Four anastomotic techniques following transanal total mesorectal excision (TaTME).

Transanal total mesorectal excision (TaTME) is a novel approach pioneered to tackle the challenges posed by difficult pelvic dissections in rectal cancer and the restrictions in angulation of currently available laparoscopic staplers. To date, four techniques can be employed in order to create the colorectal/coloanal anastomosis following TaTME. We present a technical note describing these techniques and discuss the risks and benefits of each.

Long-term follow-up of a novel anastomotic device in a canine model.

Construction of intestinal anastomosis is a fundamental general surgery skill. New constraints in creating safe, effective anastomoses are faced, however, even as minimally invasive surgery techniques continue to gain popular and scientific support. We present our experience in developing and testing a novel anastomotic device (AD) constructed of a shape memory metal, with long-term follow-up in a canine model. This device has the potential for both laparoscopic and endoscopic delivery because of its unique design and adaptable deployment system. Eight canines had gastroduodenal and jejunojeju

Biomechanical comparison of four technique for pelvic flexure enterotomy closure in horses.

OBJECTIVE: To compare 4 techniques for pelvic flexure enterotomy closure in horses. STUDY DESIGN: Ex-vivo study. SAMPLE POPULATION: Cadaveric ascending colon specimens (n = 48 horses). METHODS: Pelvic flexure enterotomies of different lengths (5 cm, 10 cm) were performed and closed with 1 of 4 techniques: handsewn 2 layer (HS2); handsewn 1 layer (HS1); skin staples (SKS); or TA90 stapling device (TA90). Time to close each enterotomy, bursting pressure, luminal reduction, and cost were calculated and compared. RESULTS: HS2 was significantly more time consuming to perform in the 5 cm group w

Sigmoid volvulus: is it a possible complication after stapled transanal rectal resection (STARR)?

OBJECTIVES: Alveolar haemorrhage (AH) can be a mild or life-threatening manifestation of antineutrophil cytoplasm antibody (ANCA)-associated vasculitides (AAV), but its prognostic impact and specific characteristics remain controversial. Our objective was to determine the prognostic value of AH in this context. METHODS: AH episodes that occurred, between 1991 and 2010, in AAV patients entered in the FVSG database were retrospectively analysed. Data on AH characteristics and outcome measures were collected on a specific form. RESULTS: Among the 80 cases analysed, AAV were 61.25% granulomatosi

Comparative study of jejunal and colonic alterations between gastrojejunoduodenal and gastrocoloduodenal interposition in dogs.

BACKGROUND: The intestinal interpositions are important in various operations of the digestive tract. However, they are capable to produce serious immediate and late complications. AIM: To compare clinical and surgical outcomes, as well as intestinal pathological findings, in dogs submitted to jejunum and colon interposition. METHODS: Forty male mongreal dogs were immunized and dewormed, between 36 and 72 months old and weighting from 6 to 16 kg divided into two groups of 20 (G1=gastrojejunoduodenal and G2=gastrocoloduodenal). They were anesthetized and submitted to laparotomy with jejunum a

Delayed primary anastomosis for management of long-gap esophageal atresia: a meta-analysis of complications and long-term outcome.

PURPOSE: Management of newborns with long-gap esophageal atresia (LGEA) remains a challenge for pediatric surgeons. Since spontaneous growth of the esophageal segments occurs without mechanical stretching, initial gastrostomy followed by delayed primary anastomosis (DPA) comprises treatment in most LGEA patients. This meta-analysis aimed to investigate the complications and long-term outcome in patients with LGEA managed by DPA. METHODS: A systematic literature search was conducted for relevant articles published between 1981 and 2011, and a meta-analysis of complications and long-term outcom

Postoperative complications after procedure for prolapsed hemorrhoids (PPH) and stapled transanal rectal resection (STARR) procedures.

Procedure for prolapsing hemorrhoids (PPH) and stapled transanal rectal resection for obstructed defecation (STARR) carry low postoperative pain, but may be followed by unusual and severe postoperative complications. This review deals with the pathogenesis, prevention and treatment of adverse events that may occasionally be life threatening. PPH and STARR carry the expected morbidity following anorectal surgery, such as bleeding, strictures and fecal incontinence. Complications that are particular to these stapled procedures are rectovaginal fistula, chronic proctalgia, total rectal obliterati

Stapled transanal rectal mucosectomy ten years after.

