Epidemiology of Lyme disease in Pennsylvania 2006-2014 using electronic health records.

Lyme disease is the most common vector-borne disease in the United States. Electronic health record (EHR)-based research on Lyme disease is limited. We used Geisinger EHR data from 479,344 primary care patients in 38 Pennsylvania counties in 2006-2014 to compare EHR-based Lyme disease incidence rates to surveillance incidence rates, evaluate individual and community risk factors for incident Lyme disease, and to characterize the proportion of cases with diagnoses consistent with post-treatment Lyme disease syndrome in the EHR (PTLDSEHR). We primarily identified Lyme disease cases using diagnos

GP perspectives on genomics in primary care: a qualitative study on using polygenic risk scores to evaluate cancer risk.

BACKGROUND: A polygenic risk score (PRS) enables personalisation of cancer risk and supporting risk stratification for melanoma, colorectal, breast, and prostate cancers. Including a PRS in a cancer risk assessment can facilitate risk-appropriate cancer screening by incorporating an individual's age, sex, family history, and genomic test results. GPs are the likely healthcare professionals to order PRS tests and deliver results to patients within existing preventative health models. AIM: To elucidate GPs' perspectives on the use of PRSs to tailor cancer screening in the Australian primary car

Bacterial vaginosis testing gaps for transmasculine patients may exacerbate health disparities.

INTRODUCTION: Bacterial vaginosis (BV) is associated with non-optimal changes in the vaginal microbiome and increased susceptibility to STIs and HIV in cisgender women. Much less is known about the sexual health of transmasculine people and susceptibility to BV, STIs, and HIV. This study's objective was to assess BV testing and outcomes of transmasculine and cisgender women patient populations at a large, LGBTQ + federally qualified health center. METHODS: Retrospective electronic health record data were extracted for eligible patients having at least one primary care visit between January 1,

Influences on Preventive Health Behaviors After Stroke with Telemonitoring: Patient Perceptions and Practices in an Urban Underserved Population.

BACKGROUND: Although Blood Pressure (BP) self-management and physical activity (PA) are secondary stroke preventive behaviors, adherence gaps exist. This study explored factors influencing these behaviors after telemonitoring experience among patients in an underserved urban community. METHODS: We conducted semi-structured interviews with purposive sampling of patients discharged home after mild stroke who had hypertension and participated in the Telehealth After Stroke Care (TASC) trial. Self-reported short form (SF) surveys included Patient Reported Outcomes Measurement Information System-P

The impact of community water fluoridation cessation on health equity and service utilization: a global scoping review.

OBJECTIVES: Community water fluoridation (CWF) is a widely used population-level intervention for preventing dental caries. However, CWF discontinuation is increasingly debated globally, often without a comprehensive synthesis of the subsequent clinical, service-related, economic, and equity implications. We aimed to systematically map and synthesize empirical evidence regarding dental health and service-based outcomes following CWF cessation. METHODS: For this scoping review, we searched 17 bibliographic databases and grey literature from inception through Oct 31, 2025. Eligible studies repo

Study protocol: a mixed-methods study of women's healthcare in the safety net after Affordable Care Act implementation - EVERYWOMAN.

BACKGROUND: Evidence-based reproductive care reduces morbidity and mortality for women and their children, decreases health disparities and saves money. Community health centres (CHCs) are a key point of access to reproductive and primary care services for women who are publicly insured, uninsured or unable to pay for care. Women of reproductive age (15-44 years) comprise just of a quarter (26%) of the total CHC patient population, with higher than average proportions of women of colour, women with lower income and educational status and social challenges (e.g. housing). Such factors are assoc

Disparities in access to and use of diabetes technologies and therapeutics: a narrative review.

This narrative review applies neo-materialist theory to examine disparities in diabetes care across four domains: clinical trials, diabetes technology, immunotherapies and adjuvant digital technologies. Although systemic marginalisation can occur in many forms (e.g. based on ability, language, sex, gender or age), this review focuses on marginalisation based on race, ethnicity and socioeconomic status, as these are the key areas supported by the current evidence base. Under-representation of minoritised groups in clinical trials of diabetes interventions may limit the generalisability of the d

The Effect of Social Determinants of Health on Clinical Recovery Following Concussion: A Systematic Review.

CONTEXT: Concussion evaluations include a multifaceted approach; however, individual differences can influence test score interpretations and validity. Social determinants of health (SDoH) differentially affect disease risk and outcomes based upon social and environmental characteristics. Efforts to better define, diagnose, manage, and treat concussion have increased, but minimal efforts have focused on examining SDoH that may affect concussion recovery. OBJECTIVE: This review examined previous research that examined the effect of SDoH on concussion recovery of athletes. EVIDENCE ACQUISITION

Healthcare Disparities Identified Between Hmong and Other Asian Origin Groups Living with Chronic Hepatitis B Infection in Sacramento County 2014-2017.

Chronic hepatitis B (CHB) disproportionately affects non-US born Asians. The Hmong have been shown to have the highest rates of CHB and mortality from liver cancer compared to other Asian groups. From September 2014 to September 2017, testing for CHB within Sacramento County was conducted through community-based testing events and an electronic health record alert that identified Asian patients by surname. Demographic and laboratory data were collected for analysis and patients were followed through the study period to assess linkage to care and treatment to compare differences between Asian o

Predicting Emergency Department "Bouncebacks": A Retrospective Cohort Analysis.

INTRODUCTION: The short-term return visit rate among patients discharged from emergency departments (ED) is a quality metric and target for interventions. The ability to accurately identify which patients are more likely to revisit the ED could allow EDs and health systems to develop more focused interventions, but efforts to reduce revisits have not yet found success. Whether patients with a high number of ED visits are at increased risk of a return visit remains underexplored. METHODS: This was a population-based, retrospective, cohort study using administrative data from a large physician

Federal Policy Changes Endanger Medicaid's Critical Role in Perinatal Care and Health Equity.

Medicaid is the nation's largest public health insurer and the primary payer of perinatal care; it covers more than 70 million low-income residents and 41% of births. The One Big Beautiful Bill Act (OBBBA), which was signed into law on July 4, 2025, introduced the largest Medicaid funding reductions in the program's history-an estimated $1.1 trillion over 10 years. In this article, we describe the major OBBBA provisions related to eligibility and Medicaid financing and the anticipated effects of these provisions on the delivery of perinatal care services and related outcomes. Although pregnant
