Implementation and Assessment of Facilitation Support for Improving Health Insurance Coverage in Achham.

Health insurance is a critical strategy for achieving universal health coverage, reducing out-of-pocket (OOP) healthcare expenditures, and improving access to healthcare services. Despite the implementation of the National Health Insurance Program (NHIP) in Nepal, enrolment and renewal rates remain low, particularly in districts like Achham. This study aimed to assess the effectiveness of facilitation support, delivered through patient navigators, enrolment assistants, and community health workers (CHWs), in increasing health insurance coverage.  Methods: A mixed-methods approach was employed.…

Community demand for comprehensive primary health care from malaria volunteers in South-East Myanmar: a qualitative study.

BACKGROUND: Malaria volunteers have contributed significantly to malaria control achieving a reduction of annual parasite incidence to pre-elimination levels in several townships across Myanmar. However, the volunteers' role is changing as Myanmar transitions from a malaria control to elimination programme and towards the goal of universal health coverage. The aim of the study is to explore the perspectives of community leaders, members and malaria volunteers in South-East Myanmar on community-delivered models to inform an optimal design that targets malaria elimination in the context of prima

An Evaluation of Practitioner's Experience of Service Users Seeking Community Detoxification from Benzodiazepines.

A recent report in Ireland identified that two-thirds of poisoning deaths involved poly drug use with an average of four different drugs involved. Of these, benzodiazepines were the most common drug group involved. Concern has been expressed regarding high levels of benzodiazepine prescriptions globally. Community-based detoxification programs are required, but detoxification is complex with associated high risks, such as overdose. This study utilized a survey to gather the experiences of a range of drug workers in addiction settings in the southern region of Ireland who are tasked with the ma

Towards occupational health and safety in sex work.

Sex workers remain overwhelmingly excluded from occupational health and safety (OHS) protections due to continued criminalisation and stigmatisation of sex work. In this Series paper, we examine evidence on sex workers' OHS using an adapted OHS framework. Our findings show how criminalisation undermines comprehensive OHS practices across legislative contexts, but despite criminalisation, sex workers adopt innovative, community-driven strategies to safeguard OHS and place a strong emphasis on community mobilisation and mutual aid. Decriminalisation markedly improves working conditions and facil

Compassion Fatigue among Healthcare, Emergency and Community Service Workers: A Systematic Review.

Compassion fatigue (CF) is stress resulting from exposure to a traumatized individual. CF has been described as the convergence of secondary traumatic stress (STS) and cumulative burnout (BO), a state of physical and mental exhaustion caused by a depleted ability to cope with one's everyday environment. Professionals regularly exposed to the traumatic experiences of the people they service, such as healthcare, emergency and community service workers, are particularly susceptible to developing CF. This can impact standards of patient care, relationships with colleagues, or lead to more serious

Inuit experiences of and expertise on the 21st-century tuberculosis epidemic in Nunavik, Quebec: a qualitative community-based participatory study.

BACKGROUND: In Nunavik, the Inuit lands in Quebec, the incidence of tuberculosis has been rising and is currently 1000 times that seen among non-foreign-born Quebec residents. To inform tuberculosis policies aligned with Inuit self-determination, we sought to explore Nunavimmiut (Inuit of Nunavik) experiences with tuberculosis care. METHODS: We undertook a community-based participatory action research project in an Indigenous research methodology (IRM) framework. From 2022 to 2023, Indigenous researchers (First Nations and Inuit) conducted interviews and focus groups with Nunavimmiut about tu

Training for hearing care providers.

The lack of an appropriately trained global hearing-care workforce is recognized as a barrier to developing and implementing services to treat ear and hearing disorders. In this article we examine some of the published literature on the current global workforce for ear and hearing care. We outline the status of both the primary-care workforce, including community health workers, and specialist services, including audiologists, ear, nose and throat specialists, speech and language therapists, and teachers of the deaf. We discuss models of training health workers in ear and hearing care, includi…

Evaluation of mHealth Usage Logs for Health Provider Performance in HIV Care in Zambézia Province, Mozambique.

