[Unexplained, subclinical chronically elevated transaminases].

Unexplained, subclinical chronically elevated transaminases is mainly a marker of non-alcoholic fatty liver disease, metabolic syndrome, alcoholism and diabetes, which are very common situations but viral hepatitis and iatrogenic origin must also be considered. Before looking for hepatic or genetic rare diseases, it is worth considering hypertransaminasemia as a clue for muscular disease, particularly in paediatric settings, and creatine phosphokinase is a specific marker. Then, patient history, examination and appropriate biologic requests can permit the identification of less frequent disord

Pharmacological and dietary prophylaxis and treatment of acute actinic esophagitis during mediastinal radiotherapy.

Our purpose was to evaluate a pharmacological and dietary protocol of prophylaxis and treatment of acute actinic esophagitis during mediastinal radiotherapy. This phase II study was conducted on 29 patients affected by cancer not directly involving the esophagus. The irradiated volume included at least 10 cm of esophagus with a median dose of 46 Gy and the incidence of clinical acute esophagitis was scored with RTOG-EORTC tables. During the entire course of radiation therapy all patients were subjected to prophylaxis pharmacological therapy in addition to dietetic rules commonly used. All pati

Aspects on reducing gastrointestinal adverse effects associated with radiotherapy.

Patients receiving cancer therapy are afflicted with a diversity of side effects. Radiotherapy for cancer affecting the head and neck, oesophagus and pelvis is associated with a marked toxicity, specifically encountered as mucosal toxicity. Pain and diarrhoea as well as nausea and vomiting are the most common symptoms, with subsequent problems such as malnutrition and decreased quality of life. These side effects need to be reduced if we are to optimize radiotherapy and to cure patients. Because there is no straightforward way of obviating these side effects, every effort to prevent aggravatio

The epidemiology of cancer.

The epidemiology of cancer has a long history. It led to the discovery of several causes of cancer before techniques for the production of the disease in laboratory animals became available. In recent years, epidemiological studies have contributed to knowledge of cancer in five ways: by demonstrating geographical and temporal variations in incidence, by correlating incidence in different communities with the prevalence of social habits and environmental agents, by comparing the experience of individuals with and without cancer, by intervening to remove suspected agents and observing the resul

Manifestation and occurrence of selective adult-type lactose malabsorption in Finnish teenagers. A follow-up study.

In 1969--1970, a simple random sample of 129 Finnish school-aged children was examined to study selective adult-type lactose malabsorption (SLM) in this age category. SLM was found in 8 children. All subjects were reexamined 5 years later. SLM was reconfirmed in these 8 persons and found in 3 additional subjects who had normal lactose absorption in the first examination. The prevalence of SLM was 9.3%, being 8.5% in the age category 12--15 years and 9.9% in that 16--20 years. Low rise of blood glucose in the lactose tolerance test of the first examination, very low milk consumption, milk intol

Nutrition care practices of primary care providers for weight management in multidisciplinary primary care settings in Ontario, Canada - a qualitative study.

BACKGROUND: Despite the recommended guidelines on addressing diet for the management and prevention of obesity in primary care, the literature highlights that their implementation has been suboptimal. In this paper, we provide an in-depth understanding of current nutrition-related weight management practices of primary care providers (PCPs) working in relatively new multidisciplinary health care settings in Ontario. METHODS: Three types of multidisciplinary primary care settings were included (2 Family Health Teams, 3 Community Health Centres and 1 Nurse Practitioner-Led Clinic). Participants

Intra-role conflict: a result of naive attempts toward professionalization.

In an attempt to become professional, many allied health occupations have focused upon acquiring the traditional attributes associated with a profession while neglecting the professionalization process. As a result, members of allied health occupations that claim professional status may experience the problems of lack of occupational prestige, limited opportunity for vertical mobility, and occupational burn-out. A study was conducted to explore the possible links among level of training program socialization, level of professional identity, and level of intra-role conflict in medical technolog

Diagnostic performance of mini nutritional Assessment-Short form against the global leadership initiative on malnutrition criteria.

