The anatomical basis for incisions around the foot and ankle in clubfoot surgery.

Skin problems are the cause of a high incidence of recurrent clubfoot problems. With respect to the demonstrated vascular territories (angiosomes) of the ankle and foot, the incisions which seem to be safest are vertical as opposed to transverse, and between angiosomes. The safest of all are the posteromedial incision with an additional vertical lateral incision if necessary, and the posterolateral with an additional medial incision.

Occult subluxation of the subtalar joint in clubfoot (using computerized tomography).

Routine radiographic analysis does not visualize the posterior aspect of the hindfoot (posterior facet of the talocalcaneal joint). To help evaluate patients with persistent hindfoot abnormalities, computerized tomographic coronal sections in 22 clubfeet were studied. Seventeen feet (77.3%) had a lateral subluxation of the calcaneus. Of 14 feet with acceptable radiographic analysis, 12 (85.7%) had a lateral subluxation of the calcaneus. This study is evidence that a lateral subluxation of the calcaneus can exist, through a horizontal rotation, in the presence of acceptable radiographic analysi

[Congenital convex clubfoot: a diagnostic and therapeutic study of 71 cases].

We have studied 71 feet with congenital vertical talus in 51 patients. The condition is characterised by two fixed deformities, equinus of the hindfoot with a vertical talus and dislocation of the mid tarsal joint due to malposition of the navicular with respect to the head of the talus. The forefoot is usually everted, but lay in inversion in 25% of our cases. The deformity must be recognised within a few weeks of birth, and the diagnosis is facilitated by radiographs taken in extreme dorsiflexion and plantarflexion. The radiographic appearances allow classification of the deformity into thre

Management of the resistant myelodysplastic or arthrogrypotic clubfoot with the Verebelyi-Ogston procedure.

The Verebelyi-Ogston (V-O) procedure, consisting of subchrondral excision of the talus and cuboid, was used for the treatment of 13 resistant clubfeet secondary to myelomeningocele or arthrogryposis. Nine feet were initially satisfactory, but the condition recurred in both feet of one patient after bracing was discontinued 3 years postoperatively. Three feet were graded as satisfactory after a second V-O procedure, and one after a third. The procedure must be monitored by intraoperative fluoroscopy or radiography, and followed by orthotic support. Despite the theoretical long-term disadvantage

A perspective focus on the indicated surgical treatment of resistant clubfoot in the infant.

Ligamentous contracture is the chief factor in maintaining clubfoot deformity. Precise plans of segmental ligamentous release are called "selective segmental syndesmotomies." Critical segmental assessment by roentgenograms is required to choose the proper syndesmotomy. Proper early correction of resistant subtalar and midtarsal deformity by selective segmental syndesmotomy can solve much of the persistent trouble confronting the surgeon.

Tibialis Anterior Tendon Transfer in an Arthrogrypotic Clubfoot: A Case Report.

CASE: A 30-month-old boy with arthrogryposis multiplex congenita presented with severe, recurrent teratological clubfoot refractory to serial casting, Achilles tenotomy, and plantar fascia release. Because of repeated cast slippage and patient distress, comprehensive posterior, plantar, and medial releases were performed, with cast exchange 2 weeks thereafter. A robust, taut tibialis anterior prompted addition of tibialis anterior tendon transfer to the lateral cuneiform. Postoperatively, plantigrade alignment was achieved both clinically and radiographically and maintained at the 2-year follo…

Pearls to Address Challenges in Initial Casting Treatment for Idiopathic Clubfoot.

UNLABELLED: Ponseti casting has established itself as the gold standard for the treatment of clubfoot over the past 30 years due to its outstanding short- and long-term outcomes. This method requires a nuanced technique throughout each phase of correction, involving gentle manipulation of the appropriate anatomic regions of the foot. Certain "pearls" should be considered to optimize this process, including minimizing infantile distress, deliberately choosing casting and padding material, and planning the casting timeline. Clubfoot providers should be aware of the "pitfalls" associated with ina…

[Physical self-concept and health perception in treated for congenital clubfoot].

