The "Inferior" Petrolingual Ligament for Endoscopic Endonasal "Lateral" Transpetrosal Approach.

OBJECTIVE: Endoscopic endonasal approach to the petrous apex is feasible but presents challenges due to the obstruction posed by the internal carotid artery (ICA). Medial or inferomedial approaches, which involve ICA transposition or contralateral transmaxillary visualization, provide only limited lateral exposure of the apex. This study introduces a novel endoscopic endonasal "lateral" transpetrosal approach (EELPA) to the petrous apex. METHODS: Endoscopic endonasal transpterygoid anatomical dissections were performed bilaterally on nine formalin-fixed cadaveric heads. The study focused on t

Alterations in Sphenoid Anatomy in Craniosynostosis: Implications for Fronto-Orbital Advancement.

ObjectiveFronto-orbital advancement involves removal of the fronto-orbital bandeau. Visualization of the saw blade is lost as it passes through the fronto-orbital-sphenoid junction (FOSJ), placing the temporal lobe at risk of injury. We aim to provide a 3D analysis of the space surrounding this osteotomy to differentiate various types of craniosynostoses.DesignRetrospective cohortSettingInstitutional.PatientsThirty patients with isolated unicoronal synostosis, nonsyndromic bicoronal synostosis, metopic synostosis, Apert syndrome, Crouzon syndrome, and Muenke syndrome.InterventionsCT scans cond

Electrocochleography summating potential seen on auditory brainstem response in a case of superior semicircular canal dehiscence.

BACKGROUND: Superior canal dehiscence syndrome (SCDS) is a condition in which an abnormal communication between the superior semicircular canal and the middle cranial fossa causes patients to hear internal noises transmitted loudly to their affected ear as well as to experience vertigo with pressure changes or loud sounds. Patients with SCDS can have an elevated ratio of summating potential (SP) to action potential (AP) as measured by electrocochleography (ECochG). Changes in this ratio have been observed during surgical intervention to correct this abnormal communication. CASE DESCRIPTION: W

A radiologic and anatomic study of the superior semicircular canal.

OBJECTIVES: The present study sought to determine whether there is a correlation between the prevalence of superior semicircular canal (SSC) dehiscence (SSCD) on temporal CT and population age. The secondary objective was to identify anatomic factors for SSCD by studying SSC diameter and its protrusion into the middle cranial fossa. The aim was to determine the acquired or congenital origin of SSCD (Minor's syndrome). MATERIAL AND METHOD: A single-center retrospective radiological and anatomic study included 180 CT scans of 354 petrous parts of the temporal bone taken between January and Dece

Surgical management of spontaneous cerebrospinal fluid leakage through temporal bone defects--case series and review of the literature.

Increasing numbers of cases of idiopathic cerebrospinal fluid (CSF) leakage through temporal bone defects (TBD) have been recently reported, mainly in otolaryngologic journals. Those cases are referred to as spontaneous temporal bone encephaloceles (TBE). Three surgical approaches have been advocated for this condition: the transmastoid approach (TMA), the middle cranial fossa approach (MCFA), or a combination of both. We conducted a retrospective study of all 11 consecutive patients who underwent 12 middle cranial fossa craniotomies for the treatment of CSF leakage through TBD in our institut

Endoscopic fenestration of middle fossa arachnoid cysts: a technical description and case series.

BACKGROUND: Arachnoid cysts are intra-arachnoidal cerebrospinal fluid collections most frequently seen in the middle cranial fossa. The optimal method of treatment for symptomatic arachnoid cysts remains controversial and includes cyst shunting, open craniotomy and endoscopic fenestration. All these techniques, however, have been associated with the development of postoperative subdural fluid collections. We describe a new endoscopic transcortical technique that attempts to avoid this complication. METHODS: Six patients with middle cranial fossa arachnoid cysts were treated with endoscopic fe

Congenital meningeal sarcoma--a case report.

A 30-year-old multigravida woman was admitted to the obstetrical unit in the third trimester of gestation, because of a large-for-date uterus. Repeated ultrasonic examinations over a 2-week period revealed an increase in biparietal diameter far too large to be normal. This progressive asymmetrical hydrocephalus was diagnosed as caused by an intracranial space-occupying lesion in the middle cranial fossa. Because of the only minimal thickness of the cerebral cortex prognosis was judged so poor, that labor was induced. A stillborn female infant was delivered vaginally after an ultrasonically gui

Mechanism of transverse fracture of the skull base caused by blunt force to the mandible.

Transverse fracture of the skull base is common both in the crushing of temporal regions of the skull and in the case of force acting on one temporal region. However, the mechanism of transverse skull base fracture caused by maxillofacial force has not been fully clarified. To provide an injury identification basis for forensic pathologists and clinicians, this paper combines accident reconstruction and finite element analysis methods to study the injury mechanism of an incomplete transverse fracture of skull base after the injured individual's mandible was subjected to violence in a traffic a

Non-keratinizing undifferentiated carcinoma of the nasopharynx.

