Damage control orthopedics applied in an 8-year-old child with life-threatening multiple injuries: A CARE-compliant case report.

RATIONALE: Damage control is a staged surgical approach to manage polytraumatized patients. The damage control approach comprises three steps. First, bleeding is controlled and fractures are stabilized temporarily; second, vital parameters are stabilized and the child is rewarmed in the intensive care unit; and third, the child is reoperated for definitive repair of injuries. We aimed to describe the feasibility of the damage control orthopedic approach in a child. PATIENT CONCERNS: An 8-year-old girl fell from the balcony of the 5th floor onto concrete pavement and was admitted to our accide

Hip arthroscope-assisted percutaneous reduction and fixation of displaced subcapital femoral neck fracture.

Reduction quality is associated with fracture prognosis. Displaced subcapital femoral neck fracture has highest possibility of avascular necrosis of femoral head and non-union among the femoral neck fractures, which commonly necessitate revisions or hip replacement. This study introduces for the first time of using hip arthroscope to directly visualize and assist the reduction of displaced subcapital femoral neck fracture when closed reduction is unsatisfactory. Due to the minimally invasive advantage of arthroscopic assistance, radiation exposure or intraoperative bleeding can be reduced, ope

Bosworth fracture-dislocation of the ankle: a case report.

Bosworth fracture-dislocation of the ankle is a rare injury in which the proximal fibular fragment is entrapped behind the tibia. Closed reduction is extremely difficult to achieve. Early open reduction and internal fixation enables a better outcome by minimising soft-tissue damage. We report on a 36-year-old man who underwent open reduction and internal fixation for a Bosworth fracture-dislocation of the ankle complicated by severe soft-tissue swelling and an impending risk of skin necrosis after failed closed reduction.

A rare Bosworth Variant Isolated Distal Fibula Dislocation with a Successful Closed Reduction: A Case Report and Technique.

INTRODUCTION: The Bosworth fracture-dislocation is a rare injury originally described as an irreducible dislocation of a distal fibula fracture where the proximal fibular fragment becomes incarcerated behind the distal tibia. There have since been a few cases described of an intact distal fibula with dislocation and similar difficulties in closed reduction attempts, typically seen with associated injuries to the ankle or proximal fibula. Regardless of the specific fracture or dislocation pattern, closed reduction attempts have consistently failed in the significant majority of these cases, whi

Distal Tibia Salter-Harris Fractures: To Fix or Not to Fix.

It is important to review treatment considerations and provide a framework to guide decision making for nonsurgical and surgical treatment of distal tibia physeal fractures in pediatric patients. In most closed injuries, attempted closed reduction and immobilization is the initial treatment for stabilization of these fractures. Postreduction fracture alignment evaluation with a focus on residual fracture displacement at the articular surface and the physis combined with residual angulation will guide further treatment after closed reduction. Discussion of commonly occurring distal tibia physea

Colorectal cancer in Greenland.

INTRODUCTION: Distal radius fractures (DRFs) are common, with over 14,000 annual cases in Denmark. Most cases are treated non-surgically and may require closed reduction prior to casting. National guidelines for anaesthesia and reduction technique for closed reduction of DRFs are lacking, impacting clinical practices and potentially optimal care. This survey aimed to describe Danish clinical practice for initial treatment and closed reduction of DRFs. METHODS: Between January and April 2025, the survey was distributed online to the 23 physicians responsible for all 44 emergency departments an

Percutaneous Screws and External Fixation in Frontal Sinus Anterior Wall Fractures.

Fractures of the frontal sinus are frequently encountered injuries of maxillofacial fractures. The most common causes are traffic accidents, followed by sports-related injuries. There is still no consensus regarding the optimal management of frontal sinus fractures. The authors report a patient with depressed anterior wall fracture of the frontal sinus and superior orbital rim fracture. Reduction was performed by traction from 2 screws applied percutaneously to the depressed fragments and external fixation with an aluminum nasal splint. This technique is both minimally invasive and permits eas

Reduction of nasal fracture using an airway tube as a pivot.

When repairing nasal fracture by closed reduction, an airway tube is useful for the pivot of the forceps. By placing the tube on the upper white lip and using it as a pivot, the fracture is reduced with Walsham forceps or Langenbeck elevator. This method is useful for all nasal fractures that are too firm to reduce by conventional closed reduction.

Variant Salter-Harris Type III Distal Ulna "T" Fracture in the Setting of Galeazzi Equivalent Wrist Injury: A Case Report.

CASE: An 11-year-old Caucasian boy presented to the emergency department with a displaced, closed, Galeazzi equivalent (GE) left wrist fracture sustained after a fall. Closed reduction was deemed unsatisfactory because of persistent displacement of the distal ulna epiphysis. An open reduction of the distal ulna and percutaneous fracture pinning was performed. At 1 year, the patient reported return to his preinjury baseline. No evidence of subsequent pathologic growth was detected on follow-up imaging. CONCLUSION: Open anatomic reduction of the distal ulna epiphysis and percutaneous fracture p

Bilateral trans-scaphoid perilunate fracture-dislocation: A case report.

BACKGROUND: Bilateral trans-scaphoid perilunate fracture dislocation is an infrequent injury that usually occurs following either high-energy trauma or a fall on an outstretched hand. It can be easily overlooked in patients with polytrauma and may lead to an unfavorable prognosis with pain and arthritis. CASE PRESENTATION: We present the case of a 26-year-old man with a history of a fall from a two-story building, diagnosed with bilateral trans-scaphoid perilunate fracture-dislocation, bilateral calcaneal fractures, and a burst fracture of the third lumbar vertebra (L3). In the emergency depa

Clinical outcomes of closed, displaced phalangeal neck fractures in children with different types of kirschner wire fixation: A retrospective observational study.

