Breaking new ground in family planning communication.

The Urban Reproductive Health Initiative has shown impact on contraceptive use from its communication components even within a few years, as described in 2 GHSP articles. One specifically addressed “ideation” about family planning in detail and was able to show both changes in ideation due to program exposure and correlated changes in contraceptive use. The other used a sophisticated analytical technique that indicated the observed changes in contraceptive use resulted from exposure to the communication efforts, and not just because people more prone to adopt family planning were also more lik

Who is Planning for Environmental Justice, And How.

PROBLEM RESEARCH STRATEGY AND FINDINGS: Environmental justice (EJ) seeks to correct legacies of disproportionately burdening low-income and Black, Indigenous, and people of color (BIPOC) communities with environmental hazards that contribute to health inequalities. Federal and state policies increasingly require plans to assess and incorporate EJ principles. The current lack of accessible data and plan evaluation on EJ has been a barrier to policy setting and benchmarking. We created a framework for analyzing content across a large corpus of plans by using quantitative text analysis on 461 Cal

A community approach to addressing excess breast and cervical cancer mortality among women of African descent in Boston.

In 2000, the REACH Boston 2010 Breast and Cervical Cancer Coalition conducted a community needs assessment and found several factors that may have contributed to disproportionately high breast and cervical cancer mortality among black women: (a) Focus group participants reported that many women in their communities had limited awareness about risk factors for cancer as well as about screening. (b) Black women experienced barriers to care related to the cultural competence of providers and of institutions. (c) Black women were not receiving adequate follow-up for abnormal mammograms and Pap sme

Adapting High Impact Practices in Family Planning During the COVID-19 Pandemic: Experiences From Kenya, Nigeria, and Zimbabwe.

BACKGROUND: We describe how High Impact Practices (HIPs) in family planning (FP) were adapted across Kenya, Nigeria, and Zimbabwe to maintain access to services in response to the coronavirus disease (COVID-19) pandemic. METHODS: Using a qualitative data collection tool structured around 3 HIP categories (service delivery, demand creation, and enabling environment), adaptations in FP programs during the pandemic were documented. We describe adaptations made to 3 specific HIPs: mobile outreach, community health workers, and digital health for social and behavior change. PROGRAM EXPERIENCES: I

New adult and adolescent tuberculosis vaccines and Indonesia: policy planning and evidence, November 2024.

This report summarizes the proceedings of a policy and planning meeting convened in Indonesia, by the Ministry of Health and the World Health Organization, on potential new tuberculosis (TB) vaccines intended for adults and adolescents. Given that Indonesia bears the second highest TB burden globally, and that it is participating in the phase III efficacy study for one of the leading vaccine candidates that could be approved for use by 2028, the meeting aimed to discuss strategies and evidence needs that could accelerate vaccine introduction. The meeting, held in November 2024, was attended by

Regional Overview on the Double Burden of Malnutrition and Examples of Program and Policy Responses: Latin America and the Caribbean.

BACKGROUND: Hunger, food insecurity, stunting, anemia, overweight, and noncommunicable diseases (NCDs) may coexist in the same person, household, and community in Latin America and the Caribbean (LAC). The double burden of malnutrition (DBM) is an important cause of disability and premature death, which could be addressed with comprehensive policies such as the Plan of Action for the Prevention of Obesity in Children and Adolescents. This paper summarizes the main policies and actions aimed to prevent undernutrition and obesity. SUMMARY: Several countries are implementing the Plan of Action,

Identifying strategies to reduce the COVID-19 burden in Iran: insights for future preparedness.

The COVID-19 pandemic exposed critical gaps in healthcare systems and underscored the need for effective strategies to mitigate its burden. Understanding lessons learned from local experiences can inform future pandemic preparedness. This study aimed to explore context-specific strategies implemented in Iran to reduce the impact of COVID-19 and provide insights applicable to future health emergencies. A qualitative study was conducted between July 2023 and February 2024 in five major Iranian cities (Tehran, Shiraz, Isfahan, Mashhad, and Kerman). Data were collected through 28 semi-structured i

A population-based study of communication impairment in cerebral palsy.

To explore factors associated with communication impairments in children with cerebral palsy. Data were obtained on children born between 1999 and 2008 from the Quebec Cerebral Palsy Registry (REPACQ). Out of 535 children with cerebral palsy, 297 were identified to have communication impairments (55.5%). Of these, 96 were unable to communicate verbally (32.3%), 195 had some verbal communication (65.7%), and 6 were unspecified (2.0%). These children were significantly more likely to have a more severe motor deficit (Gross Motor Function Classification System levels IV and V and Manual Ability C

Practical insights at a critical time: Lessons from Illinois on implementing family planning state plan amendments.

Family planning Medicaid programs via state plan amendments can extend preventative care coverage to broader populations, generate cost savings for states, and safeguard against changes to Title X and to Medicaid and its financing structure. Contraceptive access initiatives, as subject matter experts and coalition builders, are uniquely positioned to serve as "systems orchestrators", working across "the 4 Ps"-providers, patients, payers, and policymakers-to fulfill the promise of these policies to improve access to critical sexual and reproductive health services. Since 2021, ICAN! has played

Community learning disability teams: the need for objective methods of prioritization and discharge planning.

