A pilot study on craving and its relationship to self-ratings of depression and anxiety in prescription opioid use disorder.

The role of craving in opioid use disorder (OUD) has been well established with respect to heroin but less so with prescription opioids. This pilot study, conducted in 18 treatment-seeking patients with prescription OUD and 18 healthy volunteers, assessed spontaneous (in the moment) and cue-induced craving and their relationship to depression and anxiety. Patients (vs. healthy volunteers) exhibited increased spontaneous craving for prescription opioids. Relative to brief (10 s) random presentations of neutral and affective images, presented drug (prescription opioids) images elicited greater c…

A randomized controlled experiment testing the use of virtual reality to trigger cigarette craving in people who smoke.

Automated delivery of therapy in virtual reality (VR) has the potential to be used for smoking cessation. Most obviously, it could be used to practise and establish alternative reactions to smoking cues. The first step in treatment development is to show that VR environments can trigger sufficient cravings in smokers. We evaluated a new VR public house outdoor scenario with 100 individuals who smoked daily. Participants were randomly assigned to the VR scenario with smoking cues or a neutral experience in VR. The VR experiences were presented in a standalone VR headset. Before and after VR, we…

Transcranial Direct Current Stimulation for Food Craving in Women Affected by Overweight and Obesity: A Randomized Controlled Trial.

INTRODUCTION: Craving is a multifactorial behavior caused by central circuit imbalance. The proposed treatments involve exercise and reduced food intake. However, the treatments frequently fail. This study aimed to investigate the effect of 10 consecutive sessions of anodal transcranial direct current stimulation over the right dorsolateral prefrontal cortex on food craving and eating consumption of women affected by overweight and obesity. METHODS: A randomized double-blind controlled trial with 50 volunteers was divided into two groups (active-tDCS: n = 25 and sham-tDCS: n = 25). There were…

Epac2-mediated synaptic insertion of Ca2+-permeable AMPARs in the nucleus accumbens contributes to incubation of cocaine craving.

The accumulation of GluA2-lacking Ca2+-permeable AMPARs (CP-AMPARs) in the medium spiny neurons (MSNs) of the nucleus accumbens (NAc) is required for the expression of incubation of cocaine craving. The exchange protein directly activated by cAMP (Epac) is an intracellular effector of cAMP and a guanine nucleotide exchange factor for the small GTPase Rap1. Epac2 has been implicated in the trafficking of AMPA receptors at central synapses. We tested the hypothesis that Epac2 activation contributes to the accumulation of CP-AMPARs in NAc MSNs and incubation of cocaine craving. Here we demonstrat…

Differential effects of UPPS-P impulsivity on subjective alcohol response and craving: An experimental test of acquired preparedness.

Recent studies have extended the acquired preparedness model to experimental data, finding that impulsivity predicts subjective alcohol response, a related yet distinct construct from expectancies. However, studies have not tested whether specific facets of impulsivity predict subjective response, or whether impulsivity indirectly predicts alcohol craving through subjective response. Young adults who reported past-month binge drinking (N = 448) participated in a placebo-controlled alcohol administration study. Mediation models tested whether UPPS-P impulsivity facets indirectly predicted alcoh…

Reward-Related Responses and Tonic Craving in Cocaine Addiction: An Imaging Study of the Monetary Incentive Delay Task.

BACKGROUND: Cocaine addiction is associated with altered sensitivity to natural reinforcers and intense drug craving. However, previous findings on reward-related responses were mixed, and few studies have examined whether reward responses relate to tonic cocaine craving. METHODS: We combined functional magnetic resonance imaging and a monetary incentive delay task to investigate these issues. Imaging data were processed with published routines, and the results were evaluated with a corrected threshold. We compared reward responses of 50 cocaine-dependent individuals (CDs) and 45 healthy cont…

Is transcranial direct current stimulation an effective modality in reducing food craving? A systematic review and meta-analysis.

Non-invasive electrical stimulation of the brain has recently been extensively investigated to regulate food craving. However, the existing literature is controversial and there are some important questions which need to be addressed about clinical and technical factors contributing to efficacy of this method. A systematic search was performed in reliable scientific databases, and 15 eligible studies were identified. The pooled standardized mean differences for the effects of transcranial direct current stimulation (tDCS) on Visual Analogue Scale, energy intake and food craving questionnaire w…

The PPARγ Agonist Pioglitazone Fails to Alter the Abuse Potential of Heroin, But Does Reduce Heroin Craving and Anxiety.

Possibly through its effects on glia, the peroxisome proliferator-activated gamma receptor (PPARγ) agonist pioglitazone (PIO) has been shown … failed to alter the reinforcing or positive subjective effects of heroin, but it did reduce heroin craving and overall anxiety. Although we were unable to replicate the robust effects found in preclinical models, these data provide an indication of drug effects that deserves further exploration.