Nasopharyngeal carcinoma is the predominant tumor type arising in the nasopharynx with cervical lymph nodes present in 60-90% of all cases at the time of presentation. The most frequent pathological varieties include squamous cell carcinoma well-differentiated keratinizing, moderately differentiated non-keratinizing and an undifferentiated type. We present a case of non-keratinizing undifferentiated carcinoma of the nasopharynx with parapharyngeal and middle cranial fossa space involvement in an 18-year-old male who has been admitted in our hospital for recurrent right ear otitis media. Endoscope-assisted microsurgical resection of epidermoid tumors of the cerebellopontine angle.

OBJECT: Epidermoid tumors located in the cerebellopontine angle (CPA) are challenging lesions because they grow along the subarachnoid spaces around delicate neurovascular structures and often extend into the middle cranial fossa. The purpose of this study was to determine the value of endoscopic assistance in the microsurgical resection of these lesions, in which total removal is the therapy of choice. METHODS: Eight patients harboring an epidermoid tumor of the CPA were treated using an endoscope-assisted microsurgical technique. A retrosigmoid suboccipital approach was used in five patient

[The Ascending Pharyngeal Artery, Accessory Meningeal Artery, Lingual Artery, Vasa Nervorum, and Potential Anastomotic Channels].

The ascending pharyngeal, accessory meningeal, and lingual arteries branch from the proximal segment of the external carotid artery. These branches give rise to smaller branches that contribute blood supply to the pharyngeal mucosa, parapharyngeal tissue, middle ear, submandibular tissues, tongue, and dura mater of the middle and posterior fossa. These arteries may also supply the cranial nerves and have potential anastomotic channels that function with the internal carotid and vertebral arteries. M igration of embolic material into the vasa nervorum and potential anastomoses may cause complic

Facial translocation approach to infratemporal fossa and cranial base in extensive angiofibroma: A review of 7 cases.

Angiofibroma extending to infratemporal fossa, orbit and middle cranial fossa is a difficult problem for a surgeon to tackle. Traditional extracranial excision with radiotherapy for the intracranial extension was practiced for a long time with variable results. Different approaches to remove such a tumour are described but most of them are not free from resultant morbidity in the form of facial asymmetry and incomplete tumour removal. Facial translocation approach facilitates complete tumour removal without cutting through the tumour thereby reducing per-operative blood loss. It also avoids fa

[The use of the temporal muscle for closing postoperative wounds].

The correct use of the musculus temporalis may close rather extensive defects of the base of the skull, yielding good functional and cosmetic results. For plastic purposes, the following types of a graft of the musculus temporalis are applied: 1) a simple interpolated graft; 2) a split-thickness interpolated graft; and 3) a free graft (by crossing the tendon of the musculus temporalis, which allows the muscle to be additionally displaced) for closure of defects after removal of tumors from the socket, maxillary, sphenoidal sinuses, infratemporal fossa, the bases of the middle cranial fossa, fo

Surgical exposure of the facial nerve indications and techniques.

Lesions producing facial nerve palsy may occur within the temporal bone anywhere between the internal auditory canal and the stylomastoid foramen. Surgical exposure of this nerve may be necessary for decompression, grafting, rerouting, or removal of such lesions as acoustic tumour, meningioma, facial nerve neuroma, and cholesteatoma. Contemporary surgical exposure of the facial nerve has as its aim adequate exposure of the facial nerve at any point in its course, with preservation of hearing and vestibular function, without further injury to the facial nerve and the necessity for producing a m

Intracranial stenting after tumor exeresis complicated by delayed kinking of the middle cerebral artery.

During intracranial tumor resection, the delayed kinking of a major encased vessel has never been described in literature. We present a case which required urgent endovascular treatment performed through a stent positioning. A patient was hospitalized with symptomatic sphenoid meningioma in the left middle cranial fossa. Twelve days after surgery, right-sided hemiplegia and aphasia occurred. Digital subtraction arteriography revealed a kinking of the M1 segment of the left middle cerebral artery and diffuse vasospasm. At first, intra-arterial nimodipine has been administered, obtaining the rem

The zygomatico-temporal approach to the skull base: a critical review of 11 patients.

The zygomatico-temporal approach to the base of the skull is a relatively new but established surgical technique. The approach involves the removal of the zygomatic bone to provide access to the skull base, middle cranial fossa, parasellar region and interpeduncular cistern with minimal brain retraction. An excellent view of the bifurcation of the basilar artery and suprasellar region is provided. The outcome of 11 patients undergoing this procedure is reported with particular reference to the post-operative morbidity and the cosmetic result.

[Non-traumatic rhinorrhea from a latero-sellar fistula of the temporal fossa adjacent to an empty sella turcica. Computed tomography aspects; surgical treatment].

The authors report a case of non traumatic rhinorrhea due to an osteo-dural fistula communicating between the latero-sellar part of the left middle cranial fossa and the sphenoidal sinus. This fistula, probably congenital, was adjacent to an intrasellar arachnoidocele ("empty" sella turcica), but was not responsible for it. This lesion was demonstrated by CT scan associated with the injection of Metrizamide. The surgical treatment (trans-sphenoidal approach) allowed complete cure of the fistula and its associated symptoms.
