Child vaccination in sub-Saharan Africa: Increasing coverage addresses inequalities.

BACKGROUND: Vaccines have substantially contributed to reducing morbidity and mortality among children, but inequality in coverage continues to persist. In this study, we aimed to examine inequalities in child vaccination coverage in sub-Saharan Africa. METHODS: We analysed Demographic and Health Survey data in 25 sub-Saharan African countries. We defined full vaccination coverage as a child who received one dose of bacille Calmette-Guérin vaccine (BCG), three doses of diphtheria, pertussis, and tetanus vaccine (DTP 3), three oral polio vaccine doses (OPV 3), and one dose of measles vaccine.

Reduction in injection risk behaviors after implementation of a syringe services program, Miami, Florida.

INTRODUCTION: Syringe services programs (SSPs) are evidence-based HIV prevention programs for people who inject drugs. However, not all SSPs operate evidence-based syringe distribution models, such as needs-based distribution. This study aims to provide preliminary evidence from the IDEA SSP on changes in injection risk behaviors over time, and to examine factors, including syringe coverage, associated with injection risk behavior trajectories over time under a one-for-one syringe distribution model. METHODS: We used a prospective observational study design to generate a cohort of SSP clients

Anticholinergic drugs for adult neurogenic detrusor overactivity: a systematic review and meta-analysis.

CONTEXT: There is a lack of evidence about the efficacy and safety of anticholinergic drugs and about the optimal anticholinergic drug, if any, for the treatment of adult neurogenic detrusor overactivity (NDO). OBJECTIVE: Review the current evidence on the efficacy, safety, and tolerability of anticholinergic drugs in the treatment of adult NDO. EVIDENCE ACQUISITION: A literature search was conducted from 1966 to May 2011. Meta-analysis of all published randomised controlled trials (RCTs) comparing anticholinergic drugs with placebo and comparing different types, doses, and routes of adminis

Comparison of Dental Emergencies Among U.S. Military and Civilian Personnel During Combat Operations in 2007-2009.

INTRODUCTION: Information from published studies describing dental treatment of nonmilitary personnel in a military theater of operations is sparse. The primary objective of this study is to determine the number of dental emergencies (DEs) and the types of dental treatment rendered on non-U.S. military (civilian) personnel treated by Navy dentists in 2007-2008 in Iraq and 2009 in Afghanistan. The second objective is to compare the type of DE treatment procedures provided to civilian personnel to the type of DE treatment procedures performed on U.S. military personnel. MATERIALS AND METHODS: N

Integrated Pharmacies at Community Mental Health Centers: Medication Adherence and Outcomes.

BACKGROUND: Patients receiving psychiatric services at community mental health centers (CMHCs) are often prescribed medication that is critical to the treatment of behavioral health conditions, including schizophrenia, bipolar disorder, anxiety, and depression. Previous studies have shown correlation between rates of medication adherence and risk of hospitalization, but potential differences in medication adherence and other outcomes for patients of CMHCs by pharmacy type have not been widely studied. OBJECTIVE: To determine potential benefits of placing a pharmacy within a mental health serv

Taking stock: protocol for evaluating a family planning supply chain intervention in Senegal.

BACKGROUND: In Senegal, only 12% of women of reproductive age in union (WRAU) were using contraceptives and another 29% had an unmet need for contraceptives in 2010-11. One potential barrier to accessing contraceptives is the lack of stock availability in health facilities where women seek them. Multiple supply chain interventions have been piloted in low- and middle-income countries with the aim of improving contraceptive availability in health facilities. However, there is limited evidence on the effect of these interventions on contraceptive availability in facilities, and in turn on family

HIV risk perception, risk behavior, and seroprevalence among female commercial sex workers in Georgetown, Guyana.

A study of 108 female sex workers engaged in prostitution in Georgetown, Guyana, was made in April 1993. Based on interviews and procurement of blood samples, the study investigated relationships between HIV seroprevalences and AIDS knowledge, risk behaviors, client characteristics, and condom use. Street-walkers-as distinct from sex workers in bars, hotels, and Port Georgetown-tended to charge less, be worse off socioeconomically, and have clients who were similarly disadvantaged; they were therefore classified as belonging to a "lower" socio-economic stratum, while the other workers were cla

Structural Accountability and Practice-Level Governance as Dual Pathways to Accreditation-Seeking Intention in Private Dental Practice: Evidence from a Mixed Public-Private Healthcare System.

Background/Objectives: Private dental markets operating under fee-for-service arrangements lack the structural accountability mechanisms that typically drive quality governance adoption in publicly funded systems. In Romania, where no private dental office currently holds formal accreditation, accreditation-seeking reflects a genuine behavioral choice rather than a regulatory compliance. This study examined whether financing structure and Professional Governance behavior operate as distinct determinants of accreditation-seeking intention among private dental practitioners and which pathway off

Opioid dependence in 15 patients after a work injury.

OBJECTIVE: To document aspects of the management of 15 patients with workers' compensation injuries which led to opioid dependence and suggest preventive strategies. METHOD: Details of general practitioner and deputising service visits for a 38-week period, 1 May 1991 - 22 January 1992, were obtained from WorkCover case files and general practitioners' patient records, and claims for reimbursement for afterhours home visits (Item 97) were obtained from the Health Insurance Commission (HIC) for the years 1991, 1992 and 1993. RESULTS: Fourteen of the patients had a lumbosacral injury. A total

Subcutaneous Injection of Adalimumab Trial compared with Control (SCIATiC): a randomised controlled trial of adalimumab injection compared with placebo for patients receiving physiotherapy treatment for sciatica.

