Secondary surgical decompression of the inferior alveolar nerve after overfilling of endodontic sealer into the mandibular canal: Case report and literature review.

The authors report the case of a 43-year-old woman who underwent endodontic treatment of the right second mandibular molar with substantial extrusion of endodontic material into the mandibular canal. The patient presented at the Department of Oral and Maxillofacial Surgery with a persistent total anaesthesia of the lower lip and chin after two months. 2D panoramic view and 3D CT-scan examination highlighted the overfilling into the mandibular canal with a more than 50% stenosis of the canal and a consequently significant compression of the dental pedicle. A surgical decompression of the inferi

Mild diabetes is not a contraindication for surgical decompression in cervical spondylotic myelopathy: results of the AOSpine North America multicenter prospective study (CSM).

BACKGROUND CONTEXT: Cervical spondylotic myelopathy (CSM) is a chronic spinal cord disease and can lead to progressive or stepwise neurologic decline. Several factors may influence this process, including extent of spinal cord compression, duration of symptoms, and medical comorbidities. Diabetes is a systemic disease that can impact multiple organ systems, including the central and peripheral nervous systems. There has been little information regarding the effect of diabetes on patients with coexistent CSM. PURPOSE: To provide empirical data regarding the effect of diabetes on treatment outc

Laminarthrectomy as a surgical approach for decompressing the spinal canal: assessment of preoperative versus postoperative dural sac cross-sectional areal (DSCSA).

INTRODUCTION: Surgery for lumbar spinal stenosis (LSS) is today the most frequently performed procedure in the adult lumbar spine. Long-term benefit of surgery for LSS is well documented both in randomized and in non-randomized trials. In this paper, we present the results from laminarthrectomy as an alternative surgical approach, which have theoretical advantages over other approaches. In this study, we wanted to study the clinical and radiological results of laminarthrectomy. Dural sac cross-sectional areal (DSCSA) is an objective method to quantify the degree of central stenosis in the spin

Microvascular decompression for the treatment of hemifacial spasm. Retrospective study of a consecutive series of 75 operated patients--electrophysiologic and anatomical surgical analysis.

The authors report the results of a series of 75 patients operated on for hemifacial spasm by the same surgeon between October 1981 and January 1997. Microsurgical vascular decompression was performed through a retromastoid approach in contralateral position. The conflicting vessel was identified as the posterior inferior cerebellar artery in 41.3% of the cases, the anterior inferior artery in 38.6%, and the vertebrobasilar complex in 18.7%. Follow-up varied from 1 month to 13 years (with a mean duration of 5 years and 2 months). The outcome was excellent (total improvement) in 57 cases, good

Impact of Redundant Nerve Roots on Lumbar Decompression and Fusion for Lumbar Spinal Stenosis: A Retrospective Controlled Study Based on Surgical Complications.

OBJECTIVE: To investigate the impact of redundant nerve roots (RNRs) on the surgical complications of lumbar decompression and fusion for lumbar spinal stenosis (LSS). METHODS: A retrospective study was conducted on 263 eligible patients with LSS who underwent lumbar decompression and fusion between January 2019 and January 2024. Patients were stratified into 2 cohorts based on the presence or absence of RNRs identified on preoperative sagittal magnetic resonance imaging: the RNRs group (n = 87) and the Non-RNRs group (n = 176). Demographic data, radiological parameters, surgical outcomes, an

Decompressive craniectomy versus craniotomy for patients undergoing surgical evacuation of an acute subdural hematoma: RESCUE-ASDH RCT and cost effectiveness.

BACKGROUND: Traumatic acute subdural haematomas often require surgical evacuation via craniotomy or decompressive craniectomy. Decompressive craniectomy may prevent intracranial hypertension; however, it is unclear whether it is associated with better outcomes. OBJECTIVE: Multicentre, pragmatic, parallel-group randomised trial to compare the clinical and cost-effectiveness of decompressive craniectomy versus craniotomy for evacuation of acute subdural haematomas. DESIGN: International, multicentre, pragmatic, parallel-group randomised trial with additional observational arm. SETTING: Hospit

A staged approach to treating chronic low back and radicular pain: Basivertebral nerve ablation in conjunction with surgical decompression: A case report.