Stapled mucosectomy (SM) was first proposed for the management of patients with rectal internal mucosal prolapse and obstructed defecation, but gained popularity worldwide for the treatment of hemorrhoids. The present review highlights the advantages and disadvantages of the operation. SM tends to decrease postoperative pain and shortens convalescence after hemorrhoid surgery, but may be followed by severe complications, e.g. rectal obliteration and pelvic sepsis requiring a diverting stoma, more frequently than after standard hemorrhoidectomy. Moreover it carries a higher recurrence rate in t

[Modified procedure for prolapse and hemorrhoids to prevent and treat postoperative complications].

OBJECTIVE: To investigate the complications of procedure for prolapse and hemorrhoids (PPH) and their management. METHOD: The clinical data of 147 cases of severe hemorrhoids treated by modified PPH were analyzed in view of the operation method and postoperative complications. RESULT: All the prolapsed hemorrhoids recovered immediately after the operation. The main complications on the first postoperative day were abdominal distention caused by dysuria, pain, and constant sense of defecation urgency, which lasted for 1 to 16 days. Thirty-six cases had intermittent hemafecia 1 to 12 days afte

Endorectal flap advancement repair is an effective treatment for selected patients with anorectal fistulas.

Although the majority of anorectal fistulas may be treated with fistulotomy, patients with higher-lying fistulas or women with anterior fistulas in whom simple fistulotomy may threaten continence present a management dilemma. Endorectal flap advancement repair is one potential solution. Chart review was performed of consecutive patients receiving endorectal advancement flaps during a 5-year period (1999-2003). Twenty-nine patients received endorectal advancement flap repair. In 21 (72%) patients, an etiology of the fistula could not be clearly identified, and they were considered to be cryptog

The role of endoscopy in early postoperative haemorrhage.

Diagnostic as well as therapeutic endoscopy has a decisive role in management of early postoperative haemorrhage. Endoscopy combines easy access to the upper and lower gastrointestinal tract and application of an array of interventional tools. In near future, even the small bowel will be accessible for diagnostic and therapeutic measures due to the advent of double-balloon enteroscopy. Thus, the endoscopist increasingly replaces the surgeon for diagnosis and therapy of postsurgical bleeding. Published data on frequency and aetiology of postoperative haemorrhage are scarce and mainly casuistic.

[The use of fibrin glue in abdominal surgery].

The data on use of fibrin glue (FG) in 215 patients operated on the abdominal organs, employing laser surgical techniques, are presented. In 132 patients, the complicated forms of gastric and duodenal ulcer disease, in 25--gastric cancer, in 24--colonic tumors, in 26--injuries to the liver were revealed. Considerable reduction in incidence of early postoperative complications as compared with that in patients operated on with the use of conventional techniques was noted.

Transcutaneous sacral nerve stimulation for intraoperative verification of internal anal sphincter innervation.

BACKGROUND: The current standard for pelvic intraoperative neuromonitoring (pIONM) is based on intermittent direct nerve stimulation. This study investigated the potential use of transcutaneous sacral nerve stimulation for non-invasive verification of pelvic autonomic nerves. METHODS: A consecutive series of six pigs underwent low anterior rectal resection. For transcutaneous sacral nerve stimulation, an array of ten electrodes (cathodes) was placed over the sacral foramina (S2 to S4). Anodes were applied on the back, right and left thigh, lower abdomen, and intra-anally. Stimulation using th

Successful surgical management for duodenum obstruction in a 66 year-old woman previously undiagnosed intestinal malrotation.

Intestinal malrotation is a congenital abnormality and is rarely seen in the adulthood. Most adult cases would be classified to the non-rotation type with Ladd's band and Ladd procedure is the treatment of choice. A 66 year-old woman admitted to our hospital due to duodenum obstruction. Several tests revealed that she had intestinal malrotation previously undiagnosed. Operative findings showed the fusion of duodenum with jejunum by the incomplete Treitz ligament. There was no Ladd's band and the right colon was unfixed. Dissection of the fusion completely released her symptom and she discharge
