[Diverticulitis appendicis vermiformis--a case review and literature overview].

BACKGROUND: Diverticulosis and diverticulitis of appendix vermiformis is a rare diagnosis with incidency of 0.004-2%. Because of intermittent pain and atypical problems patients with diverticulitis are surgically intervened later as patients with simplex appendicitis. Complications in regard to perforation of appendix are more frequent in 27% in diverticulitis and mortality of consequent peritonitis is 30 times higher than in simplex appendicitis. PATIENTS: Authors present 0.1% incidency of diverticulitis of appendix in the group of 1496 patients with appendicitis. The case involved a man and…

Caecal diverticulitis presenting as acute appendicitis: a case report.

Solitary caecal diverticulum is an uncommon entity and therefore difficult to diagnose except at surgery. Caecal diverticulitis is an infrequent cause of acute abdomen and usually presents in a manner similar to acute appendicitis. It is extremely difficult to differentiate it preoperative from acute appendicitis and such distinction is usually made in the operating room. The optimal management of this clinical condition is still controversial, ranging from conservative treatment with antibiotics to aggressive surgical resections. We report a case of a 61 year old Caucasian who presented with a…

Benefits of laparoscopic peritoneal lavage for complicated sigmoid diverticulitis.

BACKGROUND: The traditional therapy for perforated sigmoid diverticulitis with peritonitis is emergency colectomy usually with colostomy. We report laparoscopic exploration with peritoneal lavage as an alternative in seven patients who required emergency surgery for diverticulitis. METHODS: Six patients presented with diffuse peritonitis and one with a failure of percutaneous therapy. All patients were explored laparoscopically and the peritoneal cavity was lavaged with saline in addition to receiving intravenous antibiotics. Patient demographics, clinical response, length of stay, and compli…

Acute colonic diverticulitis: a systematic review of diagnostic accuracy.

OBJECTIVE: To appraise the literature on the diagnosis of acute colonic diverticulitis by ultrasound (US), computed tomography (CT), barium enema (BE) and magnetic resonance imaging (MRI). METHOD: The databases of Pub Med, the Cochrane Library and EMBASE were searched for articles on the diagnosis of diverticulitis published up to November 2005. Studies where US, CT, BE, or MRI were compared with a reference standard on consecutive or randomly selected patients were included. Three examiners independently read the articles according to a prespecified protocol. In case of disagreement consensu…

[Vesico-sigmoid fistula secondary to diverticulitis: a case report].

Vesico-sigmoid fistula is a rare complication of colic diverticular disease. It develops when the bladder sticks to an inflammatory colon making of a communication between the bladder and the digestive segment, usually the sigmoid. Liquid usually passes from the colon to the bladder because of the existing pressure gradient. Hence, urinary symptoms are the most frequent. Surgery is the treatment of choice. The present case is about a 72-year-old man who had a total hematuria for 3 months. The diagnosis of a vesico-sigmoid fistula secondary to colic diverticulitis was established by cystoscopy,…

A rare manifestation of perforated diverticulitis: parastomal subcutaneous abscess.

Perforation is a serious complication of diverticular disease. The sigmoid is the main affected anatomic site of perforated diverticulitis and sigmoid resection followed either by Hartmann procedure or primary anastomosis are the standard surgical approaches. Surgery, however, does not cure diverticular disease. About 50% of patients have residual diverticula. The morbidity of residual diverticula appears to be low [1]. However, adequate follow-up studies on recurrent diverticulitis after surgery are lacking. We report a rare case of recurrent perforated diverticulitis in the subcutaneous tiss…

Determinants of recurrence after sigmoid resection for uncomplicated diverticulitis.

PURPOSE: This study aimed to evaluate the impact of surgery-associated variables on recurrence rates after sigmoid resection for diverticulitis. METHODS: Patients who underwent elective sigmoid resection for uncomplicated diverticulitis between 1992 and 2000 at two tertiary referral centers were followed up for recurrent disease as the primary end point. Recurrence after surgery was defined as left lower quadrant pain, fever, and leukocytosis, with consistent CT and/or contrast enema findings on admission and after six weeks. A logistic regression of the following variables was undertaken: pa…

[Sigmoid diverticulitis. Emergency intervention in abscess, hemorrhage and stenosis].

