The fate of positive intraoperative cultures following conversion total hip arthroplasty.

INTRODUCTION: The objectives of this study are to report the rates of positive intraoperative cultures obtained during conversion total hip arthroplasty (THA) according to index surgery, and to describe the natural history of treatment for a consecutive series of patients with unexpected positive intraoperative cultures during conversion THA. METHODS: We reviewed all patients at 2 institutions undergoing conversion THA from prior open reduction and internal fixation (ORIF) of acetabular and hip fractures or hemiarthroplasty for displaced femoral neck fractures from 2011 to 2018. Intraoperativ

Laparoscopic cholecystectomy for acute cholecystitis.

One hundred and eight patients with histopathologically confirmed acute cholecystitis underwent laparoscopic or attempted laparoscopic cholecystectomy in the Security Forces Hospital, from October 1991 to April 1996 were retrospectively reviewed. All the patients had routine laboratory works including abdominal ultrasonography. Females represented 75% and 57% had previous admission. Diabetes mellitus was found in 43.5%. Laparoscopic cholecystectomy was successfully completed in 71.2%. The main reasons for conversion in 31 patients were adhesions and unclear anatomy in 87%. The mean operative t

[Laparoscopy-assisted colorectal surgery. Early outcome in benign and malignant diseases--a prospective study of 120 patients].

We present our results with laparoscopic-assisted colorectal surgery in 120 patients during the time from January 1993 to September 1996. The types of procedures cover almost the whole spectrum of colorectal surgery. They included hemicolectomies, signmoid resections, low anterior resections, Hartmann closures, proctocolectomies and rectopexies. 127 patients were subjected to laparoscopic operation, 7 needed conversion to open surgery (7% conversion rate). Average operation time was 145 +/- 58 min, length of postoperative stay 12 +/- 4 days. Oral food intake was started at the second day posto…

Robot-assisted laparoscopic radical nephrectomy for renal cell carcinoma with direct colon invasion and concomitant inferior vena cava tumor thrombus: A case report.

Resectable renal cell carcinoma (RCC) with colon invasion is extremely rare and is usually managed by open surgery. To our knowledge, this is the first case report documenting the management of a patient with robot-assisted laparoscopic radical nephrectomy (RARN) with colon resection and inferior vena cava thrombotectomy. The procedure was completed without conversion to open surgery or major peri- and post-operative complications, confirming the technical feasibility of RARN in this challenging setting. However, given the complexity and high morbidity risk, this approach should be reserved fo…

Factors associated with perioperative mortality after late open conversion for failed endovascular aortic repair.

INTRODUCTION: The aim of this study was to perform a systematic review about the clinical and technical aspects of late open conversion for failed endovascular aortic repair (EVAR) of abdominal aortic aneurysms (AAA), and to investigate if the need for suprarenal aortic cross clamping, graft infection, urgent procedures, endoleaks and aortic rupture were associated with an increase of 30-days (perioperative) mortality. EVIDENCE ACQUISITION: A literature search was conducted on PubMed using the words "open conversion endovascular" on December 29th, 2021. Studies included randomized controlled

Effects of previous abdominal surgery incision type on complications and conversion rate in laparoscopic cholecystectomy.

For laparoscopic cholecystectomy, previous abdominal operations are seen as a relative contraindication. The purpose of this study was to investigate the effects of the incision type of previous abdominal surgery on laparoscopic cholecystectomy in terms of complications and conversion to open surgery. Data from 677 patients who had previously undergone abdominal surgery before undergoing laparoscopic cholecystectomy were prospectively collected and evaluated. From the previous operations, the incisions were upper abdominal in 66 patients, lower abdominal in 567, and upper plus lower in 44. Con…

Endovascular Conversion of a Failed Nellix AAA-Repair by a Custom-Made Branched Device.

PURPOSE: This technical note presents a case of a patient with a failed Nellix device (Endologix, Irvine, Calif) who was not deemed fit for open conversion. Our planned approach for repair involved an endovascular procedure utilizing a custom-made branched device. TECHNIQUE: An endovascular repair was performed via a custom-made four outer branched device in conjunction with a custom-made bifurcated graft featuring inverted limbs (Cook Inc., Bloomington, Ind). All branches were connected to the target vessel with Gore Viabahn VBX balloon-expandable covered stents (Gore & Associates Inc.). CO

[Clinical practice guideline for laparoscopic radical resection of intrahepatic cholangiocarcinoma(2026)].

Intrahepatic cholangiocarcinoma (ICC) is highly aggressive and prone to lymph node metastasis or invasion of adjacent bile ducts, blood vessels, and organs. In recent years, reports on laparoscopic radical resection for ICC have increased annually, demonstrating short-and long-term outcomes comparable to those of open surgery. However, further standardization is still required in areas such as surgical indications, major hepatectomy or resection in difficult anatomical locations, combined resection of bile ducts, blood vessels, or multiple organs, and surgery following conversion or downstagin

[Laparoscopic cholecystectomy in acute cholecystitis: predictors of conversion to open cholecystectomy and preliminar results].

Inspite of increased technical difficulties and high incidence of conversion to open procedures and complications, laparoscopic cholecystectomy is a well established treatment for acute cholecystitis. In this study we reported our results in patients with acute cholecystitis undergoing laparoscopic cholecystecomy from 1998 to 2003. We found out that laparoscopic cholecystectomy was safe and was carried out with acceptable conversion rate and low morbidity. Predictors of complications were delay of surgery more than 48 hours following the onset of symptoms, leucocytosis > 15.000 U/microl and ga
