Cerebellar atrophy after severe traumatic head injury in children.

OBJECT: The purpose of this study was to describe late neuropathological MRI findings in pediatric severe head injury and to explore the relationship between these lesions and cognitive sequelae. METHODS: Thirteen infants with severe head trauma (Glasgow 6) were included in this investigation. Clinical examination, a battery of tests designed to assess neurophysiological status, and MRI investigations of the brain were obtained in periods ranging between 8 and 20 months after the accident. Hemosiderin deposits, encephalomalacia, and cerebellar atrophy were the most frequent traumatic sequelae

Vascular compression of the trigeminal nerve in asymptomatic individuals: a voxel-wise analysis of axial and radial diffusivity.

BACKGROUND: Neurovascular compression (NVC) of the trigeminal nerve is associated with trigeminal neuralgia (TN), but also occurs in many asymptomatic individuals. The purpose of this study was to investigate the possible microstructural tissue changes of trigeminal nerves (TGN) in asymptomatic individuals with NVC by using axial diffusivity (AD) and radial diffusivity (RD) of MR imaging and to discuss its underlying mechanisms. METHODS: Twenty asymptomatic individuals with unilateral NVC and 18 healthy controls (HCs) were divided into three groups (compressed, uncompressed side in asymptomat

Characterizing changes in network connectivity following chronic head trauma in special forces military personnel: a combined resting-fMRI and DTI study.

BACKGROUND: Soldiers are exposed to significant repetitive head trauma, which may disrupt functional and structural brain connectivity patterns. PURPOSE/HYPOTHESIS: Integrate resting-state functional MRI (rs-fMRI) and diffusion tensor imaging (DTI) to characterize changes in connectivity biomarkers within Canadian Special Operations Forces (CANSOF), hypothesizing that alterations in architectural organization of cortical hubs may follow chronic repetitive head trauma. METHODS: Fifteen CANSOFs with a history of chronic exposure to sub-concussive head trauma and concussive injuries (1.9 ± 2.0

Persistent facial myoclonus: a negative prognostic sign in patients with severe brain injury.

The determination of long-term prognosis is one of the most difficult tasks in the management of persons with brain injury (BI). Initial Glasgow Score, duration of coma, brain stem reflexes, and pupillary responses are all clinical findings that have been related to outcome in BI. We describe another easily observable clinical finding, persistent facial myoclonus (PFM), which may provide additional information on prognosis after severe BI. We retrospectively reviewed 295 consecutive admissions to an inpatient brain injury rehabilitation program. Data extracted included mechanism of injury, pat

The neuropathology of the vegetative state after an acute brain insult.

The vegetative state is often described clinically as loss of function of the cortex while the function of the brainstem is preserved. In an attempt to define the structural basis of the vegetative state we have undertaken a detailed neuropathological study of the brains of 49 patients who remained vegetative until death, 1 month to 8 years after an acute brain insult. Of these, 35 had sustained a blunt head injury and 14 some type of acute non-traumatic brain damage. In the traumatic cases the commonest structural abnormalities identified were grades 2 and 3 diffuse axonal injury (25 cases, 7

A Novel Brainstem Hemorrhage Model by Autologous Blood Infusion in Rat: White Matter Injury, Magnetic Resonance Imaging, and Neurobehavioral Features.

BACKGROUND: Primary brainstem hemorrhage (BSH) has the highest mortality and morbidity as a subtype of intracerebral hemorrhage. A major limitation of BSH research is the lack of a corresponding animal model. The purpose of this study was to establish a novel rat model of BSH and to characterize the resulting brain injury, especially focusing on white matter injury. METHODS: BSH was produced by stereotactically injecting autologous whole blood into the pons. Time course of hematoma resolution was observed by 7-T magnetic resonance imaging. White matter injury was evaluated in detail by multip

Unraveling the link between brain injury and enhanced artistic skills.

Traumatic brain injury (TBI), whether from a single incident or repeated trauma, is a heterogeneous neurological condition known for its cognitive, motor, and behavioral effects. However, its potential to influence or even enhance artistic creativity remains a lesser-studied phenomenon. This review explores evidence suggesting that TBI can give rise to novel or intensified artistic abilities, drawing on case reports and neuroimaging studies. We examine brain regions commonly involved in creative processes - including the prefrontal, parietal, temporal, and occipital cortices, as well as subcor

Effective radiofrequency thalamotomy for posttraumatic Holmes tremor: illustrative case.

BACKGROUND: Holmes tremor (HT) is a symptomatic tremor that develops weeks to years after stroke, traumatic brain injury, or brain tumor and is characterized by low-frequency, high-amplitude movements at rest, during posture, and with intentional movement. It is thought to result from lesions involving the dopaminergic nigrostriatal system, Guillain-Mollaret triangle, or dentato-rubro-thalamic tract. Because HT is often drug resistant, stereotactic procedures targeting the thalamus are considered. While deep brain stimulation for HT shows long-term efficacy, evidence for radiofrequency thalamo

Early Postnatal Neuroinflammation Produces Key Features of Diffuse Brain White Matter Injury in Rats.

