Identifying the impossible: piecemeal cold snare resection perforation.

BACKGROUND AND AIM: Piecemeal cold snare resection (CSR) is an increasingly adopted technique for large nonpedunculated colorectal polyps because of its favorable safety profile. Although adverse events are rare, perforation after CSR has been reported infrequently. We present a video case of intraprocedural perforation during piecemeal CSR. METHODS: A 63-year-old woman with quiescent colonic Crohn disease underwent dysplasia surveillance, revealing multiple flat polyps, including 2 adjacent large 0-IIA transverse colon polyps. Piecemeal CSR was performed using chromoinjectate and a 10-mm col

Lacosamide, a new anti-epileptic, alleviates neuropathic pain-like behaviors in rat models of spinal cord or trigeminal nerve injury.

The effect of systemic administration of lacosamide, a newly developed anti-epileptic, on neuropathic pain-like behaviors was examined in rats after ischemic injury to the infraorbital nerve or spinal cord using a photochemical method. In rats with infraorbital nerve injury, lacosamide reduced mechanical hypersensitivity and the effect was markedly stronger in female than in male rats. In spinal cord injured female rats 10-20 mg/kg lacosamide dose-dependently alleviated the mechanical and cold allodynia-like behaviors without causing motor impairments or marked sedation. Administration of laco

Fn14 Participates in Neuropathic Pain Through NF-κB Pathway in Primary Sensory Neurons.

Fibroblast growth factor-inducible-14 (Fn14), a receptor for tumor necrosis-like weak inducer of apoptosis, is expressed in the neurons of dorsal root ganglion (DRG). Its mRNA is increased in the injured DRG following peripheral nerve injury. Whether this increase contributes to neuropathic pain is unknown. We reported here that peripheral nerve injury caused by spinal nerve ligation (SNL) increased the expression of Fn14 at both protein and mRNA levels in the injured DRG. Blocking this increase attenuated the development of SNL-induced mechanical, thermal, and cold pain hypersensitivities. Co

Study on the Role of the AMPK/PGC-1α Pathway in Cold-Induced Vascular Endothelial Cell Apoptosis and Uterine Damage.

Cold exposure may influence reproductive health through vascular changes, yet its mechanisms remain underexplored. This study aimed to investigate the impact of cold exposure on uterine blood vessels and the expression of the AMPK/PGC-1α gene and protein in adult female SD rats. A primary dysmenorrhea model was established in female Sprague Dawley rats and subjected to continuous cold exposure. Changes in body weight, ear temperature, and estrous cycle were observed. Superoxide dismutase (SOD) activity and adenosine triphosphate (ATP) levels were measured to assess oxidative stress. Uterine ti

Protection of sinusoidal endothelial cells against storage/reperfusion injury by prostaglandin E2 derived from Kupffer cells.

BACKGROUND: In clinical liver transplants, grafts are frequently exposed to endotoxin (lipopolysaccharide, LPS) before harvest and may be predisposed to dysfunction. Because graft failure is linked to sinusoidal endothelial cell injury after storage/reperfusion, we investigated the effect of donor exposure to LPS on graft survival in relation to sinusoidal endothelial cell injury after storage/reperfusion in rats. METHODS: Rats were injected with 0.5 mg/kg LPS. In some rats, 20 mg/kg GdCl3 or 5 mg/kg indomethacin was injected before LPS to ablate Kupffer cells and inhibit prostaglandin (PG) s

Attenuation of neuropathic pain by sodium butyrate in an experimental model of chronic constriction injury in rats.

BACKGROUND/PURPOSE: The present study was designed to investigate the potential of sodium butyrate, a histone deacetylase (HDAC) inhibitor, in chronic constriction injury (CCI)-induced neuropathic pain in rats. METHODS: Neuropathic pain was induced by placing four loose ligatures around the sciatic nerve. Acetone drop, Von frey hair, pin prick and hot plate tests were performed to assess cold allodynia, mechanical allodynia, and mechanical and heat hyperalgesia, respectively. The level of tumor necrosis factor (TNF)-α was measured in the sciatic nerve as an inflammatory marker. RESULTS: CCI

A free radical scavenger, edaravone, prevents ischemia-reperfusion injury in liver grafts from non-heart-beating donors.

