Living donor liver transplantation in transplant oncology: A disease-specific framework for access, timing, and donor risk.

Living donor liver transplantation (LDLT) should not only be evaluated as an alternative graft source in transplant oncology; it should also be evaluated as a disease-specific intervention on access and oncologic time. By removing dependence on deceased-donor availability, LDLT can convert an uncertain wait into a planned operation-but that temporal advantage is not biologically neutral, and its consequences differ sharply by tumor type. In hepatocellular carcinoma, an abbreviated wait can reduce waitlist dropout but may also shorten the interval during which aggressive biology would otherwise

Automated AI for real-time sperm selection in ICSI: reducing variability and studying the role of sperm in embryo development.

BACKGROUND: The application of Artificial Intelligence (AI) to sperm selection during Intracytoplasmic Sperm Injection (ICSI) procedures represents one of the most innovative advances in assisted reproductive technology (ART). Traditional sperm selection relies heavily on the subjective assessment of embryologists, which can lead to variability in outcomes. This study aimed to evaluate the performance of an AI-based software, Sperm ID (SiD™) v.1.0, for sperm selection during ICSI and to compare its outcomes with those obtained by experienced embryologists. Additionally, the study assessed the

Influence of HIV Status on Organ Procurement and Allocation: A Discrete Choice Experiment of Australian Transplant Providers.

BACKGROUND: People with HIV (PWH) experience disparate access to solid organ transplantation (SOT) but are eligible to be organ recipients a … least influential factor (0.4%). Conversely, HIV status was the second most important attribute in donor selection (26.6%), behind only organ quality (32.0%). CONCLUSIONS: HIV status had limited influence on the choice of transplant recipients but was the second most influential factor in donor selection. Our results suggest that clinicians' decisions are guide …

Tackling the shortage of donor kidneys: how to use the best that we have.

Shortage of kidney donor is still a major limitation for renal transplantation programs. This review focuses on the emerging practices, adopted to increase transplant activities, of expanding the criteria for donor and recipient selection without exposing the recipient to the drawbacks of a graft with inadequate nephron mass. Expanding the donor pool inevitably led to consideration for kidney transplantation of organs from older donors or from donors with hypertension, diabetes or other renal diseases. To fit the reduced performance of these suboptimal organs with the renal requirement of the

The neurodivergent blood donor: Fairness, discrimination, screening, and behavior.

BACKGROUND: We broaden the ongoing drive to diversify blood donor panels by proposing and testing a framework to explore neurodivergence and blood donation. Drawing on a range of theoretical perspectives, we predict that neurodivergent people will be underrepresented in donor panels, donate less frequently, perceive donor selection as less fair and more discriminatory, be more likely to guess when answering screening questions, and find these questions upsetting. STUDY DESIGN AND METHODS: 4376 members of the UK general-public and 8668 NHSBT blood donors indicated if they had a formal or self-…

Which Donor and Recipient Risk Factors Matter in Heart Transplantation? Results From a Survey of 53 Centers Across Five Countries.

INTRODUCTION: Consensus regarding what defines acceptable heart transplant (HT) donors or recipients is lacking. This survey analyzed how risk factors guide donor and recipient selection, and how practices vary across systems. METHODS: An online survey was conducted among adult HT centers in the US and Eurotransplant (ET) region. We aimed to represent at least 50% of the total adult HT volumes in both regions. Centers were stratified by their HT volumes. To compensate for non-responders, a safety margin was included, and centers accounting for at least 75% of the total HT volumes were contact

Live-Donor Kidney Transplant Outcome Prediction (L-TOP) using artificial intelligence.

BACKGROUND: Outcome prediction for live-donor kidney transplantation improves clinical and patient decisions and donor selection. However, the currently used models are of limited discriminative or calibration power and there is a critical need to improve the selection process. We aimed to assess the value of various artificial intelligence (AI) algorithms to improve the risk stratification index. METHODS: We evaluated pre-transplant variables among 66 914 live-donor kidney transplants (performed between 1 December 2007 and 1 June 2021) from the United Network of Organ Sharing database, rando…

Which Infectious Blood Donors Could Be Identified by the Donor History Questionnaire? - Comparison of Blood Donors Infected with HIV or HCV with Notified Cases from General Population in Germany.

BACKGROUND: Potential risks for transfusion-transmissible infections are identified by donor history questionnaires (DHQs), and donors with higher risks are deferred from donation. We assessed to which extent the currently used DHQs support the identification of infections among blood donors. METHODS: Between 2006 and 2013, we analyzed data from notified HIV and HCV cases in the general population and positive blood donors in Germany. Logistic regressions were used to identify relevant infection risks. We estimated the possible effect of improved capture of risk factors for donor selection by…

A donor PD-1+CD8+ TSCM-like regulatory subset mobilized by G-CSF alleviates recipient acute graft-versus-host-disease.