BACKGROUND: Perinatal infection is a major risk factor for diffuse white matter injury (dWMI), which remains the most common form of neurological disability among very preterm infants. The disease primarily targets oligodendrocytes (OL) lineage cells in the white matter but also involves injury and/or dysmaturation of neurons of the gray matter. This study aimed to investigate whether neuroinflammation preferentially affects the cellular compositions of the white matter or gray matter. METHOD: Neuroinflammation was initiated by intracerebral administration of lipopolysaccharide (LPS) to rat p

Full Recovery From Cocaine-Induced Toxic Leukoencephalopathy: Emphasizing the Role of Neuroinflammation and Brain Edema.

Toxic leukoencephalopathy (TL) is characterized by white matter disease on magnetic resonance imaging (MRI) and evidence of exposure to a neurotoxic agent. We describe a case of cocaine-induced TL in which extensive white matter disease did not preclude full recovery. A 57-year-old man with substance abuse disorder presented with a 5-day history of strange behavior. On admission, he was alert but had difficulty concentrating, psychomotor retardation, and diffuse hyperreflexia. Brain MRI revealed confluent subcortical white matter hyperintensities with restricted diffusion in some but not in ot

Early neuroimaging and ultrastructural correlates of injury outcome after neonatal hypoxic-ischaemia.

Hypoxic ischaemia encephalopathy is the major cause of brain injury in new-borns. However, to date, useful biomarkers which may be used to early predict neurodevelopmental impairment for proper commencement of hypothermia therapy is still lacking. This study aimed to determine whether the early neuroimaging characteristics and ultrastructural correlates were associated with different injury progressions and brain damage severity outcomes after neonatal hypoxic ischaemia. Longitudinal 7 T MRI was performed within 6 h, 24 h and 7 days after hypoxic ischaemia in rat pups. The brain damage outcome

Alzheimer's pathology in human temporal cortex surgically excised after severe brain injury.

Traumatic brain injury (TBI) is a risk factor for the development of Alzheimer's disease (AD). This immunohistochemical study determined the extent of AD-related changes in temporal cortex resected from individuals treated surgically for severe TBI. Antisera generated against Abeta species (total Abeta, Abeta(1-42), and Abeta(1-40)), the C-terminal of the Abeta precursor protein (APP), apolipoprotein E (apoE), and markers of neuron structure and degeneration (tau, ubiquitin, alpha-, beta-, and gamma-synuclein) were used to examine the extent of Abeta plaque deposition and neurodegenerative cha

Diffusion tensor imaging at 3T for diagnosing root avulsion in adults with acute traumatic brachial plexus injuries.

BACKGROUND: Root avulsion in patients with traumatic brachial plexus injury (tBPI) are common and MRI is used to help identify patients who need urgent reconstruction. Diffusion tensor MRI (DTI) generates proxy measures of nerve 'health' which are sensitive to myelination, axon diameter, fibre density and organisation. This prospective multicentre pilot study assessed the utility of DTI for detecting root avulsion in adults with acute traumatic brachial plexus injury. METHODS: Patients underwent DTI at 3 Tesla. Fractional anisotropy (FA) and radial diffusivity (RD) were extracted from spinal

Moderate and severe traumatic brain injury: abbreviated injury scale scoring and coding of traumatic axonal injury from early MRI.

BACKGROUND: In this large prospective study of patients with moderate and severe traumatic brain injury (TBI) examined with early magnetic resonance imaging (MRI), we aimed to explore the differences in the occurrence and degree of traumatic axonal injury (TAI) reported on early MRI compared with (1) the original Abbreviated Injury Scale (AIS)-TAI coding and (2) a pragmatic AIS-TAI coding approach. The latter approach also allowed the assignment of TAI codes to patients with short periods of coma, sedated patients and patients with concurrent mass lesions, which are exclusion criteria in the o

Translational evidence for two distinct patterns of neuroaxonal injury in sepsis: a longitudinal, prospective translational study.

BACKGROUND: Brain homeostasis deteriorates in sepsis, giving rise to a mostly reversible sepsis-associated encephalopathy (SAE). Some survivors experience chronic cognitive dysfunction thought to be caused by permanent brain injury. In this study, we investigated neuroaxonal pathology in sepsis. METHODS: We conducted a longitudinal, prospective translational study involving (1) experimental sepsis in an animal model; (2) postmortem studies of brain from patients with sepsis; and (3) a prospective, longitudinal human sepsis cohort study at university laboratory and intensive care units (ICUs).

MRI detects peripheral nerve and adjacent muscle pathology in non-systemic vasculitic neuropathy (NSVN).

BACKGROUND: Diagnosis and disease monitoring of non-systemic vasculitic neuropathy (NSVN) are based on electrophysiological and clinical measures. However, these methods are insensitive to detect subtle differences of axonal injury. We here assessed the utility of a multiparametric MRI protocol to quantify axonal injury and neurogenic muscle damage in NSVN. METHODS: Ten NSVN patients and ten age-matched controls were investigated in this single-center prospective study. All participants were assessed by diffusion tensor imaging (DTI) of the tibial nerve and multiecho Dixon MRI of soleus and g