BACKGROUND: Due to the increase in liver transplantation, the donor shortage has become a serious problem, requiring marginal, non-heart-beating donors (NHBDs). The aims of this study were to evaluate the cytoprotective effect of edaravone, a free radical scavenger, on warm ischemia-reperfusion (I/R) injury of liver grafts from NHBDs. METHODS: Rat livers were harvested from heart-beating donors (HB group) or from NHBDs undergoing cardiac arrest for 30 minutes led by thoracotomy (NHB group), and reperfused for 60 minutes with Krebs-Henseleit bicarbonate buffer after cold preservation for 6 hou

Steroid and hyperosmotic diuretic effects on the early phase of experimental vasogenic oedema.

The effects of either dexamethasone or mannitol on the early phase of vasogenic oedema were studied by evaluating proton relaxation behaviour with in vitro NMR spectroscopy. Vasogenic cerebral oedema was induced by cold injury in cats. Significant (p less than 0.05) decreases in both longitudinal relaxation rate (R1 = 1/T1) and transverse relaxation rate (R2 = 1/T2) and increases in water content were observed in the white matter beneath the cold lesions four hours after injury. In the dexamethasone treated group, the decrease in R2 in the oedematous white matter was significantly (p less than…

In vivo adenovirus-mediated gene transfer into normal and cystic rat kidneys.

Gene transfer into the mammalian kidney has proved difficult because of the structural complexity of the organ and its low mitotic index. This article describes the use of intra-arterially injected adenovirus to study gene transfer into the rat kidney in vivo. By pre-chilling the kidney, and incubating the virus with the kidney in the cold for extended periods of time, we were able to successfully transfer a beta-galactosidase (beta-gal) reporter gene into the vasculature without ischemic injury to the kidney. Transfer occurred largely in the cortex when cold was used alone, whereas with the u

Vocal cord palsy as a complication of adult cardiac surgery: surgical correlations and analysis.

Vocal cord palsy after adult cardiac surgery is often attributed to non-surgical mechanisms as tracheal intubation and central venous catheterisation. It may also be caused by injury of the recurrent laryngeal nerves by surgical dissection. We hereby present a review of relevant clinical reports. The cumulative incidence was 1.1% (33 in 2980). Main reported surgical mechanisms of injury were harvesting of internal thoracic artery and topical cold cardioprotection. Bilateral nerve palsy has been lethal on at least one occasion. Where vocal cord injury followed harvesting of the left internal th

Crystalloid cardioplegia versus cold blood cardioplegia in aortic arch surgery: A noninferiority randomized trial.

BACKGROUND: The optimal cardioplegic solution to use during aortic surgery remains unclear. While cold blood cardioplegia (BCP) has metabolic advantages, crystalloid cardioplegia (CCP) offers practical benefits. This trial investigated whether cold CCP is noninferior to cold BCP in preserving postoperative left ventricular ejection fraction (LVEF). METHODS: In this single-center, patient- and assessor-blinded, parallel-group noninferiority randomized trial, 52 adult patients undergoing elective aortic arch replacement were randomized 1:1 to receive cold BCP or CCP. The primary endpoint was LV

The impact of Escherichia coli O157:H7, Salmonella spp., and Listeria monocytogenes preparation methods on high-pressure processing efficacy in apple and orange juices.

The use of high-pressure processing (HPP) to inactivate foodborne pathogens within fruit- and vegetable-based juices is well understood; however, less is known regarding HPP-induced sublethal injury of bacterial cells and potential detection of stress-adapted bacterial cells during post-HPP cold storage. FDA Juice HACCP requires HPP-treated juice manufacturers to demonstrate a 5-log reduction of the pertinent microorganism and maintain the 5-log reduction throughout the product's shelf-life. Activated protein C in ischemia-reperfusion injury after experimental lung transplantation.