BACKGROUND: Biological treatments such as adalimumab (Humira®; AbbVie Ltd, Maidenhead, UK) are antibodies targeting tumour necrosis factor alpha, released from ruptured intervertebral discs, which might be useful in sciatica. Recent systematic reviews concluded that they might be effective, but that a definitive randomised controlled trial was needed. Usual care in the NHS typically includes a physiotherapy intervention. OBJECTIVES: To test whether or not injections of adalimumab plus physiotherapy are more clinically effective and cost-effective than injections of saline plus physiotherapy f

Engagement of non-government organisations and community care workers in collaborative TB/HIV activities including prevention of mother to child transmission in South Africa: opportunities and challenges.

BACKGROUND: The implementation of collaborative TB/HIV activities may help to mitigate the impact of the dual epidemic on patients and communities. Such implementation requires integrated interventions across facilities and levels of government, and with communities. Engaging Community Care Workers (CCWs) in the delivery of integrated TB/HIV services may enhance universal coverage and treatment outcomes, and address human resource needs in sub-Saharan Africa. METHODS: Using pre-intervention research in Sisonke district, KwaZulu-Natal, South Africa as a case study, we report on three study obj

Design of the Middle East African Registry for Women's Cardiovascular Diseases: Protocol for a Multicenter Observational Study.

BACKGROUND: Cardiovascular disease (CVD) is a leading cause of morbidity and mortality in the Middle East and Africa (MEA), with a rising incidence particularly among women. Regional factors such as limited health care access, cultural barriers, and sex-specific risk factors exacerbate this burden. Despite this, women remain significantly underrepresented in cardiovascular research, and no large-scale, multicenter prospective trials have been conducted to provide national data. To address this gap, we established the Middle East African Registry Women Cardiovascular Disease (MEA-WCVD) to creat

myoActivation® for Chronic Pain Care That Changes the Biotensegral Properties of the Human Body: A Motion Laboratory Case Report.

The human body is a biotensegral structure capable of bending and twisting in multiple directions as a functional, integrated movement unit, while maintaining balance and an ability to return to any pre-movement position. Myofascial dysfunction caused by soft tissue stiffness - muscles in sustained contraction, tethered skin, scars, or anomalous fascial tension - can affect the whole fascial continuum, leading to mobility impairment and chronic pain. Myofascial dysfunction is not just a local phenomenon, as the range of motion at one skeletal articulation may affect the whole system. myoActiva

Veterans Health Administration Pain Management Quality Improvement in a Post-Commission on Accreditation for Rehabilitation Facilities Era.

Tertiary interdisciplinary pain rehabilitation programs (IPRPs) provide integrated, comprehensive pain rehabilitation using an evidence-based biopsychosocial approach delivered by teams offering multi-modal care. A 2009 Veterans Health Administration (VHA) Directive mandated implementation of Commission on Accreditation for Rehabilitation Facilities (CARF) accredited IPRPs in each Department of Veterans Affairs (VA) service network. Commission on Accreditation for Rehabilitation Facilities accreditation included regular program surveys to reinforce continuous quality and improvement standards.

Telemedical maritime assistance over three decades in Denmark.

INTRODUCTION: Seafaring is associated with higher occupational hazards and health risks than shore-based work. Telemedical maritime assistance services (TMAS) provide remote specialist medical support to seafarers. In Denmark, a unified national TMAS, Radio Medical Denmark (RMD), was established in 1995. This study aimed to describe the 30-year development and clinical activity of the RMD. METHODS: This was a single-centre retrospective cohort study of all consultations provided by the RMD from 1 September 1995 to 31 December 2024. Data were extracted from annual reports based on original med

Paying in the dark: third-party administrator fees and the limits of employer cost control in self-insured health plans.

INTRODUCTION: Employers are one of the largest purchasers of health care in the United States, covering more than 165 million people, yet they face persistent difficulty controlling costs. Most enrollees are in self-insured plans, which rely on third-party administrators (TPAs) to build provider networks and adjudicate claims. The fees employers pay these administrators are an under-documented source of health care spending. While recent litigation and news reporting allege that administrators overpay providers and charge large fees, the peer-reviewed literature offers limited detail on how th

Depression and anxiety among seafarers in Canada during the COVID-19 pandemic.

BACKGROUND: Seafarers in the international maritime industry face longstanding mental health challenges due to the remote and demanding nature of their occupations. During the COVID-19 pandemic, these challenges were exacerbated by extended contracts, limited shore leave, and restricted access to essential services ashore. This study presents the first exploration of seafarers' mental health in Canada during the pandemic. MATERIAL AND METHODS: A cross-sectional survey was conducted between February and December 2022 utilizing the Patient Health Questionnaire-9 (PHQ-9) for depression and the G

Predictors of Medicaid managed care plan performance on opioid use disorder treatment quality metrics.

INTRODUCTION: Medicaid managed care plans (MCPs) and states play essential roles in supporting access to high-quality opioid use disorder (OUD) treatment services. This study aimed to identify MCP and state-level policies associated with better plan performance on indicators of quality OUD treatment. METHODS: Publicly available data on Medicaid MCPs' profit status, behavioral health contracting arrangements, market share, buprenorphine prior authorization and quantity limit policies and state Medicaid policies were linked with plan-level measures of OUD treatment quality from the National Com

Colonic transit studies and colonic manometry in pediatric constipation.

OBJECTIVES: In a subgroup of children with refractory constipation, colonic function investigations, such as colonic transit scintigraphy (CS) and colonic manometry (CM), are used to define the underlying pathophysiologic mechanisms. There are limited studies comparing colonic transit and contractile function. We aimed to correlate CS and CM and assess whether specific manometric patterns might predict the luminal transit findings. METHODS: Children with refractory constipation undergoing both CS and CM between 2012 and 2022 at two Tertiary Pediatric Gastroenterology Services were retrospecti