The inclusion of functional connectivity information into fMRI-based neurofeedback improves its efficacy in the reduction of cigarette cravings.

Real-time fMRI (rtfMRI) neurofeedback (NF) facilitates volitional control over brain activity and the modulation of associated mental functions. The NF signals of traditional rtfMRI-NF studies predominantly reflect neuronal activity within ROIs. In this study, we describe a novel rtfMRI-NF approach that includes a functional connectivity (FC) component in the NF signal (FC-added rtfMRI-NF). We estimated the efficacy of the FC-added rtfMRI-NF method by applying it to nicotine-dependent heavy smokers in an effort to reduce cigarette craving. ACC and medial pFC as well as the posterior cingulate …

The effects of exogenous progesterone on drug craving and stress arousal in cocaine dependence: impact of gender and cue type.

AIMS: Exogenous progesterone has been shown to attenuate the rewarding effects of cocaine. However, its effects on provoked drug craving, stress arousal and cognitive performance has not been systematically investigated in cocaine dependent men and women. Thus, we conducted a double-blind placebo-controlled study assessing the efficacy of progesterone in reducing provoked drug craving, stress system arousal and improving cognitive performance in cocaine dependent men and women. METHODS: Forty-two early abstinent treatment-seeking cocaine dependent individuals were randomly assigned to either …

Food cravings discriminate between anorexia and bulimia nervosa. Implications for "success" versus "failure" in dietary restriction.

Food cravings are subjective, motivational states thought to induce binge eating among eating disorder patients. This study compared food cravings across eating disorders. Women (N=135) diagnosed with anorexia nervosa, restrictive (ANR) or binge-purging (ANBP) types, or bulimia nervosa, non-purging (BNNP) or purging (BNP) types completed measures of food cravings. Discriminant analysis yielded two statistically significant functions. The first function differentiated between all the four group pairs except ANBP and BNNP, with levels of various food-craving dimensions successively increasing fo…

Incidence and patterns of polydrug use and craving for ecstasy in regular ecstasy users: an ecological momentary assessment study.

BACKGROUND: Previous studies employing retrospective assessments methods found that regular ecstasy users frequently use alcohol, marihuana and other drugs in combination with ecstasy. METHODS: Twenty-two participants (13 males, 9 females) wore a wrist actigraph/data recorder to record real-time drug use and ecstasy craving for 6 weeks. Rates of alcohol and drug use on ecstasy use versus non-use nights, and before, during, and after ecstasy use were analyzed with generalized estimation equations (GEE). Craving was modeled with GEE and linear mixed models. RESULTS: Approximately 70% of ecstas…

The acute anti-craving effect of acamprosate in alcohol-preferring rats is associated with modulation of the mesolimbic dopamine system.

Acamprosate (Campral) is a drug used clinically for the treatment of alcoholism. In order to examine further the time-course and mechanism of action of acamprosate, the effect of acute and repeated acamprosate administration was examined on (i) operant ethanol self-administration and (ii) voluntary home cage ethanol consumption by alcohol-preferring Fawn-Hooded, iP and Alko Alcohol (AA) rats. Acutely, acamprosate was shown to cause a significant decrease in operant ethanol self-administration by Fawn-Hooded and alcohol-preferring iP rats in part by decreasing the motivational relevance of a sp

Early event-related potential differentiation of alcohol cues associates with subjective craving levels in severe alcohol use disorder.

OBJECTIVE: Alcohol use disorder (AUD) contributes to 4.7% of deaths worldwide, yet assessment relies on subjective craving reports. Event-related potentials (ERPs) provide objective markers of cue reactivity linked to craving and motivational salience. However, emphasis on later components (P3) may overlook earlier, automatic processes reflecting the transition from controlled to habitual responding. This study examined whether early and late ERP components differentiate alcohol from non-alcohol stimuli in severe AUD and associate with craving. METHODS: Seventy-eight inpatients with severe AU…

Sequential dual-target tDCS for tobacco use disorder: A longitudinal pilot case series targeting craving and inhibitory control.

BACKGROUND: Transcranial direct current stimulation (tDCS) is increasingly explored as an adjunctive intervention for Tobacco Use Disorder (TUD). Studies typically employ single-site stimulation protocols, and reported effects on craving and relapse have varied. OBJECTIVE: To examine whether sequential dual-target tDCS could influence smoking behavior and relapse-relevant mechanisms in TUD. METHODS: In this case series, three adults with TUD underwent a two-week sequential tDCS protocol targeting the left dorsolateral prefrontal cortex and pre-supplementary motor area. Assessments were condu…
