Quantitative plaque characterization, pericoronary fat attenuation index, and fractional flow reserve: a novel method for differentiating between stable and unstable angina pectoris in a case-control study.

BACKGROUND: Accurate diagnosis of coronary artery disease is essential for preventing serious cardiovascular events. Although coronary computed tomography angiography (CCTA) is widely used in the clinic, it is limited because it only provides anatomical information, which makes differentiating in-depth between subtypes of noncalcified plaques and assessing the inflammatory state of coronary vessels difficult. Fractional flow reserve with computed tomography (FFR-CT) can be combined with CCTA to form a hybrid anatomic-physiologic diagnostic strategy. This study aimed to improve the recognition

Integrating Imaging and Invasive Pressure Data into a Multiscale Whole-Heart Model.

Cardiovascular diseases are the leading cause of death. Clinical data used to decide treatment are hard to integrate and interpret, making optimal treatment selection difficult. Personalized models can be used to integrate clinical data into a physics and physiology-constrained framework, but their clinical application faces limitations due to complex calibration and validation. In this study, we present a novel systematic calibration method for a whole-heart, multiscale, electromechanics model using emulators, sensitivity analysis, and history matching. Using cardiac motion derived from ECG-g

Evaluation of cystic neoplasms using EUS, MRI, and CT scan and treatment strategy.

BACKGROUND: Cystic neoplasms, and pancreatic cystic lesions (PCLs) in general, are increasingly a practice dilemma in diagnosis with their increasing incidental discovery and indeterminate risk of malignancy. Characterization is needed to maximize patient outcomes, balancing the risks of overtreatment against the risks of missed malignancy. In this review, endoscopic ultrasonography (EUS), magnetic resonance imaging (MRI), and computed tomography (CT) diagnostic performance, prognostic information, and clinical utility have been extensively reviewed in the imaging and treatment of cystic neopl

Robot-assisted right hemicolectomy for ascending colon cancer with a rare common trunk for the ileocolic and middle colic arteries: a case report.

INTRODUCTION: Anatomical variations in the arterial supply to the right colon are clinically relevant in patients undergoing laparoscopic and robotic colectomy, as misidentification of the vessels may lead to inadequate lymphadenectomy or bowel ischemia. A common trunk formed by the ileocolic artery (ICA) and middle colic artery (MCA), arising from the superior mesenteric artery (SMA), is an uncommon vascular configuration that may complicate surgical decision-making. PRESENTATION OF CASE: A 73-year-old man presented with constipation and abdominal pain. Colonoscopy revealed a circumferential

BACKGROUND: Hematoma expansion (HE) is a significant risk factor for poor prognosis in patients with intracerebral hemorrhage (ICH). Accurately predicting HE is crucial for determining optimal treatment strategies. METHODS: This study enrolled 452 patients with ICH from 10 hospitals. To predict HE, 28 clinical variables available on patient arrival (including medical history, ICH location, and ICH volume) and 1142 radiomics features extracted from noncontrast computed tomography images of the ICH regions were used. Clinical variables and radiomics features were selected using gradient boostin

A tensor-based measure for estimating blood damage.

Implantable ventricular assist devices give hope of a permanent clinical solution to heart failure. These devices, both pulsatile- and continuous-flow, are presently used as medium-term bridge to heart transplant or recovery. While long-term use of continuous-flow axial and centrifugal pumps is being explored, the excessive level of blood damage in these devices has emerged as a design challenge. Blood damage depends both on shear stress and exposure time, and device designers have relied traditionally on global space- and time-averaged estimates from experimental studies to make design decisi

Artificial Intelligence and Multi-Omics Approaches in the Precision Management of Pulmonary Hypertension: From Early Diagnosis to Therapeutic Stratification.

Pulmonary hypertension (PH) is a heterogeneous clinical syndrome in which similar haemodynamic abnormalities may arise from distinct vascular, cardiac, pulmonary, thromboembolic, and molecular mechanisms. This complexity limits the ability of conventional classifications and risk scores to fully capture individual disease trajectories and treatment responses. Artificial intelligence (AI) offers a framework for integrating clinical data, electrocardiography, multimodal imaging, invasive haemodynamics, biomarkers, and multi-omics information across the PH care pathway. This review summarises cur

MIDOS: a novel stochastic model towards a treatment planning system for microsphere dosimetry in liver tumors.

PURPOSE: Transarterial radioembolization (TARE) procedures treat liver tumors by injecting radioactive microspheres into the hepatic artery. Currently, there is a critical need to optimize TARE towards a personalized dosimetry approach. To this aim, we present a novel microsphere dosimetry (MIDOS) stochastic model to estimate the activity delivered to the tumor(s), normal liver, and lung. METHODS: MIDOS incorporates adult male/female liver computational phantoms with the hepatic arterial, hepatic portal venous, and hepatic venous vascular trees. Tumors can be placed in both models at user dis

Mapping cortical disease-burden at individual-level in frontotemporal dementia: implications for clinical care and pharmacological trials.

Imaging studies of FTD typically present group-level statistics between large cohorts of genetically, molecularly or clinically stratified patients. Group-level statistics are indispensable to appraise unifying radiological traits and describe genotype-associated signatures in academic studies. However, in a clinical setting, the primary objective is the meaningful interpretation of imaging data from individual patients to assist diagnostic classification, inform prognosis, and enable the assessment of progressive changes compared to baseline scans. In an attempt to address the pragmatic deman

The use of a three-dimensional imaging model to optimize the operative separation of pygopagus conjoined twins.

INTRODUCTION: The surgical management of conjoined twins has evolved to the point that elective operative separation can safely be performed in the majority. Cross-sectional imaging is invaluable to accurately delineate the site of union and configuration of shared organ systems prior to surgery. The use of three-dimensional modeling also dramatically improves the planning process and assists in intraoperative decision-making. We present a case of pygopagus conjoined twins who underwent successful operative separation at 19 months of age with the aid of such technology. METHODS: A three-dimen

Surgical decision-making in the management of childhood tumors of the CNS disseminated at presentation.

OBJECTIVE It is relatively unusual for pediatric CNS tumors to be disseminated at presentation, and the literature on the clinical features, management, and outcomes of this specific group is scarce. Surgical management in this population is often challenging, particularly in the presence of hydrocephalus. The authors present their recent experience of treating pediatric CNS tumors that were disseminated at presentation, and they compare these lesions with focal tumors. METHODS The authors performed a retrospective review of prospectively collected data on children presenting to a tertiary cen

Neural systems mediating decision-making and response inhibition for social and nonsocial stimuli in autism.

Autism is marked by impairments in social reciprocity and communication, along with restricted, repetitive and stereotyped behaviors. Prior studies have separately investigated social processing and executive function in autism, but little is known about the brain mechanisms of cognitive control for both emotional and nonemotional stimuli. We used functional magnetic resonance imaging to identify differences in neurocircuitry between individuals with high functioning autism (HFA) and neurotypical controls during two versions of a go/no-go task: emotional (fear and happy faces) and nonemotional
