Parental manual ventilation in resource-limited settings: an ethical controversy.

Lower respiratory tract infections are a leading cause of paediatric morbidity and mortality worldwide. Children in low-income countries are disproportionately affected. This is in large part due to limitations in healthcare resources and medical technologies. Mechanical ventilation can be a life-saving therapy for many children with acute respiratory failure. The scarcity of functioning ventilators in low-income countries results in countless preventable deaths. Some hospitals have attempted to adapt to this scarcity by using hand-bag ventilation, as either a bridge to a mechanical ventilator

Dysphagia Lusoria: Is the Dysmotility Connection Illusory or Real?

Dysphagia lusoria is rare embryologic defect of the aortic arch vasculature characterized by an aberrant retro-esophageal course of the right subclavian artery (RSA), comprising a vascular sling. This may manifest clinically with symptoms of dysphagia or reflux. Given the rarity of this pathology, there remains uncertainty regarding the most effective treatment algorithm. Vascular decompression procedure surgery is often used to rectify the aberrant vascular morphology. However, a low percentage of aberrant arteries result in trachea-esophageal symptoms. As such, we suggest (1) one must exclud

Discriminative Prior - Prior Image Constrained Compressed Sensing Reconstruction for Low-Dose CT Imaging.

X-ray computed tomography (CT) has been widely used to provide patient-specific anatomical information in the forms of tissue attenuation. However, the cumulative radiation induced in CT scan has raised extensive concerns in recently years. How to maintain reconstruction image quality is a major challenge for low-dose CT (LDCT) imaging. Generally, LDCT imaging can be greatly improved by incorporating prior knowledge in some specific forms. A joint estimation framework termed discriminative prior-prior image constrained compressed sensing (DP-PICCS) reconstruction is proposed in this paper. Thi

Neurosurgical interventions for spondyloepiphyseal dysplasia congenita: clinical presentation and assessment of the literature.

BACKGROUND: Spondyloepiphyseal dysplasia (SED) is a rare disease that causes vertebral abnormalities and short-trunk dwarfism. The two forms of SED are congenita and tarda. Each form arises in a genetically distinct fashion and manifests with a different set of complications. SED congenita is more severe, and patients usually display atlantoaxial instability and odontoid hypoplasia. Patients often have various neurologic deficits caused by compression of the spinal cord. The region most affected is the craniovertebral junction (CVJ). METHODS: A review of the PubMed Database, 1970 to the prese

Finite-time and predefined-time dynamical systems for solving strongly pseudomonotone mixed equilibrium problems.

In this paper, we propose a class of proximal dynamical systems (PDSs) with finite-time (FT) stability and predefined-time (PdT) stability for solving strongly pseudomonotone mixed equilibrium problems (MEPs) in Hilbert spaces. Unlike existing approaches for variational inequalities and standard equilibrium problems, the proposed framework is developed specifically for MEPs and provides two distinct dynamical models with FT and PdT convergence. Under the assumptions of strong pseudomonotonicity and Lipschitz-type continuity, we establish the existence and uniqueness of the equilibrium solution

Neurovascular injuries associated with proximal humerus fractures: a review of the current literature.

Neurovascular injuries associated with proximal humerus fractures represent a rare yet clinically significant complication with potential for devastating functional outcomes. Their multifactorial etiology encompasses direct trauma from displaced fracture fragments, indirect stretch or compression mechanisms, and iatrogenic causes during surgical management. The close anatomic relationship between the proximal humerus, axillary artery, and brachial plexus predisposes these structures to combined injury patterns that can threaten limb viability. Diagnosis relies on early recognition through meti

DBA-YOLO: A Dense Target Detection Model Based on Lightweight Neural Networks.

Current deep learning-based dense target detection models face dual challenges in industrial scenarios: high computational complexity leading to insufficient inference efficiency on mobile devices, and missed/false detections caused by dense small targets, high inter-class similarity, and complex background interference. To address these issues, this paper proposes DBA-YOLO, a lightweight model based on YOLOv10, which significantly reduces computational complexity through model compression and algorithm optimization while maintaining high accuracy. Key improvements include the following: (1) a

Update on surgical procedures for carpal tunnel syndrome: What is the current evidence and practice? What are the future research directions?

Carpal tunnel syndrome (CTS) is a multifactorial compression neuropathy. It is reported to be very common and rising globally. CTS's treatment varies from conservative measures to surgical treatments. Surgery has shown to be an effective method for more severe cases. However few unclear aspects and room for further research and improvements still remains. We performed a narrative literature review on the most up to date progress and innovation in terms of surgical treatments for CTS. The simple algorithm of leaving the choice of the surgical method to surgeons' preference and experience (toget

Validation of a decision-making strategy for systolic anterior motion following mitral valve repair.