Low back pain is the leading cause of disability worldwide and the course of treatment patients and clinicians pursue to help alleviate it can be difficult at times. When conservative approaches fail, patients may ultimately require surgical intervention. In the presence of predominant axial back pain of discogenic and/or vertebrogenic origin without deformity or spinal instability, fusion surgery is less likely to reliably result in an optimal outcome for the patient. Literature is growing in support of vertebrogenic pain as a primary etiology of axial low back pain, with the degenerated vert

Case Report: Surgical Decompression With Subsequent Selumetinib Treatment Leads to Drastic Clinical Improvement in a Patient With a Large Spinal Plexiform Neurofibroma.

BACKGROUND/AIM: Plexiform neurofibromas are the hallmark of neurofibromatosis type 1, an autosomal dominantly inherited multisystem disorder. Spinal plexiform neurofibromas can particularly cause severe neurological symptoms. Treatment options are limited due to invasive growth, and targeted therapy with selumetinib is only approved for inoperable tumors in children. The aim of this report was to highlight that selumetinib therapy post-surgery provides an alternative strategy for spinal plexiform neurofibroma, providing both an immediate relief of the symptoms and long-term tumor management.

Quantifying the impact of surgical decompression on quality of life and identification of factors associated with outcomes in patients with symptomatic metastatic spinal cord compression.

OBJECTIVE: Metastatic spinal cord compression (MSCC) imposes significant impairment on patient quality of life and often requires immediate surgical intervention. In this study the authors sought to estimate the impact of surgical intervention on patient quality of life in the form of mean quality-adjusted life years (QALY) gained and identify factors associated with positive outcomes. METHODS: The authors performed a retrospective chart review and collected data for patients who had neurological symptoms resulting from radiologically and histologically confirmed MSCC and were treated with su

Transforaminal Endoscopic Decompression of the Lumbar Spine for Stable Isthmic Spondylolisthesis as the Least Invasive Surgical Treatment Using the YESS Surgery Technique.

The first author's series has reported and published his 5- to 10-year results in a preliminary review of endoscopic transforaminal decompression for degenerative and isthmic spondylolisthesis causing sciatica and back pain. This study was initiated due to favorable results in relieving both back and leg pain after a spine surgeon with isthmic spondylolisthesis who wanted to avoid fusion requested consideration for an endoscopic procedure for his own isthmic spondylolisthesis condition. After listening to the first author's podium presentation on selective endoscopic discectomy and foraminal d

Surgical outcome following decompressive craniectomy for poor-grade aneurysmal subarachnoid hemorrhage in patients with associated massive intracerebral or Sylvian hematomas.

BACKGROUND: Patients with poor-grade aneurysmal subarachnoid hemorrhage (SAH) presenting with large intracerebral (ICH) or sylvian hematomas (SylH) have poor outcomes due to the mass effect of significant brain stem compression following mass effect. On the other hand, decompressive craniectomy (DC) can reduce morbidity and mortality in critically ill patients with massive ischemic infarction and severe head injury. However, the role of DC in SAH patients is not fully understood. We investigated the outcome of DC in poor-grade SAH presenting with large ICH or SylH. METHODS: 110 consecutive pa

Morbid Obesity and Diabetes Increase the Risk of Reoperation following Microvascular Decompression: A National Surgical Quality Improvement Program Analysis of 1,303 Patients.

Background  Microvascular decompression (MVD) is the preferred treatment for refractory trigeminal neuralgia (TN) and hemifacial spasm (HFS). MVD provides long-lasting results for these conditions with a relatively low risk of postoperative complications. However, reoperation rates are reported up to 11%, an unacceptably high rate for an elective procedure. We determined what factors may increase the risk of reoperation among patients undergoing MVD for TN or HFS. Methods  Patient data from 2015 to 2020 were obtained from the American College of Surgeons-National Surgical Quality Improvement P
