BACKGROUND: Traditionally, treatment of acute diverticulitis has mostly been based on inpatient care. The question arises whether these patients can be treated on an outpatient basis as the admissions for diverticular disease have been shown to be increasing every year. We studied whether outpatient treatment of acute uncomplicated diverticulitis is feasible and safe, and which patients could benefit from outpatient care.
MATERIALS AND METHODS: A retrospective cohort study was carried out in two teaching hospitals using hospital registry codes for diverticulitis. All patients diagnosed with a

Pneumaturia is sometimes the first symptom of colovesical fistula. Colovesical fistula presents a difficult problem with complicated diagnosis and therapy. Most common are fistulas between the colon (sigmoid) and bladder, which results from sigmoiditis and diverticulitis. The paper reports the case of a 61-year-old man with diagnosis of diverticulitis and symptom of pneumaturia in duration of 3 months. The operation was successfully performed in cooperation with a surgeon and an urologist.

Diverticulosis is an out-pocketing of the bowel wall that can affect the small bowel through the large bowel. Small bowel diverticulosis is rare and not as common as colonic diverticulosis, which is an important diagnosis for hospitalizations. Moreover, jejunal diverticulosis is rare among cases of small bowel diverticulosis. Jejunal diverticulitis is one of the complications of jejunal diverticulosis that can be conservatively managed with antibiotics instead of surgery. We report a case of a 41-year-old African American man who presented with vague epigastric pain and was diagnosed with adhe

INTRODUCTION: Hepatic abscesses have been reported as a rare complication of diverticulitis of the bowel. This complication is recognized more commonly at the time of the diagnosis of diverticulitis, or ruptured diverticula, but also can be diagnosed prior to surgery, or postoperatively.
CASE PRESENTATION: This report describes a man who developed an hepatic abscess within a simple hepatic cyst, two months after operation for ruptured diverticula of the sigmoid colon. The abscess was drained surgically and the patient made a complete recovery.
CONCLUSION: The development of an hepatic absces

Colonic diverticula are protrusions of the mucosa through the outer muscular layers, which are usually abnormally thickened, to form narrow necked pouches. Diverticular disease of the colon covers a wide clinical spectrum: from an incidental finding to symptomatic uncomplicated disease to diverticulitis. A quarter of patients with diverticulitis will develop potentially life-threatening complications including perforation, fistulae, obstruction or stricture. In Western countries diverticular disease predominantly affects the left colon, its prevalence increases with age and its causation has b

Pylephlebitis of the inferior mesenteric vein is an uncommon complication of sigmoid diverticulitis. A 46-year-old man presented with abdominal pain and features of sepsis. Blood cultures confirmed Bacteroides fragilis bacteraemia, and computed tomography imaging confirmed diverticulitis of the sigmoid colon, complicated by thrombosis of the inferior mesenteric vein. He was managed successfully with empiric antibiotic cover and anticoagulation, before transitioning to ongoing warfarin therapy long term. Treatment for pylephlebitis requires broad-spectrum antibiotic therapy. Anticoagulation is

BACKGROUND: Surgical treatment of complicated colonic diverticular disease is still debatable. The aim of this prospective study was to evaluate the outcome of laparoscopic sigmoid colectomy in patients with diverticulitis. Patients offered laparoscopic surgery presented with acute complicated diverticulitis (Hinchey type I, II, III), chronically recurrent diverticulitis, bleeding, or sigmoid stenosis caused by chronic diverticulitis.
METHOD: All patients who underwent laparoscopic colectomy within a 12-year period were prospectively entered into a database registry. One-stage laparoscopic re

Following conservative treatment of acute colonic diverticulitis at least one fourth of the patients experiences a further attack. The complication rate rises up to 60% at the recurrence. Therefore, colon resection is indicated at/following the second attack. For male patients below 50 years of age and with severe first attack, surgery is recommended already at/following the first attack. In the absence of diffuse fecal peritonitis, the one-stage colon resection with primary anastomosis is widely accepted, now. Percutaneous drainage of a peridiverticular or paracolic abscess is hazardous (succ

Chronic abdominal pain is often a diagnostic dilemma. We present a 59-year-old female with chronic generalized colicky abdominal and altered bowel habits. She was investigated with colonoscopy and CT abdomen. Patient has a history of recurrent diverticulitis and insertion laparoscopic adjustable gastric band. The colonoscopy revealed a tubular foreign body and diverticular disease. The tubular structure was confirmed to be gastric band tubing on CT abdomen, also showing simultaneous gastric and colonic erosions. She denies any gastric band port related infection or previous issues with gastric

During the last six years we have performed laparoscopy in 80 women older than 60 years of age. In 22 of these, the barium enema showed pathological colonic conditions (mostly diverticulosis or diverticulitis). Despite these findings laparoscopy was used as an additional and reliable diagnostic aid to exclude concomitant gynecological diseases. Of these 22 women, 16 had normal internal genital organs so that exploratory laparotomy could be avoided. In view of the high postoperative mortality in this age group this is regarded as a considerable advantage. In three women an additional gynecologi

BACKGROUND: Colovesical fistula (CVF) is an uncommon but serious complication most often linked to diverticular disease of the colon. Its diagnosis can be at times challenging as the symptoms are nonspecific, and timely intervention is crucial to avoid morbidity.
CASE SUMMARY: A 65-year-old Indian male was admitted with right lower limb cellulitis and was in septic shock. Despite initial management, he remained oliguric with persistent loose stools. On post-fasciotomy day 4, fecaluria was noted, prompting suspicion of a bowel-urinary tract fistula. Microscopic urine examination confirmed feca

PURPOSE: In acute diverticulitis accurate diagnosis and staging are mandatory to decide on the treatment of the patient. The impact of computed tomography (CT) on the treatment of acute diverticulitis will be discussed.
MATERIAL AND METHOD: CT is performed after distension of the distal colon by means of positive,water-soluble contrast media to depict intestinal perforation or penetration. Then intravenous contrast material is administered and spiral scanning is repeated to judge enhancement patterns of the abdominal structures/organs especially of the intestinal wall and to diagnose abscess

Today no secure consensus exists about the best treatment of complicated diverticulitis. The classic surgical procedures are associated to a high immediate and delayed morbidity. In the last few years several more conservative techniques have been suggested to allow a later elective resection. Laparoscopic exploration, peritoneal lavage, and drain of the abdominal cavity followed by an elective sigmoid laparoscopic resection is a new minimal invasive approach. This approach has been applied in our unit to treat four patients. All patients had an acute abdomen due to complicated diverticulitis

AIM: Outcomes after resident involvement in emergent colectomies have rarely been studied. The aim of this study was to analyse the outcomes of laparoscopic sigmoidectomy for Hinchey III diverticulitis performed by residents.
METHOD: This study was a retrospective analysis of patients undergoing laparoscopic sigmoidectomy for diverticulitis. The sample was divided into two groups: patients operated on by a supervised resident (SR) or a senior surgeon (SS). Supervising surgeons and SSs could be general surgeons (GSs) or colorectal surgeons (CSs). A SR was considered the first surgeon if he/she

PURPOSE: The German Classification of Diverticular Disease was introduced a few years ago. The aim of this study was to determine whether Classification of Diverticular Disease enables an exact stratification of different types of diverticular disease in terms of course and treatment.
METHODS: This was a prospective, bicentric observational trial. Patients aged ≥ 18 years with diverticular disease were prospectively included. The primary endpoint was the rate of recurrence within 2 year follow-up. Secondary outcome measures were Gastrointestinal Quality of Life Index, Quality of life measured