Low cardiac output syndrome and hypotension are dreadful consequences of systolic anterior motion (SAM) after a mitral valve (MV) repair. The management of SAM in the operating room remains controversial. We validate a recently suggested two-step management method and classification of this complication. This was a teaching hospital-based observational study. We validated a novel two-step conservative management method, consisting in intravascular volume expansion and discontinuation of inotropic drugs (step 1), and increasing the afterload by ascending aorta manual compression while administe

Asphyxial cardiac arrest, resuscitation and neurological outcome in a Landrace/Large-White swine model.

The vast majority of laboratory studies on animals have focused on ventricular fibrillation (VF) and not on cardiac arrest (CA) resulting from asphyxia. The aim of this study was to develop a clinically relevant animal model in Landrace/Large-White swine of asphyxial CA resuscitated using the European Resuscitation Council guidelines. Survival and 24 h neurological outcome in terms of functional deficit were also evaluated. Asphyxial arrest was induced by clamping the endotracheal tube (ETT) in 10 Landrace/Large-White piglets. After 4 min of untreated arrest, resuscitation was initiated by unc

Emergency neurological life support: spinal cord compression (SCC).

Acute spinal cord compression (SCC) is the most serious of the diseases of the cord and should be accorded special attention in neurocritical care. Patients with SCC have a combination of motor and sensory dysfunction that has a distribution referable to one, or a few contiguous, spinal levels. Bowel and bladder dysfunction and neck or back pain are usually part of the clinical presentation but are not uniformly present. Because interventions are time-sensitive, the recognition and treatment of SCC was chosen as an ENLS protocol.

Prediction of Bond Strength in Corroded Reinforced Concrete Using SVM and XGB Methods.

The bond strength of corroded reinforced concrete (CRC) structures is critical for structural safety and long-term durability. However, the corrosion-induced bond degradation process is influenced by multiple, coupled factors and exhibits complex, nonlinear behavior, making it difficult for traditional theoretical models to provide accurate predictions. To address this challenge, this study proposes a novel, unified prediction framework based on machine learning techniques. A total of 391 experimental datasets were collected and compiled, covering key parameters including bond strength, reinfo

Arthroscopic treatment of scaphoid nonunion, a new algorithm after six years practice.

INTRODUCTION: Scaphoid nonunion is still a challenging problem in hand surgery. Till now most of the patients have been treated with open revision, bone grafting and internal stabilization. Arthroscopy plays an increasing role in hand surgery since the last decade. In this retrospective study, we want to present our results and current treatment protocol, adapted after midterm analysis. MATERIAL AND METHODS: In 2017, arthroscopic treatment of scaphoid nonunion was started at our department. Debridement is done arthroscopically as well as insertion of radius cancellous bone graft. Fixation has

Subcutaneous mastectomy in female-to-male transsexuals: Optimizing perioperative and operative management in 8 years clinical experience.

INTRODUCTION: The incidence of complications, especially acute hematoma requiring surgical revision in female-to-male transsexuals (FTMTS), is consistently highly documented in literature with up to 33%. Since 2008 we perform subcutaneous mastectomies in FTMTS with an annually increasing number of cases. Due to an initially high hematoma revision rate in the previously published cohort (2008-2013), we implemented peri- and postoperative preventive measures and compared the results with the recent patient cohort (2014-2016). PATIENTS AND METHODS: The records of 356 patients (712 mastectomies)

An explainable machine learning framework integrating clinical and lipidomic signatures for early severity stratification of hypertriglyceridemic pancreatitis.

BACKGROUND: Hypertriglyceridemic pancreatitis (HTGP) is increasingly recognized as a major etiology of acute pancreatitis and is associated with a higher risk of early severe progression, systemic complications, and mortality compared with other etiologies. However, currently available scoring systems demonstrate limited accuracy and poor mechanistic interpretability in HTGP. AIM: To develop and validate an explainable machine learning model for early stratification of severe progression in HTGP and to explore its underlying lipidomic mechanisms. METHODS: A retrospective cohort of 288 patien

Efficient characterization of individual differences in compression ratio preference.

While wide dynamic range compression (WDRC) is a standard feature of modern hearing aids, it can be difficult to fit compression settings to individual hearing aid users. The goal of the current study was to develop a practical test to learn the preference of individual listeners for different compression ratio (CR) settings in different listening conditions (speech-in-quiet and speech-in-noise). While it is possible to exhaustively test different CR settings, such methods can take many hours to complete, making them impractical. Bayesian optimization methods were used to find CR preferences i
