BACKGROUND: Diabetic peripheral neuropathy (DPN) is commonly observed as a long-term complication in patients with type 2 diabetes mellitus (T2DM). Recent evidence suggests that metabolic disturbances and chronic inflammation may contribute to its development. Lactate dehydrogenase (LDH), a key enzyme in glycolysis, may reflect underlying metabolic stress and inflammation, but its association with DPN remains unclear.
METHODS: In this cross-sectional study, 2,060 patients with T2DM were analyzed to explore the relationship between serum LDH levels and DPN. Logistic regression and restricted c

The effect of diabetes subtypes on neuropathy prevalence and metabolic drivers of this relationship are unknown. Using electronic medical records from 50 diabetes care centers in eight states of India with biothesiometer and complete phenotyping data on 22,348 patients, we determined neuropathy prevalence stratified by India-specific diabetes subtypes and evaluated associations with neuropathy (biothesiometer). Neuropathy prevalence was highest in severe insulin-deficient diabetes (SIDD; 25.5%), followed by insulin-resistant obese diabetes (IROD; 21.8%), mild age-related diabetes (MARD; 18.9%)

Diabetes mellitus is the leading cause of polyneuropathy in the Western world. Diabetic neuropathy is a frequent complication of diabetes and may have great clinical transcendence due to pain and possible ulceration of the lower extremities. It is also a relevant cause of morbidity and mortality in patients with diabetes. Although the cause of polyneuropathy in patients with diabetes is only partially known, it has been associated with chronic hyperglycemia suggesting the possible etiopathogenic implication of advanced glycosylation end products. The strategy of choice in the medical managemen

The lower-extremity vascular injuries and neuropathy are the most salient complications of diabetes which could lead to the poor prognosis, especially for the type II diabetes. The lower extremity vascular injuries and neuropathy usually coexist, yet their correlation in the pathogenesis of lower extremity lesions has received little attention in previous studies. To investigate the correlation between the degree of lower-extremity arterial injuries and lower-extremity neurological functional status in patients with type II diabetes, 32 patients with type II diabetes were examined for the mean

AIMS: Diabetic peripheral neuropathy (DN) is a major complication of type 1 diabetes (T1D), frequently subclinical in youth. Although international pediatric guidelines recommend annual screening, adherence in pediatric care is uncertain. This study aimed to assess DN screening practices in Italian pediatric diabetes centers and identify barriers to implementation.
METHODS: Between December 2024 and May 2025, we conducted a nationwide, cross-sectional survey among Italian centers affiliated with the ISPED Diabetes Study Group. One respondent per center completed a 27-item questionnaire explor

Diabetic neuropathy (DN) represents the main cause of morbidity and mortality among diabetic patients. Clinical data support the conclusion that the severity of DN is related to the frequency and duration of hyperglycemic periods. The presented experimental and clinical evidences propose that changes in cellular function resulting in oxidative stress act as a leading factor in the development and progression of DN. Hyperglycemia- and dyslipidemia-driven oxidative stress is a major contributor, enhanced by advanced glycation end product (AGE) formation and polyol pathway activation. There are s

Disabling neuropathic pain (NeuP) is a common sequel of diabetic peripheral neuropathy (DPN). We aimed to characterise the sensory phenotype of patients with and without NeuP, assess screening tools for NeuP, and relate DPN severity to NeuP. The Pain in Neuropathy Study (PiNS) is an observational cross-sectional multicentre study. A total of 191 patients with DPN underwent neurological examination, quantitative sensory testing, nerve conduction studies, and skin biopsy for intraepidermal nerve fibre density assessment. A set of questionnaires assessed the presence of pain, pain intensity, pain

A nerve biopsy from a patient with type Ia hereditary motor and sensory neuropathy and diabetes mellitus showed hypertrophic changes of atypical appearance. The onion bulbs were composed of a central core of Schwann cells, with or without associated axons, embedded in concentrically arrayed layers of collagen fibrils. These were surrounded either by highly attenuated Schwann cell processes or by fibroblasts. The biopsy showed a severe loss of myelinated axons. It is suggested that it is necessary for the supernumerary Schwann cells of the onion bulbs to be stabilized by associated unmyelinated

INTRODUCTION: In patients with type 2 diabetes, factors beyond hyperglycemia may contribute to the development of sensory neuropathy. This study aimed to determine whether diabetes duration was the strongest predictor of prevalent sensory neuropathy and to compare the diagnostic performance of commonly used clinical screening methods.
MATERIALS AND METHODS: A total of 711 patients with type 2 diabetes from the Budapest metropolitan area were assessed between 2016 and 2022 using routine sensory neuropathy screening tests (128-Hz tuning fork and 10-g monofilament). A brief standardized question