Interventional techniques in treatment of complicated diverticulitis gain more and more importance. In particular abscesses and bleeding are treated successfully. In case of diverticular abscess (Hinchey classification stage I and II) percutaneous drainages are placed sonographically or CT guided. Interventional drainage offers the possibility of elective one-stage surgical treatment of diverticulitis with significant reduce of mortality and morbidity rates. Diverticular bleeding is usually diagnosed and treated endoscopically. Only if endoscopy is not able to manage bleeding, angiography or n…

[Prospective national study of complicated diverticulitis in Great Britain].

Three hundred cases of complicated diverticular disease were entered into a national audit organised by the Surgical Research Society from 30 hospitals between 1985 and 1988. Admission complications included acute phlegmon (n = 104), pericolic abscess (n = 34), purulent peritonitis (n = 40), large bowel obstruction (n = 31), faecal peritonitis (n = 23), pericolic abscess complicated by fistula (n = 28) and lower GI bleeding (n = 40). The overall mortality was only 11% (4% acute phlegmon, 27% purulent peritonitis, 12% pericolic abscess, 48% faecal peritonitis, 6% large bowel obstruction, 2.5% b

Colonic diverticulitis. Recognizing and managing its presentations and complications.

Diverticulitis usually manifests as pain of abrupt onset in the lower left quadrant. Complications may occur with or without an acute attack. Plain abdominal films are crucial for initial workup and follow-up. Endoscopic examination is often indicated, but barium enema study should usually be avoided during an acute attack. Computed tomography offers the best means of determining extracolonic extension of diverticulitis. Therapy is usually medical and consists of "resting" the bowel, administering antibiotics to resolve infection, and preventing or minimizing complications. Surgery is reserved…

Radiographic evaluation of spastic colon disease, diverticulosis, and diverticulitis.

Spastic colon disease is a syndrome consisting of abdominal pain and abnormal bowel function associated with thickening of the circular muscle layer of the colon. The condition may be related to changes in intraluminal pressure associated with a lifelong low residue diet. Radiologic features include thickening, crowding, irregularity, and distortion of the interhaustral folds of the sigmoid colon, which in the past have been incorrectly considered to be manifestations of acute diverticulitis. Spastic colon disease leads to diverticulosis which develops because of mucosal herniations through th…

Acute Pancreatitis Secondary to Duodenal Diverticulitis: A Case Report.

Acute pancreatitis is an inflammatory condition of the pancreas that can result in significant clinical complications. Determining the underlying etiology is essential, as management and recurrence risk depend on identifying the cause. When routine evaluation fails to reveal a clear etiology, less common causes should be considered. We present the case of a 55-year-old woman with acute pancreatitis secondary to duodenal diverticulitis. This rare condition can lead to pancreatitis through local inflammation or obstruction of the pancreatic duct. This case highlights the importance of advanced i…

An Unusual Presentation of Jejunal Diverticulitis Mimicking Acute Cholecystitis.

Jejunal diverticulosis is a rare entity and jejunal diverticulitis is even rarer. Here we review a patient who presented with right upper quadrant pain and elevated white blood cell count, mimicking acute cholecystitis, as well as thrombocytopenia (21x10^3/mcl). The patient was managed non-operatively with antibiotics and had improvement in symptoms and thrombocytopenia. Our study highlights the importance of maintaining jejunal diverticulitis in the differential for right upper quadrant pain, and its association with severe thrombocytopenia.

Outcomes Following Robotic Surgery for Complicated and Uncomplicated Diverticulitis.

BackgroundLaparoscopic colectomy is standard for uncomplicated diverticulitis (UD) but has higher conversion and morbidity rates in complicated diverticulitis (CD). Robotic colectomy (RC) is increasingly used for both UD and CD. This study compared outcomes of RC for CD and UD and evaluated factors contributing to adverse outcomes.MethodsUsing 2014-2021 NSQIP data, we identified patients with CD or UD who underwent RC. The primary endpoint was textbook outcome (TO), defined as absence of prolonged length of stay (LOS), major morbidity, readmission, anastomotic leak, or mortality within 30 days…

Laparoscopic Versus Robotic Elective Sigmoid Resection for Complicated Diverticulitis.

OBJECTIVE: Minimally invasive surgical techniques for colorectal surgery have continued to grow in prevalence with robotic surgery potentially providing advantages in complex pelvic operations. We sought to examine the outcomes of laparoscopic versus robotic elective sigmoid colon resection for complicated diverticulitis. METHODS: We performed a retrospective review of patients at an academic tertiary care center from 2018-2023 who underwent elective minimally invasive sigmoid colon resections for complicated diverticulitis. Multiple regression analysis was performed with primary outcomes bei…