BACKGROUND: the clubfoot is one of the most common pathologies in pediatric orthopedics. There are few studies of physical self-concept and perception of health in these patients. MATERIAL AND METHODS: observational, descriptive, and cross-sectional study. Population aged 16 to 18 years with the diagnosis of clubfoot. Being a sample of 55 patients, who underwent the SF-36, Laaveg-Ponseti Health Questionnaire, and the Physical Self-Concept Questionnaire. Evaluating physical abilities, physical condition, physical attractiveness. Presence of pain, general health, total function, emotional role,…

The global birth prevalence of clubfoot: a systematic review and meta-analysis.

BACKGROUND: Estimates of the birth prevalence of clubfoot in low and middle income settings range from 0.5 to 2 per 1000 births. However, there is currently no estimate of global birth prevalence of clubfoot. METHODS: We conducted a systematic review of studies reporting the birth prevalence of clubfoot across all countries and regions worldwide in the last 10 years. Africa Wide Information, EMBASE, CINAHL, Global Health, LILACS and Medline databases were searched for relevant studies from January 1st 2012 to February 9th 2023. Pooled prevalence estimates were calculated using the inverse var…

Initiating Ponseti management in preterm infants with clubfoot at term age.

PURPOSE: Currently, the optimal time to initiate treatment among preterm infants with clubfoot is unknown. The aim of this study was to describe treatment outcomes up to 1 year post-correction following Ponseti management in infants who were born preterm but treated at term age. METHODS: A retrospective chart audit was conducted at a major pediatric hospital on preterm infants with clubfoot who commenced Ponseti management at term age (≥37 weeks of gestation). Data are expressed as mean values (±standard deviation) or 95% confidence intervals (95% CIs). RESULTS: Twenty-six participants (40 f…

Kinematic differences in the presentation of recurrent congenital talipes equinovarus (clubfoot).

BACKGROUND: The tibialis anterior tendon transfer (TATT) is the suggested surgical intervention in the Ponseti method for treatment of dynamic recurrent congenital talipes equinovarus (clubfoot) presenting as hindfoot varus and forefoot supination during the swing phase of gait. The indication for surgery, however, is typically based on visual assessment, which does not sufficiently examine the variability of foot motion in this cohort. RESEARCH QUESTION: The aim of this research was to determine whether subgroups, based on foot model kinematics, existed within a clubfoot cohort being conside…

The Impact of the Digital Age and Social Media on Connecting the Clubfoot Community.

Background Internet chat rooms played an important role in the late 1990s promoting the Ponseti method as the preferred initial treatment for congenital clubfoot. The social media boom has created multiple new methods for caregivers to seek support from a global community using a variety of platforms that are now easily available. This study assesses the reach of information shared across social media platforms about congenital clubfoot and analyzes topics most commonly discussed among members in these groups. Methodology Posts and pages across Facebook, Instagram, Twitter, and TikTok were eva…

YouTube as an information source for clubfoot: a quality analysis of video content.

Idiopathic clubfoot is the most common congenital anomaly of the lower extremity. YouTube has emerged as an important source of health-related information for patients and families. Parents seek information about clubfoot on YouTube. However, the quality of these videos remains unknown. Therefore, we decided to evaluate the quality of YouTube videos about clubfoot. We searched YouTube for clubfoot videos using appropriate keywords. English language videos with more than 10 000 views were included. Three fellowship trained pediatric orthopedic surgeons independently assessed the videos and clas…

Increased Microvessel and Arteriole Density in the Contracted Side of the Relapsed Clubfoot.

BACKGROUND: Clubfoot deformity (pes equinovarus) is one of the most common birth defects, and its etiology is still unknown. Initial clubfoot treatment is based on the Ponseti method throughout most of the world. Despite the effectiveness of this therapy, clubfoot may relapse. Recent studies confirm the theory of active fibrotic remodeling processes in the extracellular matrix of the affected tissue. The aim of this study was to clarify whether relapses in clubfoot therapy are associated with altered angiogenesis and to suggest possible regulatory pathways of this pathologic process. METHODS:…
