Treatment of unscheduled bleeding in women using extended- or continuous-use combined hormonal contraception: a systematic review.

BACKGROUND: Unscheduled uterine bleeding is common among women who choose extended or continuous combined hormonal contraception and may subsequently lead to method discontinuation. This systematic review evaluates the evidence on treatments for unscheduled bleeding for women using continuous or extended combined hormonal contraception. STUDY DESIGN: We searched the PubMed database for peer-reviewed articles that were published in any language from inception of the database through January 2012 and were relevant to therapeutic or prophylactic treatments for unscheduled uterine bleeding during

Comprehensive Evaluation and Future Perspectives of Non-Surgical Contraceptive Methods in Female Cats and Dogs.

The issue of stray cats and dogs is a global concern with considerable implications for animal welfare and public health. This review aims to provide an updated and comprehensive analysis of non-surgical contraceptive methods tested in studies controlled in vivo in feline and canine females. Immunocontraception via vaccination against gonadotropin-releasing hormone (GnRH), the luteinizing hormone receptor, zona pellucida proteins, and sperm, or use of viral-vectored delivery, is yet developing. Hormonal treatment (progestins, androgens, or GnRH) analogs act directly to block the reproductive a

Analysis and new contraception frontiers with combined vaginal rings.

Combined vaginal rings (ethinylestradiol (EE)/desogestrel), indicated for contraception, are highly effective, comparable to other combined hormonal contraceptives, such as pills. In addition to this benefit, vaginal rings are easy to use, with a probable lower risk of forgetting, due to their non-daily, monthly schedule. Besides, for users with poor gastric tolerance to oral formulations, they represent a method with safety and comparable extraconceptive benefits. The latest generation rings have a novel polymeric structure, do not need special storage methods and do not generate accelerated

Skin patch and vaginal ring versus combined oral contraceptives for contraception.

BACKGROUND: The delivery of combination contraceptive steroids from a skin patch or vaginal ring offers potential advantages over the traditional oral route. The skin patch and vaginal ring could require a lower dose due to increased bioavailability and improved user compliance. OBJECTIVES: To compare the contraceptive effectiveness, cycle control, adherence (compliance), and safety of the skin patch or the vaginal ring versus combination oral contraceptives (COCs). SEARCH STRATEGY: For trials of the contraceptive patch or the vaginal ring, we searched MEDLINE, POPLINE, CENTRAL, EMBASE, LILA

Insertion of Intrauterine Devices (IUDs) in Sanctuary Chimpanzees (Pan troglodytes) Using Ultrasonography and Radiology to Confirm Placement.

BACKGROUND: Reproduction in captive chimpanzees (Pan troglodytes) can be controlled by the insertion of intrauterine devices (IUDs) for females who do not reliably take oral contraceptives. Previous literature describes the use of improvised devices made from syringe cases as a speculum to accommodate the depth of the vaginal vault. METHODS: Copper IUDs (model T380A) were inserted in two sanctuary-housed chimpanzees. A disposable human small-sized vaginal speculum (Welch Allyn KleenSpec) with an illumination system provided a good view of the cervix. A 3/4 mm dilator and a uterine sound aided

The use of newer progestins for contraception.

The synthetic progestins used for contraception so far are structurally related either to testosterone (estranes and gonanes) or to progesterone (pregnanes and 19-norpregnanes). Several new progestins have been designed to minimize side-effects related to androgenic, estrogenic or glucocorticoid receptor (GR) interactions. Dienogest (DNG) and drospirenone (DRSP) exhibit a partial antiandrogenic action, and DRSP has predominant anti-mineralocorticoid properties. The 19-norpregnanes include Nestorone (NES), nomegestrol acetate (NOMAc) and trimegestone (TMG), and possess a high specificity for bi

IUDs at 1 year: predictors of early discontinuation.

OBJECTIVE: To assess baseline dysmenorrhea and insertion-related pain as predictors of intrauterine device (IUD) removal within 1 year following insertion. STUDY DESIGN: System-wide medical record abstraction 1 year after IUD insertion to identify removals and comparison of baseline characteristics (dysmenorrhea, insertion pain) among women with a removal versus women who continued IUD use was used. Baseline data came from a randomized trial of pain control during insertion. RESULTS: Among 199 insertions, we identified 21 removals and 7 expulsions, a continuation rate of 85.9%. Women with IU

Postpartum contraception: A matter of guidelines.

The postpartum period is the perfect time to access family planning services. WHO guidelines contraindicate combined hormonal contraceptives postpartum in breastfeeding patients between 6 weeks and 6 months after delivery (Medical Eligibility Criteria category 3). On the contrary, the Faculty of Sexual and Reproductive Healthcare and the Centers for Disease Control and Prevention guidelines do not contraindicate their use in women who breastfeed from 6 weeks to 6 months postpartum. New combined hormonal contraceptives with natural estrogens have never been studied in this setting. Guidelines a

Evaluation of acceptability, safety, and continuation rates of centchroman as postabortion nonsteroidal contraceptive pill.

OBJECTIVES: To evaluate acceptability, safety, and continuation rates of centchroman following abortion. STUDY DESIGN: Prospective, observational study. Following spontaneous/induced abortion, women were offered centchroman, 30-mg tablet twice weekly for first 3 months, then once weekly with 1 year of follow-up. RESULTS: Of 120 women who opted for centchroman, continuation rate was 91% at 12 months. There was one case of user failure and one method failure and 26% had infrequent cycles. CONCLUSIONS: Centchroman is safe, effective, has good acceptance, and continuation rate post abortion wit

An Intervention Delivered by Mobile Phone Instant Messaging to Increase Acceptability and Use of Effective Contraception Among Young Women in Bolivia: Randomized Controlled Trial.

BACKGROUND: Although the most effective methods of contraception are available in Bolivia, unmet need for contraception among women aged 15 to 19 years is estimated to be 38% (2008), and the adolescent fertility rate is 71 per 1000 women (2016). Mobile phones are a popular mode to deliver health behavior support. We developed a contraceptive behavioral intervention for young Bolivian women delivered by mobile phone and guided by behavioral science. The intervention consists of short instant messages sent through an app over 4 months. OBJECTIVE: This trial aimed to evaluate the effect of the i

Hormonal contraception as treatment for heavy menstrual bleeding: a systematic review.

A systematic review to compare the effectiveness of contraceptives to treat heavy menstrual bleeding (HMB). PubMed was searched for studies using hormonal contraceptive methods to treat HMB. Two reviewers screened 734 citations and extracted eligible trials based on rigid inclusion and exclusion criteria. Fourteen articles met inclusion criteria. The evidence was good to poor quality and suggested that all contraceptives tested were effective in the treatment of HMB, but the levonorgestrel intrauterine system was the most effective method. Hormonal contraceptives effectively treat HMB. The lev

Emergency contraception: the nurse's role in providing postcoital options.

Emergency contraception refers to pregnancy prevention methods initiated after unprotected sexual intercourse. Research has shown that 75% of the 3.5 million unintended pregnancies that occur in the United States every year could be prevented through use of emergency contraception. Hormonal methods and postcoital insertion of intrauterine devices have been shown to be safe and effective. Nurses play an essential role in the distribution of emergency contraception as patient educators, advocates, and support persons.