BACKGROUND: Ischemia-reperfusion injury remains the major cause of early morbidity and mortality after lung transplantation. Activated protein C (APC) has been demonstrated to attenuate various acute inflammation-related injuries in the lung and other organs. METHODS: The effect of exogenous APC in lung transplantation was examined using a rat orthotopic lung transplantation model of ischemia-reperfusion injury with 24 hours of cold ischemia. APC was administered to the donor airway before cold pulmonary artery flush, or intravenously to the recipient before reperfusion. RESULTS: The levels

Cold Weather Conditions and Risk of Hypothermia Among People Experiencing Homelessness: Implications for Prevention Strategies.

Hypothermia is a preventable condition that disproportionately affects individuals who experience homelessness, yet limited data exist to inform the response to cold weather. To fill this gap, we examined the association between meteorological conditions and the risk of hypothermia among homeless individuals. Hypothermic events were identified from emergency department charts and coroner's records between 2004 and 2015 in Toronto, Canada. A time-stratified case-crossover design with conditional logistic regression was used to assess the relationship between the meteorological conditions (minim

Cold Atmospheric Pressure Plasma Solutions for Sustainable Food Packaging.

Increasing the number of resistant bacteria resistant to treatment is one of the leading causes of death worldwide. These bacteria are created in wounds and injuries and can be transferred through hospital equipment. Various attempts have been made to treat these bacteria in recent years, such as using different drugs and new sterilization methods. However, some bacteria resist drugs, and other traditional methods cannot destroy them. In the meantime, various studies have shown that cold atmospheric plasma can kill these bacteria through different mechanisms, making cold plasma a promising too

Long-term outcomes of homodigital neurovascular island flap reconstruction of fingertip injuries in children.

We evaluated the long-term functional and cosmetic results of homodigital neurovascular island flap (NIF) used to reconstruct extensive pulp defects with bone exposure in children. Twenty-three children (mean age 4.8 years, range 1-10 years) with fingertip injuries were reconstructed with a pedicled homodigital NIF and evaluated in terms of sensation quality, cold intolerance, scar formation, nail deformity, range of motion and overall finger length at a mean follow up of 7.8 years (range 2-13). Eleven patients reported cold sensitivity in the operated fingertip, and 15 presented with hook nai

Cardiovascular reflexes during vibration stress.

Vibration disease due to hand-held vibratory tools has various symptoms and signs which can be characterized by the severity. They include disorders of the central and autonomic nervous systems, as well as peripheral system disorders. The mechanism of Raynaud's phenomenon in vibration disease is proposed to be: Vibration and cold affect the local vessels and nerves directly, leading to enhanced release of chemical vasoconstrictors. Vibration, noise, cold, ergonomic and biodynamic conditions, and emotional stress during work result in disorders of the central and autonomic nervous systems. In t

Digital sensibility following replantation.

The amputated digit serves as an excellent model for the examination of digital nerve repair. When amputation is complete, there is no question of cross-over or anomalous innervation. The sensibility of 35 replanted digits in 29 patients was evaluated. Sensory return was related most closely to restored digital vascularity, as measured by Allen test, pulse volume flow, and Doppler. Level of amputation, mechanism of injury, and age of patient also affected ultimate sensation. All patients experienced cold intolerance and some were considerably disabled because of it. The severity of cold intole

[Results of primary repair of injuries to the median and ulnar nerves at the wrist].

INTRODUCTION: Wounds on the palmar side of the wrist affecting the median or ulnar nerves are responsible for motor and sensory sequelae, severe pain and cold intolerance. MATERIALS AND METHODS: Thirty-nine patients with 40 nerve sections were retrospectively reviewed with a mean follow up of 23 months. The median nerve alone was affected 20 times, the ulnar nerve seven times and both nerves simultaneously 13 times. In 75% of the cases, there was an associated vascular injury (radial artery and/or ulnar artery). The average number of tendons cut was 4.25. RESULTS: After repair of the median