In 2018, the Global Leadership Initiative on Malnutrition (GLIM) experts accepted screening for malnutrition with any validated tool. In gerontological dietetics, Mini Nutritional Assessment-Short Form (MNA-SF) remains the most frequently used one. We evaluated the diagnostic performance of MNA-SF against the GLIM criteria in a representative group of older adults (PolSenior2 study population). The complete dataset was available for 5614 participants. We compared two groups of respondents: fulfilling GLIM criteria (GLIM malnutrition, positive screening) and GLIM malnutrition, screening negativ…

Nutritional management in head and neck cancer: United Kingdom National Multidisciplinary Guidelines.

Nutritional support and intervention is an integral component of head and neck cancer management. Patients can be malnourished at presentation, and the majority of patients undergoing treatment for head and neck cancer will need nutritional support. This paper summarises aspects of nutritional considerations for this patient group and provides recommendations for the practising clinician. Recommendations • A specialist dietitian should be part of the multidisciplinary team for treating head and neck cancer patients throughout the continuum of care as frequent dietetic contact has been shown to

Evaluation of change in dietitians' perceived comprehensibility and difficulty of the Patient-Generated Subjective Global Assessment (PG-SGA) after a single training in the use of the instrument.

BACKGROUND: The Patient-Generated Subjective Global Assessment (PG-SGA) is an instrument used to assess malnutrition and its risk factors. Some items of the PG-SGA may be perceived as hard to comprehend or as difficult by healthcare professionals. The present study aimed to determine whether and how dietitians' perceptions of comprehensibility and difficulty of the PG-SGA change after a single training in PG-SGA use. METHODS: In this prospective evaluation study, Dutch PG-SGA-naïve dietitians completed a questionnaire regarding perceived comprehensibility and difficulty of the PG-SGA before (

Transitioning the child fed by gastrostomy into school.

Young children dependent on gastrostomy feeding are examples of special-needs children now eligible for expanded nutrition services in schools through Public Law 99-457. With the implementation of this law, nutritionists have an opportunity to expand their roles as consultants to special education teachers. A team approach involving nutrition, nursing, and special education was used to prepare a gastrostomy-fed preschool child and his family for school. The family of the multihandicapped child had been isolated by the child's feeding problems and viewed the problems as barriers to his entering

Assessment of protein-energy malnutrition in children and adolescents with juvenile rheumatoid arthritis.

The incidence of protein-energy malnutrition (PEM) in juvenile rheumatoid arthritis (JRA) patients referred to a dietitian was compared with PEM in a nonreferred JRA population. One pediatric dietitian evaluated all 28 patients using a standardized, 11-parameter comprehensive nutritional assessment profile. Thirty-six percent (10) were found to have PEM (greater than or equal to 6 of 11 parameters abnormal as defined for this study), 36% (10) had no abnormal parameters, and 28% (8) had one-five abnormal parameters. Seventy percent (7 of 10) of the patients referred to a dietitian had PEM: 17%

Effect of the Ketogenic Diet on the Prophylaxis and Treatment of Diabetes Mellitus: A Review of the Meta-Analyses and Clinical Trials.

The exponentially growing frequency of diagnosing diabetes mellitus means that a verification of the previous dietetic approach to treating the disease seems justified. The simultaneous growth of interest in the ketogenic diet and the development of knowledge in this field have contributed to the increasingly frequent application of the ketogenic diet in diabetes treatment. This paper also deals with that issue; its aim includes an extensive analysis of the influence of the ketogenic diet on the prophylaxis and treatment of diabetes. The paper has been prepared based on a wide, meticulous anal

A new phenotype of aldolase a deficiency in a 14 year-old boy with epilepsy and rhabdomyolysis - case report.

BACKGROUND: Glycogen storage disease type XII is a rare metabolic disease resulting from Aldolase A deficiency that causes muscle glycogen accumulation, with crisis of rhabdomyolysis and hemolytic anemia. In the very few cases described, rhabdomyolysis crises are caused by fever and/or exercise and can accompany acute hemolytic anemia. Although currently there is no therapy available for this disease, the guidelines for the management of other forms of glycogen storage diseases recommend a nutritional therapy in order to avoid hypoglycemia or prevent exercise-induced rhabdomyolysis.