Community health workers (CHWs) offer essential services for people living with HIV (PLHIV) in low-resource settings. However, understanding and optimizing CHW performance is challenging as CHWs work remotely and over large geographical areas. A mobile health (mHealth) application, mUzima, was introduced in one district in Mozambique to support CHWs during patient visits. Usefulness of mUzima-derived logs to inform CHW performance and work patterns was evaluated. Derived from 113,893 cleaned log records, the 29 CHWs who used mUzima for 8,896 distinct encounters worked an average 2.65 days per …

Methodology for adapting a co-created early childhood development intervention and implementation strategies for use by frontline workers in India and Guatemala: a systematic application of the FRAME-IS framework.

There is little evidence on optimizing the effectiveness and implementation of evidence-based early childhood development (ECD) interventions when task-shifted to frontline workers. In this Methods Forum paper, we describe our adaptation of the International Guide for Monitoring Child Development (GMCD) for task-shifting to frontline workers in Guatemala and India. In 2021-2022, implementers, trainers, frontline workers, caregivers, and international GMCD experts collaborated to adapt the GMCD for a task shifted implementation by frontline workers. We used an eight-step co-creating process: as

Turning liabilities into resources: informal village doctors and tuberculosis control in Bangladesh.

In 1998, the Damien Foundation Bangladesh invited semi-qualified, private "gram dakter" (Bangla for "village doctors") to participate in tuberculosis (TB) programmes in a population of 26 million people in rural Bangladesh. The organization trained 12 525 village doctors to not only refer suspected TB cases for free diagnosis but also to provide directly observed treatment (DOT) free of charge. Source of referral and place of DOT was recorded as part of the standardized TB recording and reporting system, which enabled us to quantify the contribution of village doctors to case detection rates a

Feasibility of controlling lymphatic filarial diseases through primary health care system.

A study was undertaken to explore the feasibility of treatment of clinical cases of filariasis by the Village Health Guides (VHG) and to assess the impact of different dose schedules by Diethylcarbamazine (DEC) on the frequency of acute attacks. It was observed that six days treatment with DEC at a dose of 500 mg daily was the most effective regimen in controlling recurrence of filarial attacks. The VHGs can serve the community in treating filarial cases with DEC.

Community-based strategies for tuberculosis control in migrant communities: how to integrate determinants of vulnerability into decision-making.

BACKGROUND: Tuberculosis (TB) remains a major public health challenge, particularly among migrant populations, who face increased vulnerability due to social and structural determinants of health. In Catalonia, Spain, the Model of Community and Public Action (MACIP) programme was launched in 2023 to improve TB prevention and care through culturally adapted, community-based interventions delivered by community health workers (CHWs). This study aims to describe the vulnerability determinants among migrants diagnosed with TB and enrolled in the MACIP programme, and to present the community-based …

Empowering sex workers? Critical reflections on peer-led risk-reduction workshops in Soweto, South Africa.

Background: Sex workers in South Africa face various forms of structural and interpersonal violence, including police violence, exclusion from health services, and stigmatization and marginalization within their communities. In an attempt to counteract the harmful health effects of criminalization and exclusion, risk-reduction workshops are a key component of HIV prevention programs globally. This paper offers a critical investigation of Creative Space workshops - a South African model of risk-reduction workshops for sex workers - taking place in Soweto, Johannesburg. Drawing on Paulo Freire's

Abortion services for sex workers in Uganda: successful strategies in an urban clinic.

Sex workers' need for safe abortion services in Uganda is greater than that of the population of women of reproductive age because of their number of sexual contacts, the inconsistent use of contraception and their increased risk of forced sex, rape or other forms of physical and sexual violence. We sought to understand sex workers' experiences with induced abortion services or post-abortion care (PAC) at an urban clinic in Uganda. We conducted nine in-depth interviews with sex workers. All in-depth interviews were audiotaped, transcribed, translated, computer recorded and coded for analysis.
