The communication skills in a 4-year-old girl with Rett syndrome (RS) prior to, during and after the characteristic regression period are presented. The case study serves to provide a detailed description of the typical communication skills seen in this population, even though this child must be considered an atypical case of RS. The case study emphasizes the need for the assessment and treatment of communication skills in cases with RS to focus on the intentionality and functionality of the child's communication.

The effects of drugs related to cyclic AMP and a tumor promoter, phorbol ester, on intercellular communications via gap junctions were investigated by the Lucifer Yellow-transfer method in cultured rabbit gastric epithelial cells. The communication skills, oral-motor function and respiration patterns of 20 girls with Rett syndrome were studied. Loss of communication has been previously recognized as a sign of the onset of Rett syndrome, but oral-motor dysfunction has not been reported. All the girls showed a regression in speech and language function by the onset of stage II Rett syndrome. Changes in oral-motor function and respiration correlated with the progression of the disease from stages II to IV.

Four cases of congenital left ventricular-right atrial communication were presented. Correct diagnoses were made in two cases before operation by left ventriculography. The other two cases were misdiagnosed as ASD or VSD by right heart catheterization and ventriculography. After surgical repair under cardiopulmonary bypass, they were all cured and discharged. During the follow-up period, one patient with residual leak was reoperated upon and died on the sixteenth postoperative day from arrhythmia and heart failure. The other three patients secured complete restitution.

Augmentative and alternative communication (AAC) aids were used in three young, intellectually normal patients with dystonia musculorum deformans (DMD) who had severe speech and writing impediments. These aids included speech therapy, communication boards, and voice synthesizers for verbal communication and typewriters, memowriters, and computer software and printers for written communication. To assess the contribution of adrenal-derived androgens in women with hirsutism, adrenal scintigrams under dexamethasone suppression (DS) were performed on 35 women with increasing facial or body hair and irregular or absent menses. Based upon the DS regimen chosen (8 mg/d for 2 days or 4 mg/d for 7 days before the injection of 6 beta-[131I]iodomethylnorcholesterol), three imaging patterns were identified. Clofibrate-related compounds were synthesized from substituted phenoxyacetic acids by esterification with salicylic acid derivatives, by amidation of thiazolidines or by reaction with cinnarizine. Lipid lowering effect was investigated in normolipemic and hyperlipemic rats. The long-term hyperlipemic model used permits differentiation of hypolipemic efficacy, i.e., from clofibrate or compounds of similar activity.