Donor selection determines the occurrence of acute graft-versus-host-disease (aGVHD) following allogeneic hematopoietic stem cell transplantation (allo-HSCT). To optimize the current clinical donor selection criteria and identify putative donor lymphocyte subsets associated with better recipient outcomes, we analyzed the peripheral CD4+ and CD8+ subsets in 80 granulocyte colony-stimulating factor (G-CSF) mobilized donors and examined the aGVHD incidence of the corresponding 80 haploidentical and identical allo-HSCT recipients. The G-CSF-induced expansion of subsets varied among donors. We disc…

Establishment and validation of porcine embryo genomic selection system through SCNT technique.

BACKGROUND: Embryo genomic selection (EGS) refers to the technique of performing genotyping and genomic prediction at the preimplantation stage. When combined with in vitro embryo production method, EGS enhances selection intensity and offers the potential to prevent the birth of offspring carrying genetic defects or an undesired sex. Given that the somatic cell nuclear transfer (SCNT) technique can copy the genetic information from donor pigs into early embryos, this study aims to establish and validate a complete porcine EGS workflow by integrating SCNT with genomic selection methodology. The History and Challenges of Blood Donor Screening in China.

Since the establishment of People's Republic of China in 1949, the Chinese government has encountered several catastrophes related to transf … pathogen detection and serology tests. The year 1998 marked the most transformative change in blood donor selection and screening policies in China. Before 1998, paid donation was the predominant mode of blood donation. Donor screening and donor testing were conducted before donation, and only those who were eligible were allowed to donate. To ensure the safety o …

Split liver transplantation: Current developments.

In 1988, Rudolf Pichlmayr pioneered split liver transplantation (SLT), enabling the transplantation of one donor liver into two recipients - one pediatric and one adult patient. In the same year, Henri Bismuth and colleagues performed the first full right/full left split procedure with two adult recipients. Both splitting techniques were rapidly adopted within the transplant community. However, a SLT is technically demanding, may cause increased perioperative complications, and may potentially transform an excellent deceased donor organ into two marginal quality grafts. Thus, crucial evaluatio

The use of gametes from donors who have undergone expanded carrier screening: a guidance for UK practice.

Expanded carrier screening (ECS) is increasingly offered within assisted reproductive technologies (ART), particularly as a component of donor gamete selection. As genomic testing options rapidly evolve, the complexity of interpreting ECS results presents significant challenges for donor gamete recipients and the healthcare professionals supporting them. At the same time, regulatory frameworks and professional guidelines have not kept pace with these technological developments, resulting in gaps between clinical practice and formal guidance. This paper aims to summarise current practice, highl

Dechlorinating ability of TCE-fed microcosms with different electron donors.

The main objective of the work presented herein is to assess the effect of different electron donors (butyric acid and methanol) on the dechlorinating activity of two microbial cultures where active methanogenic populations are present, in an effort to evaluate the importance of the electron donor selection process. The ability of each anaerobic culture to dechlorinate TCE, when enriched with either butyric acid or methanol, was verified based on the results of gas chromatography. In addition, the fluorescent in situ hybridization (FISH) and the polymerase chain reaction (PCR) methods gave pos…

Current trends in cardiac transplantation.

More than 30 years of experience in cardiac transplantation have resulted in cardiac transplantation being the primary therapeutic choice for patients under 65 years of age who have advanced heart failure and who remain symptomatic despite maximal medical therapy. The success and widespread use of cardiac transplantation is attributed to more liberal guidelines in recipient and donor selection, effective immunosuppressive therapy, close monitoring for rejection, and effective management of rejection. This article presents the current status of recipient and donor selection, surgical techniques…

Genetic testing in living kidney donor candidates.

PURPOSE OF REVIEW: This review focuses on genetic evaluation in biologically related living kidney donor (LKD) candidates and candidates with suspected inherited disease, highlighting how genetic testing can extend beyond traditional clinical assessment to inform donor selection and counseling. RECENT FINDINGS: Genetic screening of LKDs can identify monogenic kidney diseases, enable exclusion of at-risk donors and support long-term donor health. Sequential testing that includes genetic confirmation in the intended recipient followed by targeted testing of related donors, has effectively exclu…

World marrow donor association crisis response, business continuity, and disaster recovery guidelines.

Multiple institutions, such as donor registries, donor centers, transplantation centers, collection centers, and courier companies, are involved in the international exchange of hematopoietic stem cells. The ability to safely and efficiently ensure continued operation of a donor registry relies on an organization's resiliency in the face of an incident that could impede donor search, donor selection, stem cell collection, or transportation. The Quality Assurance Working Group of the World Marrow Donor Association has developed guidelines on how to establish an organizational resiliency program…

Donor-recipient gender mismatch in lung transplantation: impact on obliterative bronchiolitis and survival.

BACKGROUND: Because of the shortage of donor lungs, liberalization of donor selection criteria in terms of age, gas exchange, and smoking history has been proposed. METHODS: We evaluated a single-institution population of lung transplant recipients (n = 98) for donor-recipient gender matching. We measured overall survival, time to acute allograft rejection, and time to development of obliterative bronchiolitis (OB). RESULTS: We found significant improvement in overall survival for gender-mismatched donor and recipient pairs (p = 0.078) and a significantly shorter OB-free period for male dono…