As part of caseload management for community learning disability teams (CLDTs), it would appear reasonable that services would have a mechanism for prioritizing referrals and discharge planning. However, any formal mechanism in relation to these two aspects apparently is lacking within the literature. This theoretical article attempts to illustrate this evidence-base need, and demonstrate how a prioritization and/or discharge planning system would aid CLDTs. In light of the scarcity of material for the learning disability sector, information has been examined from other health domains such as

Experience of Health System Personnel in the Implementation of Mass Distribution Campaigns for the Control of Lymphatic Filariasis in Rural Guinea in 2022.

This study documents the experiences of health system personnel in the implementation of mass distribution campaigns for the control of lymphatic filariasis in rural Guinea. This was an exploratory qualitative study using data collected from implementing actors of mass distribution campaigns in the Boké health district. The results showed four main facilitators of mass distribution campaign rollout in the Boké health district: (i) support to the district teams in the organization of the campaigns; (ii) involvement of community-based associations in social mobilization; (iii) strong adherence o

The case for more active policy attention to health promotion.

Until recently, when anthrax triggered a concern about preparedness in the public health infrastructure, U.S. health policy and health spending had been dominated by a focus on payment for medical treatment. The fact that many of the conditions driving the need for treatment are preventable ought to draw attention to policy opportunities for promoting health. Following a brief review of the determinants of population health-genetic predispositions, social circumstances, environmental conditions, behavioral patterns, and medical care-this paper explores some of the factors inhibiting policy att

Ethical and scientific consequences of abrupt federal grant terminations in community-engaged environmental health research.

The abrupt termination of federally-funded research studies, traditionally uncommon, carries scientific and ethical consequences - including the loss of scientific results, lost investments in data and research infrastructure, disruptions to training pipelines and career trajectories, and curtailed dissemination of results. In the case of community-engaged research, there may be additional damage, given the substantial involvement and investments of time, effort, and resources made by external partners (community members, clinicians, local agencies, and others). External stakeholders may have

Developing integrated community-based HIV prevention, harm reduction, and sexual and reproductive health services for women who inject drugs.

BACKGROUND: Despite being a priority population for HIV prevention and harm reduction programs, the sexual and reproductive health (SRH) needs of women who inject drugs are being overlooked. Furthermore, models for providing integrated SRH, HIV, and harm reduction services for women who inject drugs are rare. This article reports the development of community-based outreach services that integrated family planning and other SRH interventions with HIV and harm reduction services for this population in coastal Kenya. METHODS: Using mixed-methods implementation research, a qualitative baseline ne

The Excluded Voices from Africa's Sahel: Alternative Meanings of Health in Narratives of Resistance to the Global Polio Eradication Initiative in Northern Nigeria.

Although previous inquiry into resistance to the polio vaccines in northern Nigeria has been launched from several disciplines, inquiry has been limited to the 2003 revolt and has rarely been informed by theory. This study drew on the culture-centered approach to health communication to argue that the exclusion of marginalized communities from decision-making by the Global Polio Eradication Initiative (GPEI) resulted in a vaccine resistance which found expression in health activism that engaged the local news media. To recover the excluded voices, this study examined community members' narrati

Task shifting and community engagement as the keys to increasing access to modern contraception: Findings from mixed methods operational research in coastal Kenya.

RATIONALE: Kilifi County, Kenya, has a modern contraceptive prevalence of 44%, compared to the national prevalence of 61%. In 2018, the Government of Kenya and Population Services Kenya implemented a pilot project in Kilifi to improve contraceptive awareness and access. AIMS AND OBJECTIVES: The Riziki Demonstration Project task shifted contraceptive injectable and implant provision to community health extension workers (CHEWs) and supported community engagement led by community health volunteers (CHVs). This mixed methods evaluation examined the effect of Riziki on contraceptive service provi

The Docent Method: A Grounded Theory Approach for Researching Place and Health.

To understand health, research needs to move outside of controlled research settings into the environments where health activities occur-homes, streets, and neighborhoods. I offer the docent method as a qualitative place-based approach for exploring health in a participant-driven, structured, and flexible way. The docent method is a participant-led, audiotaped, and photographed walking interview through broad "sites of interest" (SOIs). It is rooted in grounded theory and influenced by community-based participatory research and walking interviews. The three stages of the docent method involve:

Health and substance use behavior: the longitudinal Harlem Health Study.

Described here is a 22 year prospective study of adolescents focusing on health problems, substance use and other health behaviors, and treatment needs of urban African Americans. The study panel was drawn from a probability sample of households in Central Harlem (NYC). Accordingly, it is a community representative sample comprised about equally of males and females. Data from the study have been used for planning the local municipal hospital's adolescent service and for informing drug policy and community education activities regarding trajectories in and out of drug use. Given the timing of

Family planning services for adolescents and young adults.

If we are to influence the numerical trends of venereal disease and of unwanted pregnancies in the young, family planning services should be made easily available to them. To encourage the widest possible and most effective use of such services requires that health professionals openly endorse their ready availability. They must foster non-judgmental attitudes, however unorthodox patient life styles may be, and provide the young with opportunities to explore their own sexual behavior. The Youth Clinics of the Department of Community Health Services in Los Angeles are designed to meet both the