OBJECTIVES: Inappropriate treatment of Closed displaced phalangeal neck fractures (CDPNF) in children usually leads to poor outcomes. This study was to evaluate the clinical outcomes of closed/open reduction, age, and different types of fracture and Kirschner wire (K-wire) fixation in the treatment of CDPNF. MATERIALS AND METHODS: Participants: Sixty patients (male, 46 and female,14; right-handed, 35 and left-handed, 25; mean age, 7.9-years-old [range, 1.0-14.5 years]) who had CDPNF were included. Preoperative x-rays showed that the fractures were displaced and exhibited obvious deformities. I

Percutaneous versus open cannulated screws fixation for displaced isolated medial malleolar fractures in adults: a randomized controlled clinical trial.

INTRODUCTION: Isolated medial malleolar (MM) fractures are infrequent injuries. Studies focused on their treatment are scarce. The aim of this study was to compare functional and radiographic outcomes of two surgical techniques for treatment of displaced isolated MM fractures in adults: closed reduction and percutaneous fixation (CRPF), and open reduction and internal fixation (ORIF) by using same implant; two partially-threaded cannulated cancellous screws. MATERIALS AND METHODS: A prospective randomized controlled clinical trial (RCT) was conducted on 50 patients with isolated displaced MM

A Displaced and Entrapped Anterior Malleolus (Tillaux Fragment) Causing an Irreducible Syndesmosis and Ankle Fracture-Dislocation in an Adolescent: A Case Report.

INTRODUCTION: Tillaux fractures are Salter-Harris type III injuries of the anterolateral distal tibial epiphysis seen in adolescents. Entrapment of the Tillaux fragment within the syndesmosis is rare and may lead to an irreducible ankle fracture-dislocation. CASE REPORT: A 13-year-old girl sustained an ankle fracture-dislocation following an external rotation injury. Imaging revealed a displaced Tillaux fracture with an associated posterior malleolus fracture. Closed reduction was unsuccessful. Open reduction revealed incarceration of the Tillaux fragment within the syndesmosis, preventing re

Olfactory Dysfunction in Nasal Bone Fracture.

BACKGROUND: All nasal bone fractures have the potential for worsening of olfactory function. However, few studies have studied the olfactory outcomes following reduction of nasal bone fractures. This study evaluates posttraumatic olfactory dysfunction in patients with nasal bone fracture before and after closed reduction. METHODS: A prospective study was conducted for all patients presenting with nasal bone fracture (n=97). Each patient consenting to the study underwent the Korean version of Sniffin' Sticks test (KVSS II) before operation and at 6 month after closed reduction. The nasal fract

Irreducible fracture-dislocation of the knee.

Dislocation and fracture-dislocation of the knee are serious injuries, often related to high-energy trauma. Irreducibility with closed techniques is an extremely rare and challenging problem that has been described in posterolateral, posterior and lateral knee dislocations. Irreducibility in fracture-dislocations around the knee has only been described twice in the literature and never in association with a tibial plateau fracture. We report a unique case of an irreducible tibial plateau fracture-dislocation in which closed reduction was prevented by incarceration of the medial meniscus within

The treatment of supracondylar fractures in children with an absent radial pulse.

The purpose of this study was to determine the outcome of patients with a supracondylar fracture and an absent radial pulse. All children with displaced extension-type supracondylar fractures from 1984-1992 were eligible. Of 326 children with supracondylar fractures, 22 had an absent radial pulse on admission. Fifteen of the 22 children had a well-perfused hand after closed reduction and K-wire fixation. Five had no pulse but a well-perfused hand after reduction and immobilization of the elbow in slight flexion; none had any problem at final review. Seven patients who had a cold white hand aft

Management of Atypical M-Shaped Zygomatic Arch Fractures.

BACKGROUND: M-shaped zygomatic arch fractures can usually be treated effectively through closed reduction. It consists of 2 fracture segments: the anterior zygomatic segment and the posterior temporal bone segment. In clinical practice, atypical M-shaped fractures are often encountered, in which the anterior and posterior fracture segments are discontinuous and separated. Closed reduction usually cannot achieve the desired anatomic reduction effect for this type of fracture. METHODS: The preoperative design showed that the anatomic reduction of the posterior zygomatic arch fracture segment wa

Repurposing Spinal Distractor to Reduce Pediatric Wrist Fractures.

Fracture geometry, particularly a jagged bone spike, can present a physical barrier in closed reduction of pediatric distal radius-ulna fractures. When closed reduction of the fracture is not possible, accepting an incomplete reduction and hoping for remodeling, or open reduction, which poses a greater risk for infection and potential physeal injury, are alternative treatment options. The objective of this study was to describe a technique, coined as Percutaneous Skeletal Traction Aided Reduction (P_STAR), for reducing these fractures, thereby eliminating the acceptance of an incomplete reduct

Predictors of failed closed reduction in subaxial cervical dislocations with interlocked facets: a retrospective cohort study.

STUDY DESIGN: Retrospective cohort study at a tertiary trauma center. PURPOSE: To identify morphological predictors of failed closed reduction in subaxial cervical dislocation with interlocked facets, focusing on a dichotomized compression-extension (CE) versus non-CE classification, and to assess the association of vertebral artery injury (VAI) with reducibility. OVERVIEW OF LITERATURE: Closed cranial traction is rapid and minimally invasive, yet certain morphologies resist reduction. Prior studies proposed various predictors, but few directly compared CE with non-CE injuries or evaluated V
