drug Administration (FDA) provides guidance on managing and monitoring diets in clinical trials, the extent to which these practices are implemented remains unclear. This review examines diet management practices in phase 2 and 3 clinical trials for weight loss, type 2

Immunotherapy with Mycobacterium vaccae in the treatment of tuberculosis.

All the trials of immunotherapy of tuberculosis with killed Mycobacterium vaccae, published or not, that are known to the authors are reviewed here. Following an introduction giving a brief account of some earlier immunotherapies for tuberculosis, the origins of the concept of immunotherapy with M.vaccae are considered. Progress is traced from the early work with irradiation-killed organisms in leprosy to the study in London of modulation of tuberculin skin-test responses, and the first comparative trials in The Gambia and Kuwait. In the last of these studies, dosages and different preparation

Current investigations on clinical pharmacology and therapeutics of Glucose-6-phosphate dehydrogenase deficiency.

Glucose-6-phospate dehydrogenase (G6PD) deficiency is estimated to affect more than 400 million people world-wide. This X-linked genetic deficiency puts stress on red blood cells (RBC), which may be further augmented under certain pathophysiological conditions and drug treatments. These conditions can cause hemolytic anemia and eventually lead to multi-organ failure and mortality. G6PD is involved in the rate-limiting step of the pentose phosphate pathway, which generates reduced nicotinamide adenine dinucleotide phosphate (NADPH). In RBCs, the NADPH/G6PD pathway is the only source for recycli

Transdermal fentanyl: suggested recommendations for clinical use.

Transdermal fentanyl offers the advantage of providing continuous administration of a potent opioid in the absence of needles and expensive drug-infusion pumps for the treatment of cancer pain. When transdermal fentanyl is initiated, it may be necessary to change the dose every 24-48 hr until an appropriate dose is titrated to the needs of the patient. This should be done by providing short-acting opioids as rescue analgesics for breakthrough pain. Well-accepted principles established for chronic opioid use in cancer pain management should apply to the administration of transdermal fentanyl as

Mipomersen sodium: a new option for the treatment of familial hypercholesterolemia.

In collaboration with Genzyme, Isis Pharmaceuticals has developed mipomersen sodium (ISIS-310312), a synthetic second-generation 20-base phosphorothioate antisense oligonucleotide (ASO) that targets messenger RNA encoding apolipoprotein B-100 (Apo B-100). Elevated cholesterol levels, in particular low-density lipoprotein cholesterol (LDL-C) which contains a single apolipoprotein B (ApoB) molecule, have been directly correlated with the incidence of cardiovascular events. Preclinical investigations in transgenic mice have demonstrated that lowering LDL-C or ApoB can reduce aortic plaque formati

Albumin for Adult ICU Patients With Shock: Protocol for the INCEPT-Albumin Platform Trial Domain.

BACKGROUND: Intravenous albumin is used for resuscitation of adult intensive care unit (ICU) patients with shock and substitution in those with hypoalbuminemia, which is common in critically ill patients. There is, however, substantial clinical practice variation as it is uncertain if albumin use improves patient-important outcomes. METHODS: The INCEPT-Albumin domain is an investigator-initiated, open-label domain with an integrated feasibility phase on the international, pragmatic, parallel-group, randomised, embedded, multifactorial, adaptive Intensive Care Platform Trial (INCEPT). Pacritinib: a new agent for the management of myelofibrosis?

INTRODUCTION: Myelofibrosis (MF) is a clonal haematological disease associated with recurrent somatic gene mutations (JAK2V617F, MPL, CALR) and constitutive activation of the Janus kinase (JAK)/Signal Transducer and Activator of Transcription pathway. MF is often characterised by debilitating symptoms and JAK inhibitors (JAKIs) have revolutionised available therapeutic options. Ruxolitinib, a JAK1 and 2 inhibitor, is the only currently approved agent. Several other JAKIs are undergoing evaluation in the clinical trial setting and Pacritinib , a novel JAK2 and FLT3 inhibitor, is at an advanced

Cangrelor AstraZeneca.

AstraZeneca is developing the P2T (P2YADP) purinoceptor antagonist and platelet aggregation inhibitor, cangrelor, for the potential treatment of unstable angina and as an ultrafast-acting intravenous antithrombotic agent. It is in phase IIb clinical trials [315723]. NDA and MAA applications are planned for after 2003 [275466], [314472]. It superseded the earlier compound, ARL-67085, which also reached phase II trials [328760]. In ex vivo samples of angina patients' blood, cangrelor inhibits platelet/monocyte conjugate development, which indicate the drug has some degree of disease-modifying ac

Unresectable Intermediate-Size (3-5 cm) Colorectal Liver Metastases: Stereotactic Ablative Body Radiotherapy Versus Microwave Ablation (COLLISION-XL): Protocol of a Phase II/III Multicentre Randomized Controlled Trial.

BACKGROUND: Although microwave ablation (MWA) has a low complication rate and good efficacy for small-size (≤ 3 cm) colorectal liver metastases (CRLM), local control decreases with increasing size. Stereotactic body radiotherapy (SBRT) is gaining interest as a potential means to treat intermediate-size CRLM and might be less susceptible to increasing volume. The objective of this study is to compare the efficacy of MWA to SBRT in patients with unresectable, intermediate-size (3-5 cm) CRLM. METHODS: In this two-arm, multicentre phase II/ III randomized controlled trial, 68 patients with 1-3 un

Antimicrobial Stewardship for Ventilator Associated Pneumonia in Intensive Care (the ASPIC trial): study protocol for a randomised controlled trial.

INTRODUCTION: Ventilator-associated pneumonia (VAP) remains the leading cause of infections treated in the intensive care units (ICU). In a personalised care approach, we hypothesise that the duration of treatment of VAP can be reduced in function of the response to treatment. METHODS AND ANALYSIS: The Antimicrobial Stewardship for Ventilator-Associated Pneumonia in Intensive Care (ASPIC) trial is a pragmatic national multicentre, phase III, non-inferiority, comparative randomised (1:1) single-blinded clinical trial. Five hundred and ninety adult patients hospitalised in 24 French ICU with a
