The influence of patients' decisions on treatment planning in restorative dentistry.

UNLABELLED: As part of treatment planning, options are presented to patients by dentists. An informal discussion takes place involving a cost-benefit analysis and a treatment plan is agreed. Evidence-based dentistry takes into account the best available literature, clinical experience and patient factors to guide the dentist. Dentists exert considerable influence on which treatment modality is selected. This paper focuses on the importance of patient factors which lead to less than ideal, clinically acceptable, treatment plans that nevertheless give patient satisfaction over the long term. Tho…

The preterm child in dentistry. Behavioural aspects and oral health.

BACKGROUND: Advancements in medical care have enabled more children born preterm to survive and develop as healthy individuals alongside their full-term peers. However, a higher frequency of medical health problems, cognitive and behavioural disturbances, including problems in school, has been reported. There is limited knowledge about how this affects preterm children (PT) in dentistry, and few studies have evaluated behavioural problems related to dental treatments and oral health in PT as compared with full-term control children (C). AIMS: The overall objective was to investigate behaviour…

A dental esthetic checklist for treatment planning in esthetic dentistry.

A dental esthetic checklist form has been introduced for use in the diagnostic work-up of a patient accepted for treatment in an Advanced Education in General Dentistry program. The checklist, divided into seven basic criteria, is not a complete list of all possible categories and nuances that an esthetic restorative assessment might include. The correct initial identification of these criteria does not limit the dentist from pursuing more detail; in fact, this checklist is an introductory organizer that invites further exploration. In the authors' experience, the use of this form prompts the …

Retrospective review of voluntary reports of nonsurgical paresthesia in dentistry.

OBJECTIVE: Paresthesia is an adverse event that may be associated with the administration of local anesthetics in dentistry. The purpose of this retrospective study was to analyze cases of paresthesia associated with local anesthetic injection that were voluntarily reported to Ontario"s Professional Liability Program (PLP) from 1999 to 2008 inclusive, to see if the findings were consistent with those from 1973 to 1998 from this same source. MATERIALS AND METHODS: All cases of nonsurgical paresthesia reported from 1999 to 2008 were reviewed; cases involving surgical procedures were excluded. V…

Antibiotic prophylaxis in dentistry and oral surgery: use and misuse.

INTRODUCTION: Antibiotic prophylaxis (AP) still represents a common but often misused procedure in dental practice, thus aggravating the risk for antimicrobial resistance and adverse effects occurrence. AIMS AND METHODS: Our primary objective is to review the available scientific evidence regarding AP in dentistry both among healthy subjects and medically compromised patients. Additionally, the latest available guidelines provided by some of the most authoritative associations are here discussed. RESULTS AND CONCLUSIONS: AP is advisable only in a small percentage of patients where a risk of …

Pharmacologic management of type 1 diabetes: a review for dentistry.

The incidence of diabetes mellitus has increased dramatically in the United States over the last 40 years. In 2006, reported cases of diabetes mellitus increased by 6.0%. The vast majority of these increased cases involve Type 2 diabetes, which is becoming much more common in children, adolescents, and young adults. This article reviews the pharmacological treatments available for Type 1 diabetes, the monitoring necessary for diabetes patients, the regimens of intensive insulin therapy that have replaced conventional therapy, and the complications of insulin therapy. Alternate methods of insul

[Specialties in dentistry. Salivary flow and swallowing in Parkinson's disease].

Parkinson's disease is a slowly progressive and irreversible disorder of the nervous system. Drooling is listed as a secondary symptom of Parkinson's disease. Its cause is insufficiently clear. In the literature 2 possible causes are described: hypersalivation and swallowing abnormalities. These parameters have not been measured in a single study before. This article presents a review of the literature on the subject and describes the design for a future clinical study. The aim of this clinical study is to gain an insight into the cause and the prevalence of drooling in Parkinson's patients. A

Oral candidosis and the therapeutic use of antifungal agents in dentistry.

This paper reviews the current concepts of mycology and candidal infections as they relate to the oral cavity. Proposed classification for the presentation of oral candidosis is outlined as are examples of these topical infections, such as erythematous, pseudomembranous and hyperplastic candidosis, as well as angular chelitis and median rhomboid glossitis. Advertising, commercialism, and professionalism: a history of the ethics of advertising in dentistry.

The authors "read" the historical dental codes prohibiting advertising and the U. S. Supreme Court decision striking down prohibitions against advertising by lawyers in Arizona, and by extension, professional advertising generally. The arguments presented in defense of prohibiting professional advertising and the court's responses to each are presented in detail. The current ADA code is analyzed in this context.

Occupational hazards of dentistry. A review of literature from 1990.

A brief survey of the scientific and clinical literature (1990-present) on occupational hazards in dentistry is presented. The hazards identified are associated with chemical and biological agents. Yet, dentistry is a relatively safe profession. Other adverse health risks arise as new technologies and materials are developed. However, once identified and recognized as a risk, new guidelines, precautions and protocols are rapidly instituted to greatly reduce or even eliminate the occupational hazard.

"The profession of dentistry:" the University of Kentucky's curriculum in professional ethics.

Among the most important learning that occurs in our nation's colleges of dentistry is learning to be a professional. While knowledge, perceptual-motor skills, and problem-solving abilities are basic to becoming a dentist, helping aspiring colleagues to apply their newly developing skills with integrity must be a fundamental concern. Increasingly, we are realizing that the quality of health care depends as significantly on the character of the health professional as it does on the individual's knowledge and skills. Concern for character, virtue, and integrity is the domain of ethics. This pape…

The role of dentistry in patients undergoing bone marrow transplantation.

It is imperative that a comprehensive dental examination should be performed in all patients who are to undergo a bone marrow transplantation. Such assessments should be carried out prior to and after the transplantation procedure. This paper addresses the implications of marrow transplantation and the impact on dental treatment. The side-effects of transplantation chemotherapy, radiation treatment and graft versus host disease and their impact on the oral cavity are also discussed in this paper.
